For every English polo player, and indeed for the greatest names in the sport worldwide, the Coronation Cup – the feature match on Cartier International Day at Guards Polo Club – remains the most coveted event to play in during the English polo season. This is the day when the polo fields of English summer enjoy full bloom, infused with a spice of glamour, celebrity and royal patronage.

The Dubai Duty Free Shergar Cup, on the 6th August is Britain’s premier international jockeys’ competition, where teams of riders from all over the world compete at Ascot over six races for the coveted trophy. The teams are Great Britain, Ireland, Europe and Rest of the World.

Now in its 20th anniversary year, GuilFest is an independently run festival with a fun, family-friendly atmosphere, held in the beautiful surroundings of Stoke Park, Guildford over the weekend of the 15th, 16th, and 17th July 2011.

BBC One’s ever popular Sunday evening programme, The Antiques Roadshow will be filming for its 34th series at the All England Lawn Tennis Club, Wimbledon, on Thursday, 25th August, 2011. The doors open at 9.30 am and close at 4.30pm. Entry to the show is free.

The London-Surrey Cycle Classic will bring the best athletes from around the world to the streets of London and the challenging terrain of Surrey on Sunday

14th August 2011. Part of the London Prepares series, the event will allow the London 2012 Organising Committee to test crucial aspects of its operations, less than one year ahead of the Olympic and Paralympic Games.

As one of only three International Cycling Union (UCI) road events taking place in the UK in 2011, the race will showcase the spectacular sport of cycling to London and Surrey-based fans. The London – Surrey Cycle Classic is a free event and spectators are welcome to watch anywhere along the 140km route. The route takes competitors past thousands of households, enabling fans to see world-class cycling action from their front door.

The race will start at The Mall at 9am, with riders setting off for a gruelling 140km race around the streets, lanes and hills of this challenging course. After weaving their way through the capital to the historic Hampton Court Palace, the riders will head

out into the rolling countryside, ancient woodland and picturesque villages of Surrey. Two zigzag circuits around Box Hill will test the mettle of riders before they set their sights back on London and a fast finish at The Mall – against the backdrop of Buckingham Palace.

BBC ONE’s ever popular Sunday evening programme Antiques Roadshow will be filming for

its 34th series at the All England Lawn Tennis Club, Wimbledon, on Thursday, 25th August, 2011. The doors open at 9.30 am and close at 4.30pm. Entry to the show is free.

This will be presenter Fiona Bruce’s fourth year with the Roadshow and she says, “Presenting the Antiques Roadshow is, for me, one of those rare and very lucky coincidences in television when you get to work on a show that you already love to watch. Exploring the human story behind every object is what makes Antiques Roadshow so fascinating. And everyone loves the agony and ecstasy of the ‘what’s it worth? moment. The AR isn’t just about antiques - it’s history, beauty and drama all wrapped up in one.’

Some of Britain’s leading antiques and fine arts specialists will be on hand to offer free advice and valuations to visitors, who are invited to raid their attics and bring along their family heirlooms, household treasures and car boot bargains for inspection by the experts.

The last series, watched by an average of six million viewers, included some great finds such as an Egyptian stone head dug up in a garden worth £10,000, a medieval ring found on a farm and valued at £20,000 and a Meissen cup and saucer bought at a car boot sale for £2.50 which was worth £5,000. Among some of the stranger items brought along were uneaten pieces of Royal wedding cake and a life-sized model of a lion once used in a TV series. People with large pieces of furniture or other big

Visitors to the show should use public transport and entry is via Gate 5 in Church Road. We regularly see between 1500 and 2000 visitors on the day. Despite the high turnout everyone will get to see an expert.”

Epsom Live! is back with a bang this summer as star-studded musical talent, old and new, are set to perform live

after racing at Epsom Downs Racecourse throughout June and July.

Texas, fronted by lead singer Sharleen Spiterim, will hold court and entertain crowds on June 30th, the first evening of the summer line-up. Performances of Texas’s rock anthems I Don’t Want a Lover and When We Are Together will be showcased from their UK number one albums, White On Blonde and The Hush, as the first music night sounds across the Surrey Downs.

Legendary American rock group, The Beach Boys, will bring Rock And Roll Music to Epsom Downs Racecourse when they perform after racing on Thursday 7th July. Fifty years since The Beach Boys formed and were first heard on the radio, the group will travel from California to the Surrey Downs, where they will perform a selection of their greatest

hits, including number one singles ‘Good Vibrations’ and ‘California Girls’ at the live outdoor concert.Brit Award nominees Scouting For Girls will arrive at Epsom on 14th July, fresh from their UK Arena tour. The phenomenal popularity of Scouting For Girls has seen the band tour internationally, as well as achieving double platinum status for their eponymous debut album. The indie-pop trio will perform a selection of their greatest hits including Elvis Isn’t Dead and She’s So Lovely.

