Welcome to the Louisiana Pharmacists Association’s 136th Annual Convention and Trade Show in

Lafayette. We as the representatives for pharmacy in Louisiana are here to conquer as the heroes of

pharmacy and advance the profession of pharmacy to protect the health and well-being of our patients.

This year’s convention is an opportunity for you to connect with your colleagues and fellow

association members who share similar professional interests. We will address issues across the

profession—from regulations to opioid challenges, to diversity in patient care, to the every day changes

we see in medicine that affect our patients.

We hope you use this weekend to connect with your fellow heroes. Whether acquired during a

continuing education session or through a conversation with a peer, the information you will take from

this meeting is invaluable. We must use the information to strengthen our professional connections to

provide a stronger voice supporting and developing our profession. Please take the time during this

convention to establish new contacts, share thoughts and ideas, and continue paving the path for future

pharmacists.

Comparing 2nd Line Diabetes Drugs (Oak Alley room) Target Audience: Pharmacists and Pharmacy Technicians; Live Contact Hours: 1.5 UAN: 0133-0000-18-010-L01-P/T Faculty: Dr. Emily Weidman-Evans, PharmD, Associate Professor, LSUHSC-S School of Allied Health Pharmacist Objectives: Identify the appropriate use of Metformin, including dosing, contraindications, and monitoring; Compare and contrast the second-line drugs for type 2 diabetes mellitus (per ADA and AACE clinical algorithms) in relation to their adverse effects, contraindications, and monitoring; Assess when an "add-on" drug might be indicated in a patient with type 2 diabetes mellitus and what factors would be considered when selecting one; Educate patients or providers about appropriate therapy when metformin is not optimal, based on patient- and drug-specific factors. CONTINUING EDUCATION CREDIT - The Louisiana Pharmacists Association is accredited by the

Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Fees

for all continuing education activities are included in the convention registration fee. Successful

completion of continuing education activities entail signing in on the sign-in sheet and completing

the Credit Report Form and Evaluation. LPA shall submit all continuing education activities to CPE

Carl Aron, RPh

Louisiana Board of Pharmacy

Carl Aron is the current President of the Louisiana Board of Pharmacy, a board he has served on since 1971. He is the owner and manager of Aron’s Pharmacy in Monroe, Louisiana. In his professional career, Aron has been active in the local, state, and national

pharmacy associations, including the National Association of Boards of Pharmacy, where he has served on several committees.

Aron served in the U.S. Army as a Captain of the Medical Service Corp before beginning his career in pharmacy. He is a graduate of the University of Mississippi and holds a license in Louisiana, Mississippi, and Georgia.

Dr. Nancy Balch is the senior attending pharmacist and the Emergency Department pharmacist at Massachusetts General Hospital in Boston. She has an extensive background in the health-system pharmacy practice setting, as well as

community pharmacy. She has instructed many courses related to emergency medications and herbal supplements.

Dr. Balch currently serves as a volunteer for both the NorthEast Animal Shelter and the Lead Volunteer Pharmacist for Pharmacy Team of Medical Crew for the Boston Avon Breast Cancer 2-Day Walk. She earned her Doctor of Pharmacy from Northeastern University in Boston and has previously served in the U.S. Army.

KEYNOTE SPEAKER

Congressman Ralph Abraham, M.D. United States House of Representatives

Congressman Ralph Abraham, M.D., was first elected to represent Louisiana's 5th District in December 2014 and reelected to a second term in November 2016. The 5th District is the largest in terms of area in the state, spanning 24 parishes across most of Northeast and Central Louisiana and parts of five Florida parishes in the state's southern "boot." This is his first publicly held office after spending most of his life as a practicing physician, veterinarian, business owner, farmer, pilot and veteran.

