Employer Address
Company Information is posted so that you may notify our headquarters in the event of an accident, injury, and/or incident on this job site.
It is a guide in the event that any employee has questions/concerns that arise as part of their everyday activities.
We truly wish to hear from staff in order to answer questions as they occur.
Emergency Contact Number
Although, the emergency contact number is typically 911, in some instances it may be different depending upon the location of the job site and/or military project which may have specific criteria for their locations.
You must review this information at each and every job site.
Also, you must note the location of telephones for the job site and/or Superintendents/Forepersons with cellular telephones.
Your Job Site Address
Each staff member must be able to state his/her job site address at any time.
• You may be questioned from time to time to ensure you know it.
• If necessary, carry it in your wallet.
In the event of an Emergency, you must be able to communicate this information to Emergency Medical Services, Fire Department, Police, and/or your Company Headquarters.
Health & Safety Department
The Health & Safety Department is a primary contact in the event of an Emergency regarding any of our job sites.
You must call the Health & Safety Department any time you have questions and/or concerns regarding Health & Safety on your job site before you do anything which may put you, co- workers, and/or sub-contractors in a hazardous situation (s).
Our office number is 912-871-8771 (please write it down).
Please communicate your concerns with your Superintendent/Foreperson first to ensure they are aware of the issue (s) as well.

First Responders - General Information
The Sack Company/SCM, Inc. has ensured each job site has qualified first responders in the event of an Emergency on our job site (s).Please find your Superintendents, Forepersons, and/or Safety Advisors on each job site.
Superintendents, Forepersons, and/or Safety Advisors will be your first point of contact with CPR/First Aid qualifications.
Employees are to notify First Responders immediately, and assist them as directed.
Standbys
Standby: Are individuals the First Aid/CPR person directs as needed.
In the event of an emergency, a Standby may be asked to:
1. Call 911 - if Emergency Medical Services are necessary
2. Direct Traffic: at Gate Entrances, then guide to actual location(s) of injured party. Provide P.P.E. as necessary.
3. Radio/Cell Phone Contact to Company Headquarters and/or Health & Safety Department. Assist First Aid/CPR to attend injured.
4. Direct On-lookers away from injured-privacy5. Back up person as needed basis
Inside the Emergency Contact Binder
Only those with current CPR/First Aid Cards shall act as First Responders in the event of an Emergency. They will be your Superintendents and Safety Advisors. See them immediately if needed.
Hazard Communications
Hazard Communications:
Ensures that the potential hazards of all chemicals produced or imported are evaluated and that information concerning their hazards is transmitted from employers to employees.
Specifically, understanding labels associated with chemicals/products you may use on and/or as part of your tasks associated with your Profession/Trade.
Hazard Communications
Hazard Communications often refers our employees to our Material Safety Data Manual. This manual is located inside your Job Site Office Trailer.

You will be questioned regarding the information you have been given and
asked to review such verbally, and/or written
format today and randomly at job sites.
Material Safety Data Sheets
Material Safety Data Sheets are often referred to as M.S.D.S. and are a part of our Hazard Communications program.
Material Safety Data Sheets offer employee information regarding hazards associated with the use of a chemical/product prior to use. This information must be reviewed BEFORE use of any chemical/product each and every time as information may change and you must be informed.
Material Safety Data Sheets
The information contained in a M.S.D.S. will vary in the format but generally contain the following information:
Now I will explain each of these area(s).
Name of Product
We list chemicals and products in our M.S.D.S. by the name of the product. Products are listed in order in the table of contents within the M.S.D.S. binder.
If you and/or another employee are unable to speak, read, and/or understand English you/they must seek assistance by another party who is capable of reading, writing, and speaking English.
Name of the Manufacturer
The manufacturer of the chemical/product is the
company who makes and/or produces it. This is very
important. In the event of an Emergency, you may need to call the manufacturer to ask
questions if an employee thinks that they have been
accidentally exposed to a chemical/product.
Manufacturer Emergency Contact Number
This is the telephone number the manufacturer has listed for us to call in the event of an Emergency.
It is usually listed as a 24-Hour Contact Number. A specific telephone extension number may be listed if a company has numerous chemicals/products.
It is crucial that you read and understand all of this information. In an emergency, you will need the Manual open in front of you and your first responder with you in order to pass information on to Emergency Medical Services and/or to take the injured party to a Doctor.
The more information that we have, the more likely that we will be able to comfort and aid the injured party.
Composition/Information on Ingredients
The manufacturer (maker) of the chemical/product gives specific information about how a chemical/product was made. Ingredients are all the items in the ‘mix.’
An industrial hygienist and/or a health & safety specialist is usually needed to review this information.
There are a number of letters (acronyms) that you may need assistance remembering. Please refer to the definitions page within your M.S.D.S. and discuss this information with your Superintendent/Foreperson, and/or call your Health & Safety Department if you need more detailed information prior to use.