With boundless energy, talent and looks to match, boy-band of the moment The Wanted are back to perform as part of Epsom Live! for the second consecutive year. Shortly before their debut single shot to number one, the group supported The Feeling at Epsom in August 2010. Now, they return, a phenomenon, and are sure to shine as they perform hits including ‘All Time Low’ solo on Thursday 21st July, 2011, the penultimate night in the series of live outdoor concerts.

Blondie has been confirmed as the fifth and final act to take to the stage this summer on 28th July as part of the series. Vocalist Debbie Harry will lead the American rock band – who pioneered the early American new wave and punk scenes of the 1970’s and sold more than 40 million records worldwide – to Epsom Downs Racecourse where they will perform their chart-topping hits Heart of Glass and Maria in front of racegoers and fans on Thursday 28th July.

For every English polo player, and indeed for the greatest names in the sport worldwide, the Coronation Cup – the feature match on Cartier International Day at Guards Polo Club – remains the most

coveted event to play in during the English polo season. This is the day when the polo fields

of English summer enjoy full bloom, infused with a spice of glamour, celebrity and royal patronage.

This year, the Hurlingham Polo Association, the sport’s governing body in the UK, will be celebrating the centenary of the Coronation Cup. Of course, the Cup has not been played for in every one of these 100 years – two world wars saw to that – but it has been one of the most famous matches in global polo over the last 10 decades. In fact today, the Cartier International Day is considered the world’s largest spectator polo event, attracting more than 20,000

like it anywhere in the world. No event can claim those sorts of numbers, and when you get a great, close match, and the crowd in those stands gets behind you, the hairs stand up on the back of your neck. It is the biggest game in the world. We love that as players. There’s no greater feeling.”

Alan Kent, another England stalwart, who has also coached the England team, says: “My memories of Coronation Cup are either adulation and joy, or complete demoralisation. It’s all or nothing, highs and lows. But win or lose, you cannot help looking back on every match you played in, or were involved in, with a deep sense of pride.”

The Coronation Cup is framed by Cartier International Day, which has become a late summer addition to the social sporting calendar. The event slots in alongside Henley, Wimbledon, Ascot and the Lord’s

Andrew Hine, a former captain of England, and now the England team coach, believes the match day has a flavour like no other. “You can play in the Argentine Open in Palermo, Buenos Aires, which for many is the Mecca of the sport, with the noise and 5,000 people on top of you; but when you play at Guards Polo Club in The Coronation Cup, and you hear the roar of 25,000 people, there is nothing

Test match. The Day itself offers two outstanding polo games – the Coronation Cup in the afternoon and a morning game – showcasing up-and-coming English talent – for the Golden Jubilee Trophy. In addition to this first-class sport on the field, the day provides some fantastic hospitality opportunities, including the chance to spend the day – and the evening – in the Chinawhite marquee. Chinawhite has been the post match party provider for the past 10 years and this year has invited the legendary producer Mark Ronson and Axwell from the Swedish House Mafia to be the guest DJs during the evening.

If a quieter afternoon is needed, then Keith Prowse offers a more relaxed lunch and afternoon tea option in their marquee, situated within the Smith’s Lawn Enclosure, close to the Cartier Marquee. Cartier announced earlier this year that this will be their last year of sponsorship of this high-goal event – after 27 years – as they will be sponsoring Guards Polo Club’s Queen’s Cup tournament from 2012. The Cartier Marquee guest list is always impressive, with an array of stars from the world of film, television and music. With renowned chef Anton Mosimann and his team

providing a delicious menu and John Carter creating the breathtaking floral displays, a Cartier Marquee invitation is always one to treasure.

If a more relaxed picnic with friends is what is needed then the Club has arranged for Foreman and Field, its official picnic suppliers, to create a special Cartier International Day menu which can be delivered to you at home or can be collected at the Club on the day. In addition, Guards Polo Club creates an extensive shopping village, offering everything from Audis – an appropriate venue for them as they are the sponsors of the England team – to Pommery Champagne.

In fact, there are so many stalls that you will need to keep an eye on the time to make sure that you don’t miss the match. The first Proms concert took place on 10th August 1895 and was the brainchild of the impresario Robert Newman, manager of the newly

built Queen’s Hall in London. While Newman had previously organised symphony orchestra concerts at the hall, his aim was to reach a wider audience by offering more popular programmes, adopting a less formal promenade arrangement, and keeping ticket prices low.

The 100th Proms season took place in 1994 and the festival now includes over 70 main Prom concerts every year, ever widening the range of symphonic and operatic music presented. The BBC Proms continues to welcome leading international performers whilst showcasing the best of the British music scene, including the BBC’s own orchestras and choirs.