Dr. Abraham is a member of the House Armed Services Committee, a post he will use to advocate for America's men and women who serve in our military. Dr. Abraham was a First Lieutenant with the 20th SFG Airborne Division of the Army National Guard from 1986-1989. Additionally, he is a fixed-wing and helicopter pilot who currently flies reconnaissance missions for the Coast Guard Auxiliary, and he is a mission pilot for the Air Force's Civil Air Patrol Green Flag Program.

Dr. Abraham also serves on the Agriculture Committee. The 5th District is one of the largest row crop districts in the nation, and many of the district’s farmers raise livestock. Through his lifetime of working on his farm, he understands firsthand the hard work that America’s farmers put in daily. Dr. Abraham also sits on a third committee, the House Committee on Science, Space and Technology. He is the chairman of the Subcommittee on Oversight. His legislative priorities for this committee will focus on enhancing America's cybersecurity capabilities.

Dr. Abraham grew up in rural Richland Parish where he and his wife, Dianne, reside today. Dr. Abraham graduated from Mangham High School before earning his degree at Louisiana State University. He graduated from the LSU School of Veterinary Medicine and practiced for 10 years before eventually seeking his Medical Doctor degree, which he earned at the LSU School of Medicine in Shreveport. As Dr. Abraham likes to say, he wanted to be able to “treat anything that walks on two or four legs.”

Elizabeth Harris, PharmD, BCACP

Dr. Elizabeth Harris, PharmD, BCACP, earned her Bachelors and PharmD at the University of Pittsburgh and completed a PGY-1 Community Practice Residency at the University of

Pittsburgh School of Pharmacy in cooperation with a regional grocery store chain pharmacy. She is now an Assistant Professor at the Philadelphia College of Pharmacy where she teaches self-care and disease prevention, clinical skills, and medication therapy management (MTM). She provides MTM and Transitions of Care (TOC) services to patients within an independent pharmacy in Northeast Philadelphia and formerly practiced in a Federally Qualified Health Center (FQHC) in West Philadelphia.

Stephen Hill, PharmD

University of Louisiana at Monroe College of Pharmacy

Dr. Stephen Hill, PharmD, was born and raised in Monroe, and graduated from Neville High School in 2001. He completed his Pre-pharmacy requirements at LSU in Baton Rouge from 2001-2004. He then attended

ULM School of Pharmacy starting in 2004 and graduated in 2008. Dr. Hill completed a PGY-1 Pharmacy Practice Residency at the University of Mississippi Medical Center in Jackson, MS from 2008-2009.

Dr. Hill started as an Inpatient clinical pharmacist at Methodist Dallas Medical Center in Dallas, TX from 2009- until July 2014 (5 years). He moved back to his hometown and accepted a clinical assistant professor position with ULM on August 1, 2014. His clinical practice site is at University Health-Conway Family Medicine Clinic where he precepts P4 pharmacy students on an APPE AM Care rotation.

Ricky Indovina, Jr., RPh

Ricky Indovina currently serves as the Vice President of Specialty and Retail Pharmacy with the Ochsner Health System in Louisiana. In this position, he develops the strategic vision and long term planning and direction for system-wide retail and specialty pharmacy

functions. He has over 25 years of experience in optimizing operations and developing innovative business strategies. Prior to his current position, he has worked in multiple areas

of the Ochsner Health System and with Walgreens Pharmacy.

Indovina earned his pharmacy degree from Xavier University, as well as his MBA with concentration in healthcare management from the University of New Orleans. He is also currently a member of the Louisiana Board of Pharmacy.

Randal Johnson

Louisiana Independent Pharmacies Association

As the landscape of the healthcare continuum across the state and nation undergoes constant change, Randal Johnson is a prominent expert in healthcare policy impacting individuals,

providers, and businesses of various sizes. Knowledgeable on matters at both state and federal levels, his management of healthcare issues through legislative and regulatory action has led to the retention and creation of thousands of jobs in Louisiana. Having served as Deputy Commissioner of the Louisiana Department of Agriculture for fourteen years,

Johnson handled the Department’s legislative package for more than twenty sessions. With political activities ranging across both party and geographical lines, he has been recognized by Campaigns & Elections’ Politics magazine as one of the top 100 “Louisiana Influencers.”

