Link-Up aims to reduce loneliness, isolation and depression amongst vulnerable older people by supporting them to attend Open Age activities. We aim to help those most at risk of isolation by providing:
1 to 1 support & encouragement (including home visits) to help people attend activities at their own pace
Accompaniment to first session/sessions
Support to identify interests & matching suitable activities to individual needs
Support finding alternative transport arrangements if public transport is unsuitable or inadequate
Practical problem solving to reduce & remove barriers to Open Age opportunities
Friendly and patient advice about older people’s services in the area.
By engaging people to access physical, creative & mentally stimulating activities Link-Up can help prevent some of the negative consequences of social exclusion. The service helps improve the health and wellbeing of older residents both at home and in the community increasing independence and reducing demand on secondary Health & Social Services