The BBC Proms are broadcast on BBC Radio 3, and many are broadcast on BBC television and all broadcasts are available via the BBC iPlayer for 7 days.

Auctioning off over 600 vintage and modern wrist watches,

including specialist pieces from Rolex, Omega, Cartier and Patek Philippe, there are plenty of opportunities to add to your collection. There is however, one exceptional lot, this 1969 Rolex Cosmograph Daytona, the ‘Paul Newman Daytona’ estimated at £30,000 - £40,000. Described as ‘the vintage sports watch to own’, this Paul Newman Daytona is one of the original early models and has the unique and highly sought after black and cream exotic dial.

In the 1960’s Rolex gave its chronograph model a major facelift, introducing the Rolex Cosmograph. With its accuracy and unique ability to calculate the average lap speed of racing cars, the 1961 Rolex Cosmograph became known as the Daytona through its association with racing and the great track.

Paul Newman’s affiliation with the Rolex Daytona began with his appearance in the 1969 Indy Car racing film Winning in which he appeared to wear the model. Although Newman himself does not recall wearing this

specific Daytona in the motion picture, his 1973 appearance on the front cover of Italian magazine Moda, wearing what again appeared to be a Rolex Daytona, only went further in cementing his association with the model. Indeed, Paul Newman himself identifies that his appearance in Moda magazine may have been the catalyst for his lifetime association with the Daytona Model.

With a love affair stemming from his role in the film Winning, Newman’s racing

career began in 1972 and he made his first real public connection with the Rolex Daytona when his wife Joanne Woodward bought him a Rolex Daytona with the inscription ‘Drive Slowly, Joanne’ on its case. Upon stepping into the racing car Newman became a supreme driver. In 1995 Newman won the GT1 Class in the Rolex 24 Hours of Daytona and in doing so was presented with a new Rolex Cosmograph Daytona, featuring the inscription, ‘Rolex 24H at

Daytona, Paul Newman, Rolex Motorsport Man of the Year, 1995’. His ability as a driver and love for the sport even gained acclaim from champion racing driver Mario Andretti describing Newman as ‘one of us.’

Interestingly, unlike more modern actors and celebrities, although Newman wore this Rolex Daytona model for over 30 years, he never officially endorsed the model or publically endorsed Rolex as a brand. Thus, the term ‘Paul Newman Daytona’ was adopted by collectors and enthusiasts and has never been officially used by Rolex. Nevertheless, as a result of Paul Newman’s dedication to his career, his love for racing and indirect contribution to the legacy of Rolex the Paul Newman Stainless Steel Cosmograph Daytona is considered to be ‘the holy grail’ for serious watch collectors.

Despite the challenging warm and dry weather conditions throughout April and May, exhibitors excelled in both planting and design, with the world class growers and exhibitors in the Great Pavilion amassing over fifty gold medals in total.

The Daily Telegraph Garden, designed by Cleve West, was awarded gold and has been given the accolade of Best Show Garden. This is a stunning hat trick of Best in Show awards for The Daily Telegraph.

Among the gold winning highlights, Diarmuid Gavin achieved his first gold medal at the RHS Chelsea Flower Show for his Irish Sky Garden. The judging panel awarded a total of eight gold medals, five silver - gilt flora medals and silver flora medals in the show garden category.

In the Urban Garden category, eight gardens were awarded medals - three gold; four silver-gilt and two silver medals. The Best Urban Garden 2011 title went to Stockton Drilling Ltd, with Lands’ End and Worcester Bosch gardens also winning gold.

The Best Artisan Garden accolade and a gold medal were awarded to the Hae-woo-so (Emptying One’s Mind) garden. Other awards in this category are one other gold, one silver – gilt, three silvers and a bronze.

In the Great Pavilion, right at the heart of the RHS Chelsea Flower Show where world class nurseries exhibit a vast range of top quality plants, there were over 100 medals awarded, of which 56 were gold. The President’s Award in the Great Pavilion was awarded to Crug Farm Plants, Gwynedd

Four gold medals were awarded in the Continuous Learning category in the Great Pavilion, with Sparsholt College being awarded gold and Best Continuous Learning Exhibit.

In the first session of the Floral Arrangement Awards a total of nineteen awards were presented including four gold medals.

Tradestands are an important part of the RHS Chelsea Flower Show and Alitex, Hartley Botanic Ltd and David Harber Ltd were awarded the highest accolade of a Trophy award.

was the first year of what is now known as the London Polo Club, beginning life as the Ham

Common Polo Club it was part of a whole host of equestrian clubs that offered Londoners the opportunity to watch one of the most thrilling sports to have ever been conceived.

The club was formed for players who may not have played in the major high goal tournaments but shared a love for the sport. This is a strong tradition that has been carried through to the present day. Other similarities to the modern club included the presence of the 10 goalers of yesteryear. Johnny Traill the first Argentine 10 was a regular player and coach.