Johnson’s work is especially apparent during times of emergency situations. During hurricanes Katrina, Rita, Gustav, and Ike, he commanded the emergency power and fuel needs for all uses, including each of the licensed healthcare facilities in Louisiana – a position he continues to hold today in the event of another state declared emergency situation. By his efforts, facilities were able to maintain their life-sustaining needs through the evacuations, emergency response, and recovery efforts.

With political activities ranging across both party and geographical lines, Johnson, who holds a B.S. in business administration, has consulted and worked in various local, state, and federal campaigns in his native state of Louisiana. Johnson has also coordinated successful statewide get-out-the-vote efforts for elected offices ranging from Police Juror to President of the United States. In addition, he graduated from regional and national law enforcement academies administered by the Department of Justice.

Ruth Kennedy

Southern Strategy Group of Louisiana

Ruth Kennedy is currently the Health Policy Director for Southern Strategy Group of Louisiana. She previously worked in multiple positions within the Louisiana state government health

services administration as Medicaid Director (2012-2016), Medicaid Deputy Director (1999-2012) and LaCHIP Director and Medicaid Expansion Project Director. Her accomplishments include planning, implementing and ongoing operations for LaCHIP and children’s Medicaid, which resulted in lowering Louisiana’s rate of uninsured children from 3rd highest of any state to well below the national average at under 5%.

Before serving as Louisiana’s Medicaid Director, Kennedy led Louisiana’s transformational initiative to transition from fee-for-service to a managed care delivery system. During the final year of her career with the state, she worked exclusively on directing Louisiana’s highly successful and innovative implementation of Medicaid Expansion, which launched July 2016 with more than 375,000 adults successfully enrolled in the first year.

She is nationally recognized for expertise in streamlining and simplifying business processes, maximizing retention, culture change, health care delivery system reform and health care administration. A graduate of Southeastern Louisiana University, Kennedy resides in Hammond, Louisiana.

Kevin LaGrange, RPh

Professional Arts Pharmacy

Kevin LaGrange is a graduate of the University of Southwestern Louisiana and Northeast Louisiana School of Pharmacy at Northeast Louisiana University. He is currently a compounding pharmacist and the QA-QC Director at Professional Arts Pharmacy where he oversees the

pharmacy operations to assure pharmacy is in compliance with all standards established by the “Pharmacy Compounding Accreditation Board.” He has previous experience in the hospital and clinical pharmacy settings.

LaGrange was named Compounding Pharmacist of the Year by LPA in 2014 and is actively involved in multiple industry and community organizations.

Louisiana House of Representatives

Representative Bernard LeBas represents District 38 encompassing St. Landry and Evangeline parishes. He is active on several House Committees, including Health and Welfare. Currently, LeBas is a practicing pharmacist and owner of

Evangeline Drug Store in Ville Platte, Glenn’s Family Pharmacy in Opelousas, and Fuselier’s Pharmacy in Pine Prairie. He is a graduate of University of Louisiana at Monroe School of Pharmacy, and remains active on the ULM School of Pharmacy Advisory Board.

Representative LeBas has contributed much to the profession of pharmacy by founding the Cajun Area Pharmacist Association, serving on the LA Board of Pharmacy, as well as serving in many different capacities on the LPA Board of Directors, including President. He comes from a family of pharmacists and a family involved in politics – his father and brother were both pharmacists and Mayors of Ville Platte. He is the father of five, and two of his sons and one daughter-in-law are carrying on the tradition as pharmacists.

Joseph “Gary” LeBlanc, Jr., PharmD, MHA, MBA

Heart Hospital of Lafayette

Gary LeBlanc attended pharmacy school at the University of Louisiana at Monroe College of Pharmacy where he obtained is Doctor of Pharmacy degree in 2010. He received his Master of Health Administration Degree from Simmons

College in 2014 and his Master Business Administration from New England College in 2016.

