WENTZ SCHOLARSHIP 2012-2013 Undergraduate Application Materials GENERAL INFORMATION -- The Lew Wentz Foundation and Oklahoma State University are happy to provide one-year $2,750 cash scholarships for OSU undergraduates, especially those who have excelled as campus and community leaders, for use in the 2012-2013 academic year. To be considered, complete this form, attach copies of materials requested (see below), and return them to 334 Student Union no later than 5:00 P.M. on March 2, 2012. Only one Wentz award of any type, per year, per student, is allowed. One-page summary of your activities and honors at OSU; 3. An official transcript; 4. One letter of recommendation from an OSU faculty member, department head, or academic advisor also to be received no later than March 2, 2012. Unless otherwise arranged, the letter is assumed confidential, not available to persons other than the screening committee, and should be enclosed in a signed/sealed envelope. By signing below, I affirm that all information is correct and I understand that any false or misleading information will cause my application to be withdrawn from the competition. Further, by signing below I consent to allow OSU to publicize my participation by using any of my quotations and pictures concerning the program in the various media.