England’s inter war years saw a temporary halt to polo in the UK, however, the late great William Francis Walsh, affectionately known as Billy, set to in 1946 reviving polo in the Royal Borough of Richmond Upon Thames. The first post war polo tournament was played at Roehampton in 1947 and fittingly Billy, boasting a 5 goal handicap, and the Ham team took the trophy home.

Club. Now played at an eight goal level, major english polo teams will be competing in the tournament played in August.

Billy continued to develop polo in London; though he is remembered well for this, he is also recognised for being one of the key figures in the post war revival of the sport inthe UK. As a token of recognition, on the polo international day in 1985 HM the Queen presented Billy with a bronze of a polo player.

In 2010 there is only one polo club in London, it has become one of the most idyllic settings in the capital city and goes from strength to strength. Upon arrival players and spectators find themselves nestled between the leafy cover of Richmond Park and the River Thames, overlooked by the stunning Jacobean Ham House.

The club now boasts one of the busiest schedules of competitive polo and charity events. Over the last few years in excess of £1.5 million has been raised for worthy causes and every season around 2000 chukkas are played on the Richmond grounds.

This year will be no exception, at the time of

writing the ponies are in training and the opening matches of the London Polo Season will be played on Sunday 2nd of May. The first of the charity events, the Childline Sundown Polo Challenge, will be on Saturday the 19th of June.

This event, now a regular fixture, often features world number one polo player Adolfo Cambiaso making the match one of the most exciting to watch. Famous faces at the dinner and party have included Esther Rantzen the founder of the charity, Neil Fox, John Hurt, Andrew Neil, Elle Macpherson and Jodie Kidd.

Over the past few Years fundraisers for foundations like Laureus, hosted buy Boris Becker, and Adolfo Cambiaso’s charity in partnership with Jaeger LeCoultre have been held. Uma Thuman, Rob Brydon, Fabio Capello and a host of sports personalities have all been present supporting these fantastic causes.

The Chackravarty Cup has also been played on several occasions at the London Polo Club and always features a few members of the Royal family on the team sheet.

Being a sports club there are of course our

competitive tournaments, aside from the Roehampton cup, the fixture list now features the Dubai trophy. Played at a 12 goal level this is the highest standard of competitive polo played during our season. The tournament will consist of a choice selection of teams and finals will be held on Sunday 8th of August.

One of the most exciting developments for players and spectators this season will be the addition of a number of international matches. The London Polo Club will hold matches between the home team and Moscow, Barcelona and Bangkok for the ‘Polo International Club Series’, the aim of the series is to promote international cities and polo.

Reciprocal away matches will be held in the various cities and this looks to be nail-bitingly exciting. Matches will be played every Sunday from May till September, picnickers and spectators are welcome. The clubhouse also has a lively Bar and Restaurant, for information on becoming a playing or social member please contact the polo office.

The Sunday Times Rich List is the definitive guide to the nation’s wealth. The 23rd edition of the high profile and much anticipated list hit the newsstand

on in May. Further exclusive bonus content was rolled also online over the next month.

As well as the perennially popular main supplement, this year’s Rich List will had greater accessibility, a dedicated iPad edition as well as exclusive content online for digital subscribers. This year for the first time the information will be fully interactive, creating a uniquely personal and even more useful experience for The Sunday Times customers.

On May 8 the 104 page supplement listed the 1,000 richest people in the UK as well as Ireland’s 250 richest. A perpetual favourite with customers and advertisers, each of the entries are profiled and there are specific tables for the music millionaires, the richest in fashion, the wealthiest hedge fund managers and the top political donors. The list appeared simultaneously on www.thesundaytimes.co.uk/richlist2011 as part of the online subscription - available to new users for just £1 for the first 30 days, subscribers received access to both The Times and The Sunday Times websites, award-winning iPad editions, as well as Times+ membership benefits.

The print campaign plotted the financial back stories of various rich listers against iconic portrait shots. Additionally there will be a concerted multimedia campaign encompassing PR, eCRM, digital and search.

On May 15th the publication of The Sport Rich List. This innovation first appeared with great success last year. The 2011 version will included the 100 richest sportsmen in Britain and Ireland, the 40 richest young sportsmen, the world’s richest tennis players and golfers plus interactive tables of the richest owners and managers in British football. There is also be a table of the highest paid football players, graphics of Britain’s richest football team by position based on current players and a listing of the wealthiest British F1 drivers over the years. This will appeared as a stand-alone supplement within The Sunday Times as well as online at www.thesundaytimes.co.uk/richlist2011 and through iPads.