LeBlanc has seven years experience as a licensed pharmacist, four of which has been spent in hospital pharmacy management. He has served as a sterile compounding clean room consultant for independent, specialty pharmacies and as a consultant on director of pharmacy advisory boards for pharmaceutical companies. He currently serves as director of pharmacy at Heart Hospital of Lafayette and as a licensed staff pharmacy for Wal-Mart Pharmacy.

LeBlanc also serves as President-Elect of the Louisiana Society of Health-System Pharmacists and as a Louisiana delegate on the American Society of Health-System Pharmacists House of Delegates. He is the 2012 recipient of the Louisiana Pharmacist Association Distinguished Young Pharmacist of the Year Award, the 2014 recipient of the Louisiana Society of Health-System Pharmacists Outstanding Affiliate Chapter President Award, and the 2015 recipient of the Louisiana Society of Health-System Pharmacists Health-System Pharmacist of the Year Award.

Charrell Porter, PharmD

Dr. Charrell S. Porter, PharmD, BCACP is Assistant Professor of Clinical Pharmacy at University of the Sciences Philadelphia College of Pharmacy, located in Philadelphia. She earned her Doctor of

Pharmacy degree at Virginia Commonwealth University in Richmond, Va., prior to completing a first year pharmacy practice residency at Sibley Memorial Hospital- Johns Hopkins Medicine in Washington, DC. She then went on to complete a second year specialty residency in ambulatory care at Temple University School of Pharmacy.

Dr. Porter’s practice interests include medication management of chronic diseases, transitions of care, and barriers to medication adherence. As a Board Certified Ambulatory Care Pharmacist she has worked with physicians and nurses in West Philadelphia to create a patient-centered medical home model.

Her research interests include interprofessional education and the impact of pharmacist-provided medication and therapy management on clinical outcomes. At Philadelphia College of Pharmacy, Dr. Porter teaches in a variety of course topics including those related to diabetes, community practice, clinical skills, and physical assessments. She also serves as a preceptor to pharmacy students on rotations.

Carleigh Roberts, PharmD Candidate

Carleigh Roberts is an incoming P3 student at the University of Louisiana-Monroe College of Pharmacy. She grew up

in Utica, MS and has been living in Monroe for five years now. Roberts has a passion for fighting the opioid epidemic and educating the community about addiction. This passion developed after losing a close friend to addiction. She is a current APhA-ASP at ULM Generation Rx co-chair. This is her second year in this position.

Courtney Robertson, PharmD

Dr. Courtney Robertson, PharmD began her career as a faculty member for the ULM School of Pharmacy in 2013. After

Connie Smith, PharmD

Associate Professor, Director of Experiential Education, University of Louisiana at Monroe School of Pharmacy

As director of experiential education, Dr. Connie Smith’s primary focus is to plan and direct the College of Pharmacy’s

experiential training program. Her administrative responsibilities account for 75% of her job and is responsible for managing the Office of Experiential Education and overseeing the day-to-day activities of over 25% of the curriculum. She oversees over 300 adjunct and full-time faculty preceptors who provide experiential training opportunities for our Pharm.D. students. Recruitment and retention of these valuable faculty preceptors is crucial to the success of ULM’s program.

In addition to the responsibilities of the Office of Experiential Education, Dr. Smith also supervises the Coordinator of Introductory Pharmacy Practice Experiences, the Pharmacy Care Lab team, and support staff in the accomplishment of these duties. She works in a collaborative manner with school faculty as well as area preceptors and health care organizations to achieve the essential duties and responsibilities of the position.

