West Virginia Water Emergency Survey 2014

- Questionnaire -

You are being invited to participate in a research study. This research is being conducted by the Harvard School of Public Health to learn about population's exposure to risk communication messages during the West Virginia chemical spill in the Elk River. Specifically, we are interested in learning about your experience and opinions about the information you received during the emergency. Your participation in this study is voluntary and you may withdraw from participation at any time. As part of your participation in this study, you will complete a 12-minute online survey administered by Knowledge Networks Inc. During the survey, you will be asked various questions related to your opinions on how the information was delivered to you and how easy it was to follow the recommendations. You can decline to answer any of these questions for any reason and can terminate the survey at any time if you wish to do so. If you have any questions about this study, you can contact Dr. Elena Savoia , at 617-834-9055 or [email protected]. Thank you again for your time and participation. Please feel free to contact me anytime with additional questions, Best regards, Elena Savoia 617-384-9055 (office) 617-735-5273 (mobile) [PPGENDER] GENDER [SP]