Digital subscribers were able to access the 2,000 wealthiest in the UK (the 1,000 listed on May 8 and the 1,000 below them in riches). This fully interactive database allowed searching of all entries by region. There is also live tables for the richest women in the top 2,000 plus extended data from The Giving Index.

A first for the list in May, saw the online publication of The World’s Richest 200 as well as Europe’s Richest 100. Available to all digital subscribers these international listings include biographies for every entry and fully interactive tables. There is also a facility to search the UK online list for the richest people by country of origin. The lists are based on identifiable wealth (land, property, other assets such as art and racehorses, or significant shares in publicly quoted companies), and excludes bank accounts (to which the paper has no access).

The garden, not surprisingly, tends to take a back seat. However now is not the time to forget your plants, if you want to enjoy

then all year round. With the heat and dryness of summer, it only takes a day or two to turn a healthy flourishing garden into a scorched savannah. Now is the right time to make sure that watering is in hand.

Before leaving the garden to it’s own devices whilst you take your well earned break, it is essential that any prized plants will be watered whilst you are away. Some of you will have friends or house sitters that can be cajoled or reminded to keep an eye on things, for the rest of us it involves some form of automatic watering system. Watering systems can range from state of the art self contained systems, with their own storage tanks, to small but effective systems using a water timer and soaker hose or drippers.

Unfortunately it is not possible to say which type of system would be best for your garden or lifestyle and prices vary accordingly. What I can say is that there is a system for most budgets.

For most situations I would advocate the use of a water timer, such as those sold by Hozelock and Gardena, attached to the outdoor tap, to control the water to a soaker hose. Soaker hose, also known as leaky or drip hose, is best used for plants in beds and borders, it is wrapped around the base of the plants gently leaking water into the soil whilst the timer is on.

For containers and isolated plants a micro sprinkler or dripper would be better, applying the water directly to the root ball of the plant.

Although watering systems may seem complicated, in reality they are quite easy to install effectively with the correct advice, most garden centres should be capable of providing this.

What I would suggest is that any system you install is put in place at least a few weeks before you first go away, as the timing of the water will need to be adjusted. In this time you will need to keep an eye on the system until the amount of water being applied is adequate, but not

excessive. Care is initially required to ensure that the water is being applied long enough for it to soak into the soil as far as the root ball. Whatever the system you use, this shouldalways be done in combination with mulching the surface to reduce evaporation. Putting a layer of mulch, such as bark on the surface, dramatically reduces water loss from the soil. It enables the water to soak in slowly, rather than run off and for it to remain in the upper soil levels, which is important for establishing plants. Once the watering is in hand I would recommend you relax and enjoy your holiday, with one less thing to worry about.

urrey resident, Charlotte Skinner began composting when she moved into a house with a garden and decided to start a

vegetable patch, just over 2 years ago. Charlotte tells us why she composts and gives some top tips for green-fingered gardeners.

What do you use the compost for?

For our vegetable patch.

Other than garden waste, what weird & wonderful household waste goes in?

All our veg peelings and other food waste including tea bags, egg shells and egg boxes

(cardboard ones), some shredded paper and wood ash.

What does composting involve, how much time do you spend a week doing it and how costly is it?

Time is minimal; you just need to put waste in the compost bin rather than your kitchen bin so

a short walk outside is all that’s required and turning the compost with a fork every now and then (we do it about every 4 to 6 months). The only cost was for the compost bin itself approx £17 but with free delivery.

What are your top tips for what makes good composting?

Research what items from around your home and garden can be used eg coffee

grounds and cardboard from cereal boxes.

What effect has making compost had on your kitchen rubbish bin?

Our kitchen waste has reduced considerably. It’s now only used for some items that can’t

be recycled and a bit of excess food waste.

Do your children join in and what are they learning about?

Our 4-year-old daughter loves to help in the garden and one of her favourite jobs is

taking her wheelbarrow of garden waste to our larger compost heap and tipping it out. This keeps her amused for hours, as well as enjoying time in the garden she has learnt that some things (weeds) can be pulled out of the flower beds and flowers should be left for everyone to look at and enjoy.

The veg patch has taught her about where food comes from and composting food waste

has introduced her to recycling. Weeding the garden, digging and sowing new seeds have turned into one of our ‘favourite family times’.

Are there any downsides to composting?

The compost bin is a bit smelly in the summer when you take the lid off but when

the lid’s on it’s fine.

If there was one piece of advice you would give to someone thinking about starting to compost, what would it be?

Put your compost bin in an accessible place close to the house. When we first moved in

to our new house, we put the compost bin out of the way at the end of the garden.

When it’s raining etc we didn’t want to go tramping down the garden so didn’t use the compost as much as we could have. I would also advise getting a good odour free compost caddy for the kitchen top.

Why compost?Compost is a natural, nutrient-rich food product for your garden. help buffer soils that are very acidic or alkaline. Compost improves your soil’s condition and your plants and flowers will love it!

