Foreign body in airway
• ไอส าลก (choking, gagging)
• เสยงแหบ (hoarseness)
• ทางเดนหายใจอดกน (airway obstruction)
• ตวเขยว (cyanosis)
• อาจมไอเปนเลอด หรอ มาดวยภาวะปอดอกเสบ
Foreign body in airway
Radiologic evaluation: when Airway stable
- lateral neck (soft tissue technique): opaque, non-opaque
- CXR-PA (inspiration & expiration): unilateral hyperaeration, air trapping
uncooperative child:
lateral decubitus lung Rt. and Lt. to compare
- dependent lung remain inflated in bronchial obstruction (dependent lung should collapse in normally)
Management Foreign body in airway : Complete airway obstruction : airway distress, cyanosis, unconscious - age<1year: back blows-chest thrust - age>1year: abdominal thrust while supine - older children and adults: Heimlich maneuver Not recommended in partial obstruction ‘cause it will turn to be
completely obstruction !!! Incomplete (partial) airway obstruction : cough, gagging, conscious
Foreign body
• Emergency condition:
Severe airway obstruction large FB obstruct at glottis or subglottis
large FB at upper part esophagous with posterior compression of airway
Disc battery (corrosive injury: base)

FB that absorb water : Nuts mucosal reaction, edema, UAO

• suspected in children who present with choking, refusal to eat, vomiting, drooling, may be UAO (posterior compression airway if large FB)

Deep Neck Infection Evaluation of patient
• History sorethroat, odynophagia ,dysphagia
• PE. Evaluate airways first ( UAO, stridor?), V/S (high fever, sepsis)
Affected site: Intraoral bulging (anterior pillar, tonsillar area, lateral or posterior pharyngeal wall),
trismus,
palpate neck (swelling, tender)
Deep Neck Infection
Management - secure airway first : ETT +/- tracheostomy
- fluid resuscitation
- antibiotic cover Streptococci & anaerobe (เชอในปาก)
If immunocompromise ATB cover gram neg.bacilli & Staph.
- surgical drainage indication: deep abscess, no response to antibiotic within 24-48hrs., sepsis, mediastinitis, neuro.deficit

Deep Neck Infection
Ludwig’s angina Definition: rapid progressive cellulitis, not abscess direct extension, involve both sublingual and
submaxilllary space, bilaterally (abscess form called --> pseudo-Ludwig’s angina) 70% dental source ( immunocompromise host)
Ludwig’s angina Clinical: trismus, floor of mouth massive swelling, tongue and base of tongue was push
superior and posterior cause --> upper airways obstruction progress within 12-24hrs.
Px: airway manage (intubation or tracheostomy) Resuscitation IV antibiotic Indication surgical drainage: UAO, turn to abscess, fail ATB use
Deep Neck Infection
Retropharyngeal space abscess Clinical: (child) fever, poor appetite, neck rigidity-swelling-tenderness,
muffled cry(hot-potato voice), drooling, resp.compromise
(adult) fever, sorethroat, odynophagia ( +/- Hx FB )
PE: abscess form lateral to midline of posterior pharyngeal wall,
tilt head to uninvolved side(child)
Film: lateral neck (soft tissue technique)
Retropharyngeal space abscess
Px: secure airway fluid resuscitation ATB cover Strep.& anaerobe consider surgical drainage
Laryngeal injury
Blunt laryngeal injury :
Assessment ---> 1.concern C-spine injury (lateral neck film)
2.airway compromise : tracheostomy prefer
3.neuro.deficit
Blunt laryngeal injury :
Signs and symptoms ---> voice change (hoarseness to aphonia), UAO,
subcutaneous emphysema, hemoptysis, dysphagia,
hematoma on neck, loss of thyroid or cricoid prominence
เนองอกกลองเสยง (laryngeal tumor)
• Benign (eg.papilloma, hemangioma)
• Malignant (heavy smoking, hoarseness, stridor)
Upper airway obstruction (UAO) Management: Immediate Px: Aim --> bypass obstructive area Aware vital signs (sepsis) , C-spine injury conservative Px: oxygen,+/- steroid, antibiotic endotracheal intubation Tracheostomy Specific Px : vary as cause of UAO
Indication: • Upper airway obstruction eg. CA laynx, subglottic stenosis, Ludwig’s angina with airway obstruction, severe OSA
• Prolonged respiratory support • Pulmonary hygiene eg. bedridden with chronic lung disease • Prevent aspiration to lower airway eg. Patients with severe
maxillofacial trauma who cannot intubation

