One theory is that the Tartars (people from an undefined region between the Sea of Japan and Dnieper, known as Tatary) attacked an Genoese trading outpost (Kaffa) on the coast.

The Genoese people were walled in their city and they were starving. The Tartars suddenly started dying in large numbers, but before they retreated, they used catapults to hurl dead bodies over the town walls, spreading the Black Death among the people.

The Genoese people panicked and fled in ships, landing in Messina in northern Italy and bringing the plague to Europe. By the time they reached Messina, most of the people onboard were already dead. Once the authorities in Messina realized that the ships had brought a terrible disease, they ordered them out of the harbour, but it was already too late.