Home-made compost is natural and peat-free, which means it’s good for the environment outside your garden too. It reduces the need to buy peat products, which have been commercially sourced and extracted from peat bogs, resulting in the release of the carbon stored in them.

A step-by-step guide1. Find the right site. Site your bin or

heap on bare soil. If space is limited, put your bin on concrete, tarmac or patio slabs but make sure there’s a layer of soil or existing compost on the bottom so garden creatures can colonise.

2. Add the right ingredients. Fill your kitchen caddy with the right ingredients such as tea bags, taking care not to compost cooked food, meat or fish.

3. Fill it up. Empty your kitchen caddy and your garden waste into your bin or heap. A 50/50 mix of greens and browns is the perfect recipe for good compost.

4. After 9-12 months. The ingredients you have put in your bin or heap should have turned into a dark brown, earthy smelling material at the bottom of the bin, which can be dug out, with the newer material being left. Don’t worry if your compost looks a little lumpy with twigs and bits of eggshells – this is perfectly normal. Simply sift out any bits and return them to your bin or heap.

5. Ready! Once your compost resembles thick, moist soil and smells very earthy, it’s ready to use.

6. Collect the compost. Instructions will be delivered with your bin. Simply lift the bin slightly or open the hatch at the bottom and scoop out the fresh compost with a garden fork or spade. If you have built your own, simply rake out the ready compost.

7. Use it. And watch your garden bloom. Compost can be used for all types of planting needs – vegetable plants for allotments; general potting (if mixed with something like ordinary soil); or flowerbeds and enriching new borders by mixing in existing soil.

Whether you’re a keen gardener or just want to choose the greenest, most natural way to deal with your food and garden waste, composting is the perfect solution.

It is completely understandable that you might worry about the possibility of breast cancer if you find a lump, but remember; most lumps are not cancer. The most likely cause for a lump depends on your age. In women from 20 to 30 a definite lump is most likely to be a fibroadenoma.

These are benign lumps that can sometimes be painful or enlarge but they are not cancerous and do not turn cancerous.

Between 40 and 50 breast cysts become more common. These are also entirely benign. They are collections of fluid within the breast and although they can also be painful they are not dangerous. From the age of 30 onwards the gland tissue in the breast changes and it becomes more fibrous and nodular. The way the breast feels has been described as like a scaled down packet of frozen peas and occasionally, in just the same way that frozen peas can stick together to form a clump, so breast tissue can “stick together” to form definite nodular areas. These areas are also often tender but, once again, are not a sign of cancer.

The rate of breast cancer begins to rise after the age of 40 and increases with age. Of course, breast cancer can occur at any age and I have seen cases in women as young as 19 and in their early 20s, but they are very, very rare.

The important message is, if you notice a change in your breast please get it checked. Let’s go through what you need to do.

The first thing to do is to see your GP and explain what you have noticed. They will examine you and in some cases may suggest a further check at a different time of your menstrual cycle. Some of the nodular areas can change with the cycle and this is actually a reassuring sign. If a discrete nodular area

persists or your GP finds a definite lump, or is unsure, they will refer you to a breast clinic for a full assessment by the breast team. The team should consist of several consultants working together, and includes a surgeon a radiologist and a pathologist.

The investigation of any breast problem revolves around, what is called, the triple assessment. It is like doing a jigsaw puzzle; pieces (of information) are collected and put together until the whole picture (the diagnosis) becomes clear. Indeed, this is often termed “the diagnostic jigsaw”. In some cases only 2 pieces may be needed, in others 4, but 3 (the triple assessment) is most common. Let’s have a look at what the pieces are.

The first specialist you meet will often be the breast surgeon. They will ask questions about your symptoms (called taking

your “history”). They will also ask about previous medical problems and your family history. They will then examine both breasts. How the breast feels when it is examined is the first part of the jigsaw.

If there is a definite area of change then a fine needle aspiration may be carried out. This test uses a normal sized syringe and needle. The needle is placed into the area of the breast and “jiggled” so that a small amount of material is sucked

into the syringe. This is sent away to be looked at under the microscope. More commonly now a test known as a core biopsy is done. A core biopsy takes a core of breast tissue about 1.5 cm long and 2 mm in diameter. The core biopsy is carried out by first putting some local anaesthetic into the skin of the breast. Once this has worked (it is quite quick) a small nick is made in the skin to allow the core biopsy needle, attached to the biopsy “gun”, to be placed into the breast. When the “gun” is “fired” (it is painless, but does make a noise rather like a large stapler) the needle takes

the sample. This is often repeated a few times. The appearance of the cells that have been removed with a fine needle aspiration, or the result of the core biopsy, is the second part of the jigsaw.