พฝาก กอนconsult เจาะคอ (tracheostomy) 1. Diagnosis (underlying)+ Indication
2. V/S, anterior neck exam: mass?, trachea in midline?, pulsatile?
3. Lab: CBC, coagulogram, anti-HIV, CXR , อนๆท abn.significant
4. Culture (H/C, MUC, sputum) ถาม : ดเชอดอยา MDR เชน MRSA, Pseudo.,
acinetobacter
5. Anticoagulant drug eg. ASA, Plavix ควรoff 7 วนกอน 6. please do not extubate before tracheostomy
พฝาก กอนD/C คนไขเจาะคอ 1. deflate cuff if no ventilator need
2. stitches off silk at Sx site
3. change shiley’s tube to silver tube if no ventilator need 4. home care : suction, dressing trach. wound
Postoperative complications • Hemorrhage : from Sx site or bloody sputum or erosion of
innominate artery (sentinel bleed) • Pneumothorax, subcutaneous emphysema

Nose and PNS disease
เลอดก าเดาไหล (Epistaxis)
ประเมนความรนแรงของภาวะเลอดออก
• Condition ผ ปวย : รสกตว, ซมลง
• Vital signs : stable or not
• ENT exam, Systemic exam
• Ix: พจารณาเปนกรณ ตามสาเหตทสงสย CBC, platelet count
Coagulogram LFT, Bun, creatinine film PNS, CT/MRI, angiogram
Causes of epistaxis
Local Systemic
1. Trauma :
- direct - maxillofacial trauma
1. Hypertension
2. Infection and inflammation :
- rhinosinusitis
2. Bleeding disorder : leukemia,
ITP, coagulopathy, hereditary hemorrhagic telangiectasia
3. Neoplasm :
- Angiofibroma
- CA nasopharynx - CA nose&PNS
3. Systemic disease eg. Liver cirrhosis
Severe epistaxis (vital signs ไม stable, massive bleeding)
ใหoxygen รวมกบให 0.9%NSS 1000ml load 100-200ml จากนนวดความดนโลหตซ า
Monitor BP, pulse, oxygen saturation
สงเลอดตรวจ CBC, coagulogram, cross match PRC (+/- LFT, BUN,
Creatinine )
เตรยมอปกรณ packingใหพรอม Stop bleeding : anterior +/- posterior
nasal packing

ไมหยด bleed พจารณา angiogram – embolization or surgery
(arterial ligation)
อปกรณทใช Nasal speculum (ทถางจมก)

Moderate epistaxis (หามเลอดดวยวธเบองตน เลอดไมหยด)
- วด BP,pulse ทก 3-5นาท
- ใช local vasoconstrictor eg. Ephedine, dilute adrenaline pack จมก
- anterior nasal packing with vaseline gauze, gelfoam
- nasal balloon packing
ประเมน blood loss หาสาเหตและรกษา ในรายสงสยวาเกดจาก bleeding disorder ควร pack with gelfoam
• กอนใส nasal packing ผ ปวยควรไดรบการอธบายถงความจ าเปนในการใส ความปวด และความไมสบายในการหายใจ (หายใจทางปาก) และกนอาหารล าบาก และควรเตรยมอปกรณใหพรอม
• ขณะใส nasal packing ควรสงเกตการหายใจของผ ปวย รวมไปกบการให cold pack,
ยาแกปวดและantibiotic, นอนหวสง ,mouth care
• หลงเอา nasal packing ออก ควรแนะน าผ ปวย ดงน
- อยาแคะ แกะ เกา ขยจมก หรอ งดสงน ามกแรงๆ ประมาณ 2 สปดาห - งดบหร เหลา เพราะท าใหเยอบโพรงจมกแหงและหายชา - อยายกของหนกหรอออกก าลงกายหนกๆ 2 สปดาห - นอนศรษะสง - ถาจะจามหรอไอ อยาลมอาปาก!
Mild epistaxis สวนใหญออกจากผนงกนจมกดานหนา จากการแคะจมกหรอไหลออกเอง การหามเลอดเบองตน - ใหผ ปวยกมหนาอาปากหายใจ ใชนวชและนวหวแมมอบบจมกใหแนนประมาณ10-15นาท - cold pack ทบรเวณดงจมกและหนาผาก