Roxie Stewart, PharmD

Clinical Associate Professor, University of Louisiana at Monroe College of Pharmacy

Dr. Roxie Stewart, PharmD, is a Clinical Associate Professor at the University of Louisiana at Monroe College of Pharmacy, as well as a Coordinator for Introductory Pharmacy Practice

Experiences at the university. Her professional background includes serving as a pharmacist in the health-systems practice areas and being a certified faculty trainer for both the APhA Immunization Certificate Program and the AHA Basic Life Support for the Health Care Provider.

Dr. Stewart has served as President of the Louisiana Society of Health-System Pharmacists and the Fifth District Pharmacists Association. Her research interests include experiential education, disease prevention, interprofessional education, and student assessment.

Emily Weidman-Evans, PharmD Louisiana State University Health Shreveport

Dr. Emily Weidman-Evans is a 2002 graduate of the University of Florida, and completed a residency focusing on Community Pharmacy

at Texas Tech University Health Sciences Center in 2003. She has been a faculty member at South University in Savannah, GA, and at University of Louisiana at Monroe, and during that time worked in retail and in a family medicine clinic.

Dr. Weidman-Evans now is on faculty part-time at the LSU Physician Assistant Program, as well as the University Health Department of Family Medicine, where she sees patients to help them with their medication therapy management.

She is married to Dr. Jeff Evans and mommy to two beautiful little girls, and in her (limited) free time, she likes to swim, bike, and run…in that order.

Dr. LaKeisha Williams, PharmD, MSPH

Dr. LaKeisha Williams is a Clinical Assistant Professor in the Division of

Clinical and Administrative Sciences at Xavier University College of Pharmacy. She received her Master of Science in Public Health from Tulane University and a Doctor of Pharmacy from Xavier University of Louisiana. Following her graduate studies, she completed an American Society of Health-System Pharmacists accredited Pharmacy Practice residency.

Dr. Williams is passionate about optimizing patient care through effective therapeutic management. Her professional and research interests include medication safety, health disparities and public health, cardiovascular disease and medication adherence, interprofessional education and mentorship within the profession. She currently serves as a preceptor for third and fourth year pharmacy students in an ambulatory care setting, incorporating drug information and chronic disease state management. She previously served as a Drug Information Specialist at the Xavier University Drug Information Center Satellite, and she currently maintains a practice site at the Interim LSU Hospital Ambulatory Care Clinics, focusing on diabetes, hypertension and weight management.

LPA Anti-Trust Policy During all convention activities, the Louisiana Pharmacists Association would like to remind all attendees to please comply with the association’s antitrust policy.

It is the policy of the Louisiana Pharmacists Association (LPA) to be in strict compliance with all Federal and State Antitrust laws, rules and regulations. This policy and procedures apply to all membership, board, committee and other meetings of LPA, and all meetings attended by representatives of the Association. Therefore:

1. Discussions of prices or price levels is prohibited. In addition, no discussion is permitted of any elements of a company's operations which might influence price such as:

a. Cost of operations, supplies, labor or services; b. Allowance for discounts; c. Terms of sale including credit arrangements; and, d. Profit margins and mark ups, provided this limitation shall not extend to discussions of methods of operation, maintenance, and similar matters in which cost or efficiency is merely incidental.

2. It is a violation of Antitrust laws to agree not to compete, therefore, discussions of division of territories or customers or limitations on the nature of business carried on or products sold are not permitted.

3. Boycotts in any form are unlawful. Discussion relating to boycotts is prohibited, including discussions about blacklisting or unfavorable reports about particular companies including their financial situation.

4. All meetings attended by representatives of LPA where discussion can border on an area of antitrust sensitivity, the LPA representative will request that the discussion be stopped and ask that the request be made a part of the minutes of the meeting being attended. If others continue such discussion, the LPA representative will excuse himself from the meeting and request that the minutes show that he left the meeting at that point and why he left. Any such instances should be reported immediately to the President and staff of LPA.

5. A copy of these Antitrust Compliance Policies and Procedures is to be given to each officer, director, committee member, official representative of member companies and LPA employees annually and that the same be read, or understood at all meetings of the membership of LPA.