The final part of the triple assessment involves taking images of the breast. In ladies under 35 an ultrasound scan is the commonest way of doing this. In ladies over 35 they will have a mammogram as well. Full field digital mammograms are the most sensitive type available at the moment. What an area of the breast looks like on a mammogram or an ultrasound scan is the third piece of the jigsaw.

If the triple assessment produces the same answer at each stage the result is said to be concordant and no further tests may be needed. For example, a lump in a 30 year old lady may feel like a fibroadenoma, look like one on an ultrasound scan and a core biopsy shows pieces of a fibroadenoma.

What if the pieces don’t quite fit and the picture (diagnosis) is not initially obvious (the triple assessment is then said to be discordant)? For example, in a 50 year old lady a lump may feel like a benign nodular area but look odd on an ultrasound with a normal mammogram. In this case another piece of the jigsaw may be needed, such as an MRI scan or even, occasionally an operation to remove an area for full examination under the microscope.

An important part of the triple assessment that is not commonly mentioned is to watch how things change (or not) with time. It would not be unusual to check things a few months later, repeating some, or all, of the assessment to make sure nothing has changed. A thorough triple assessment may actually be a quadruple assessment!

Once again it should be emphasised that in the majority of cases a triple assessment (or a quadruple one) will reveal that cancer is not present. Most symptoms are age related changes or benign lumps. However, it is important that any new change is thorough investigated by a specialist to make absolutely sure that all is well.

There is quite a lot of differing opinion about whether by using high SPF creams and thereby protecting ourselves from potentially harmful skin cancers, that we may be risking increased illness as a result of vitamin D deficiency. I do not believe this to be the case.

While UV radiation is one source of vitamin D, I do not consider it to be the best source because the benefits of obtaining vitamin D through UV exposure cannot be separated from an increased risk of skin cancer. UV radiation is the most preventable risk factor

for the development of skin cancer and that should be the primary concern of any doctor or person who is concerned about reducing their own risk of developing skin cancer.“

A healthy diet and knowledge of what sunscreen suits what skin are also vital elements in maintaining awareness and best practice when under those rays, Dr Ariel Haus continues;

“Provided that you follow a healthy diet that includes foods naturally rich in vitamin D (e.g., dairy products and fish), foods/beverages fortified with vitamin D (e.g., fortified milk and fortified cereals), and/or vitamin D supplements then your body will absorb an adequate amount of vitamin D to protect you from illness associated with a lack of vitamin D.

However in considering the right sunscreen to look for it is important to keep in mind that what makes sunscreens different is the level of protection from UV exposure that they provide. A common misconception is that the higher the SPF the better the protection offered.

The sun contains both UVA rays (which pass through window glass, penetrate into the deepest layer of the skin and are associated with premature aging and melanoma) and UVB rays (the sun’s burning rays, which are blocked by window glass, but are the primary cause of sunburn, and also are linked with skin cancer). It is important that any sunscreen provides protection against both UVA and UVB rays but the SPF number on sunscreens

Aesthetic medicine is a rapidly growing area, with non-surgical treatments such as muscle relaxant injections, Dermal fillers, light therapy, mesotherapy and face peels to name a few, available to help replace what has been taken away through ageing and the environment. These procedures are no longer sought by just women, with increasing numbers of men also looking for a fresher, younger look.

Recent media coverage via the documentary Botox Britain (BBC 3, Thursday 7th April) highlighted the growing trend for injectable treatments and dermal fillers, including the worrying ease of obtaining DIY kits over the internet, and the so-called teen-toxin phenomenon. The documentary quite rightly reinforced the need for those considering non-surgical cosmetic treatments to seek a reputable provider.

This article looks at what regulations are in place for non-surgical cosmetic procedures and what questions you should be asking the healthcare professional before undertaking such procedures.

In the UK, only Doctors, Dentists and Nurses registered with the Nursing and midwifery council can administer injectable muscle relaxants and dermal fillers, although there are many reports of individuals without the correct medical training, such as beauticians, running such services. The increasing reports

of unqualified providers, inappropriate treatment environments and incidents of remote prescribing (where an individual is review over the phone by a doctor, who then prescribes the cosmetic

injectable for the nurse to administer) have concerned healthcare professionals in the cosmetic treatments sector. Until now these services have fallen outside the scope of the Quality care commission and other regulators. The Independent healthcare Advisory Service (www.

independenthealthcare.org.uk) have recently launched a register of injectable cosmetic providers in conjunction with the Government and Department of Health. This involves background screens, and registers Quality Assurance Mark approval to regulated healthcare professionals (doctors, registered nurses and dentists).