Clinical signs and symptoms of ear diseases
• ปวดห (otalgia)
• มน าไหลออกจากห (เลอด, หนอง, น าใส) (discharge)
• หออ (hearing loss)
• เสยงในห (tinnitus)
• เวยนศรษะ (vertigo)
• สงแปลกปลอมในห (foreign body)
• การบาดเจบทห (trauma)
External ear disease
First exam.: Better ear ( compare with disease ear)
Perichondritis of ear pinna
secondary to trauma laceration or hematoma or infection Pain, tender, swelling, redness of ear pinna (spare ear lobule) Px: antipseudomonas antibiotic, aspiration+/- I&D, pressure dressing Complication: cartilage ischemiainfectionCauliflower ear

Immunocompromise host eg. DM, post chemo., SLE Severe otalgia, otorrhea, facial nerve palsy Px: treat underlying disease(esp.DM) , prolonged antipseudomonas antibiotic 6weeks surgical debridement

• vegetable eg. เมลดถว เมลดผลไม(สม) จะพองตวถาโดนน า remove ยากขน • non-vegetable eg. ลกปด ยางลบ เมดพลาสตก • แมลง ควรท าใหตายกอน หยอดหดวยยาทเปนน ามน เชน chloram ear drop, Dewax หรอใช olive oil

Plan: หลกเลยงไมใหน าเขาห และผาตดปะแกวห (tympanoplasty)

Eustachian obstruction: adenoid hypertrophy, allergy, CA nasopharynx, post H&N
radiation หรอ เกดตามหลงโรคหชนกลางอกเสบเฉยบพลนทหายไมสนท หออ ไมปวด เสยงดงในหเวลากลน เดกอาจพดไดชา หรอเรยกไมไดยน Px: valsava, decongestant, ATB, +/- myringotomy +ventilation tube

Px: หลกเลยงน าเขาห หามปนห analgesic drug , ไมตองใหยาหยอดห สวนใหญหายไดเองภายใน 1 - 3 เดอน

ปรบความดนในทอeustachian : กลน,หาว,ท า valsava, decongestant ในคนทเปนหวด งดด าน าถาclearหไมได ควรสงเกตอาการเวยนศรษะและหดบ อาจมการรวของน าในหชนในตองรบรกษา

maybe viral infection (HSV or VZV)
microcirculation failure or ischemic neuropathy
autoimmune reactions
Px: corticosteroid high dose (1mg/kg/day) if no contraindication
antiviral controversy
Prognosis: full recovery ~ 80-90%
Herpes Zoster Oticus (Ramsay-Hunt syndrome)
severe otalgia with group of vesicles at pinna and EAC (Herpes
Zoster) + sensorineural hearing loss, vertigo + acute facial nerve palsy(LMN type)
Etiology: VZV
Px: antiviral(early)
corticosteroid(early,maximal dose),
analgesics
EYE-CARE
Prognosis: แปรตาม duration of Px and severity
most full recover ~20%
Inner ear diseases
• sensorineural hearing loss
• vertigo (BPPV, Menierre’s)

