To enroll in the LSP program for 2018, follow these steps:

1. Please contact Superior Watershed Partnership to request an application for the program 906-

273-2742 x16 or go to http://superiorwatersheds.org/energy-conservation to print out an appli-

cation from our website.

2. Provide proof of a valid identification and a copy of the Social Security card for the account hold-

er and social security numbers and birthdates for all other individuals in the household.

3. Provide proof of income of all individuals living in your household from the last 30 days.

In order to qualify the federal and state eligibility criteria are:

Income must be equal to or less than 150 percent of the Federal Poverty Level (FPL). See chart below.

You must not have used more than $2,150 in natural gas for the year

Must have an active service and no unauthorized usage.

Must have past due charges on your account at the time of application.

Arrears must not exceed $4,000

Cap on arrears is $4,000

Your monthly payment will be determined by your Federal Poverty

Level (FPL) and your energy consumption history. If approved, your

payment will be $55 or $75 per month. Your rate will determined at

the time that a decision is made on your application.