Aesthetic nursing practitioners are also coming under more pressure. The nursing governing body, Nursing and Midwifery Council and the Royal College of Nursing, advise against ‘remote prescribing’, and say this should be the exception, rather than routine practice. They recommend that individuals undergoing cosmetic injectable treatments should have a face to face review with the prescriber (Doctor, Dentist or independent nurse prescriber) prior to a cosmetic injectable being given, although there is currently no legal requirement for this. In practice this means that aesthetic nurse practitioners are advised either undergo an extensive independent prescribing course, alter their clinical practice, or in extreme circumstances risk potentially losing their nursing registration. Nurse

Independent Prescribers can legally prescribe and administer licensed injectable cosmetics on their own initiative.

What questions should you ask prior to your non-surgical cosmetic treatment?

Who will carry out the treatment & are they registered (e.g. GMC for Doctor, NMC for nurse? ✱ How long have they been trained in this treatment & do they have a certificate of accreditation? ✱ How frequently do they carry it out? ✱ Do they have professional indemnity insurance? ✱ Is the treatment painful and if so what form of anaesthesia is used? ✱ Is all equipment used sterile and used only for me? ✱ Will I see a doctor, dentist, or an Independent Nurse Prescriber and will he/she prescribe the cosmetic injectable? ✱ What are the risks involved? ✱ Do you have any ‘before and after’ photographs that I can see? ✱ What type of aftercare regime will I need after treatment? ✱ Will I be reviewed after the treatment – and is this included in the price ✱ What will happen if I am unhappy with any aspect of the treatment?

Iwould like to thank the Life series of magazines for selecting me to do a regular feature on how to achieve a healthy mouth and an attractive smile. I will be focusing on useful

and important topics for readers such as prevention of gum disease, how to have fresh breath, what causes decay and how to avoid dentistry!

I would like to start the first feature on cosmetic dentistry and I will address various questions that have been emailed to me.

I have crooked teeth and do not really want to have braces for 2 years. What are my other options?

Creating the life changing transformations for the people on the TV programme has been one of the highlights of my professional career.

EROSIONA relatively new problem is becoming apparent in dentistry. is is ‘wear’ of teeth due to chemical erosion. is causes tooth surface damage, especially to the inner surfaces of the upper front teeth leading to sensitivity, pain and other problems. Erosion is totally different to dental decay, which is caused by plaque bacteria that cause cavities in teeth. Enamel is the strongest substance in the human body and is the protective outer layer of teeth. However, acid can slowly wash away or wear away the enamel irreversibly!

What are the different types of harmful acids in different foods and fruits?e main types of acid that are dangerous for your teeth are: Citric

acid, lemons, grapefruit, oranges, Lactic acid, yoghurt, fromage frais, cottage cheese, Phosphoric acid, Coke, Pepsi, diet Coke, diet Pepsi, Acetic acid, vinegar as in oil and vinegar dressing, Malic acid, apples, Oxalic acid, rhubarb, It must be remembered that fresh fruit juices are just as, dangerous for teeth as fruit juice mixtures e.g. Five Alive.

Why is acid erosion more common now?Many of these fruits and juices formerly were only available in season. With refrigeration these

are now available all year round and are doing phenomenal damage to teeth. An exaggerated emphasis on slimming for ‘health’ reasons has not helped. Also, teenagers are having a lot of fizzy drinks and alcoholic drinks mixed with soft drinks.

How can I protect my teeth and avoid this problem?ere are many things you can do to avoid acid damage to teeth: -

1. Limit the number of episodes when acidic foods or drinks are taken. Try to eat all your fruit at one time of the day.2. DO NOT clean teeth after taking acidic foods or drinks. 3. If possible, rinse your mouth with milk or a fluoride mouthwash after eating or drinking acidic foods.4. Drink fruit juices through a wide bore straw thus getting the acid past the teeth, and away from the mouth.5. Try to avoid diet drinks. If you have them, drink them through a wide bore straw or drink them very quickly to reduce acid contact time with the teeth.

Should I brush my teeth straight after having something sweet such as a fizzy drink?No, this is not a good idea. e

acid in fizzy drinks causes softening of the enamel on the surface of teeth. If the teeth are then brushed with abrasive toothpaste, the problem is made worse due to the “scouring action” of the toothbrush. e result is that the “softened” enamel is often “spat” down the sink! It is therefore better to leave the teeth for an hour or two for the saliva in the mouth to neutralize the acid first.

How will I know I have this problem?e only way is to see an experienced dentist that can recognize this subtle problem and give appropriate

guidance and advice. It is better to prevent erosion than to treat it!

Apart from the diet, what else can cause chemical damage to teeth?Acid can also be produced from the body to cause damage to teeth.

Two fairly common causes are anorexia bulimia and a condition caused gastro-oesophageal reflux. Both these conditions can wear away teeth at an alarming rate and may sometimes require extensive and therefore also expensive dental treatment. It is better to get appropriate advice from a doctor AND a dentist to help with this.