การสญเสยการไดยนอยางฉบพลน (Sudden sensorineural hearing loss)
หออทนท most ภายในสามวน normal ears exam – no neuro.deficit , Rinne & Weber test : SNHL, confirm with
audiogram
อาจมเสยงในหหรอเวยนศรษะรวมดวย โดยมากไมทราบสาเหต เชอวาเกดจาก 1. Viral infection (post-URI) 2. Cardiovascular cause
3. Perilymphatic fistula จากการไอ จาม เบง ท าvalsava หรอด าน า Ix: CBC, FBS, Chol., TG, Bun, Cr. ,ESR, uric acid, VDRL, anti-HIV (+/- TFT)
Px: steroid (1mg/kg/day) if no contraindication
Early Dx , early Px , increase chance of recovery

Peripheral Vs Central
• Ask for associated symptoms i.e. discharge, tinnitus, aural fullness and hearing loss
• Ask for focal neurological complaints i.e. diplopia, dysphagia, dysarthria, paresis, paresthesia ,imbalance .
Inner ear disorders should never be associated
with a loss of consciousness

อาการรวม คลนไสอาเจยน หออ เสยงในห อาจมเดนโคลงเคลงชวงมอาการ
ปวดศรษะ เหงอแตก ใจสน ตาพรามว เจบหนาอก หนามด จะเปนลม เดนเซไปขางเดยว
อาการทางระบบประสาท

• Balance Tests : Romberg’s and tandem gait tests (both eyes open and closed).

การรกษา peripheral vertigo
1. รกษาตามอาการ เปนการรกษาเรมตน
• ใหยากลมกดการรบรทางการทรงตว (vestibular suppressants)
• ยาแกอาเจยน antiemetic เชน motilium, plasil
• ยากลม H3 receptor antagonist eg. betahistine
• ยากลม antianxiety eg. diazepam, antidepressant eg. Amitryp,
Nortrip. จะใหในกรณทอาการวตกกงวลหรอซมเศราเปนสาเหตกระตนใหเกดอาการเวยนศรษะ
2. รกษาตามสาเหตของโรค
ยากลมกดการรบรทางการทรงตว (vestibular suppressants)
• ยากลม H1 receptor antagonist eg. Dramamine เพอบรรเทาอาการเวยนศรษะ คลนไสอาเจยนในชวงแรก หรอกลม calcium
channel inhibitor eg. Cinnarizine, flunarizine เพอลดการกระตนระบบการทรงตว
• ยากลมนเมอพนระยะเวยนศรษะรนแรง แลวตองหยดยากลมนใหเรว เพอใหกลไกการปรบตวเกดขน (central compensation) ถาปรบไมไดจะเวยนศรษะเรอรง
ยากลม H3 receptor antagonist eg. betahistine
local vasodilation dilate blood vessels within inner ear (H1 agonist effect) increase blood flow to labyrinth and brainstem

Benign paroxysmal positional vertigo (BPPV)
• เชอวาตะกอนหนปนซงเปน calcium ในหชนในสวนการทรงตวเคลอนท
• เวยนศรษะบานหมน relate change
head position เชน ลมตวลงนอน เงยหรอแหงนหนา
• มอาการบานหมนชวงสนๆเปนวนาท
• การทดสอบ Hallpike maneuver ด nystagmus (vestibulo-ocular reflex)
Meniere’s disease
• Endolymphatic hydrops
• ควรจะมอาการรวมกนคอ หออ เสยงในห เวยนศรษะบานหมนเปนนาท
• audiogram: fluctuate sensorineural hearing loss

การรกษา
• ขณะมอาการใหหลกเลยงทาทางทจะกระตนใหเกดอาการ
• รกษา acute symptoms • ใหยากลมกดการรบรทางการทรงตว

• ยาแกอาเจยน antiemetic เชน motilium, plasil
• canal repositioning vertigo

การรกษา

• ยาบรรเทาอาการเวยนศรษะ กนคลนไสอาเจยน
• ยาขยายหลอดเลอดในหชนใน
• พกผอน ลดความเครยด
• CATS avoidance : caffeine, alcohol, tea, stress