Hazards Identification
This is a section which indicates primary routes of entry: a way the chemical/product may enter your body. Ex: 1.inhalation-you may breathe it into your body. 2. skin contact-you may have the chemical/product absorbed through contact with your skin.
It may indicate specific Organs that the chemical/product could cause harm to.
It may indicate acute effects -meaning a major concern and/or likely concern.
Chronic effects -meaning-of a long duration. In short, this section tells of the hazards which may effect both short and/or long term issues associated with your health and wellbeing.
First Aid Measures/Physician Information
This is the section you use prior to use so you know ahead of time what to do in the event of an emergency involving yourself and/or another co-worker/sub-contractor. It gives basic information.
You may also need the Emergency Contact or 24-Hour Telephone Number to ensure we have all
necessary information to respond to an Emergency and/or pass on to
E.M.S. and/or a treating Physician (doctor).
Fire Fighting Measures
This section gives information about flash point, flammability classifications, extinguishing methods, fire/explosion hazards, fire-fighting instructions, and fire-fighting equipment, just to mention a few.
Please note you must also be aware of and adhere to company related policies/procedures. Many times, you are to extinguish a fire only as a means to escape, not to act as a fire brigade.
The goal is to be aware of hazards and to not directly or indirectly create another hazard which may be of equal or more extreme danger to you, co-workers, the environment, and/or the community at large.
Accidental Release Measures
This is a section indicates what to do if the chemical/product accidently leaks or spills.
You may also need to review local, state, and/or federal standards associated with such an issue.
You may also find specific information pertaining to the disposal of a chemical/product.
Handling and Storage
This section generally explains handling precautions and storage specifics.
Storage must indicate the environmental temperatures that the chemical/product may withstand. It may indicate other chemicals/products it CAN NOT be stored near. It may also advise against sudden punctures, pressure build ups, vapors and the like.
Exposure Controls Personal Protective Equipment
ALWAYS try to use engineering controls before reverting to the use of Personal Protective Equipment (P.P.E). Engineering controls should be used in order to engineer the hazard out or to minimize it to the best of our ability.
P.P.E. may be required.
This section shall be reviewed by employees prior to use of chemicals/products each and every time.
Staff shall use the P.P.E. as directed by the M.S.D.S. in order to be in compliance of local, state, and/or federal regulations.
Ex: safety glasses…
Exposure Controls Personal Protective Equipment
If you are unfamiliar with any personal protective equipment, you must tell your Superintendent, Foreperson, and/or Health & Safety Director so that we can train you on how to use your P.P.E. properly.
Fit, cleanliness, proper ratings (such as weight), and, in some instances, medical evaluations are vital prior to use.
Without such, you may inadvertently cause a health hazard.
So please ASK we need to know!
Physical and Chemical Properties
This section gets very specific regarding ingredients used to create the chemical/product
you are reading about.
This area requires an industrial hygienist and/or health & safety specialist.

Stability and Reactivity
Stability usually refers to storage and handling
of the chemical/product.
Additional items may be of a scientific nature and require a industrial hygienist and/or health & safety specialist.

Toxicological Information
This section gives an overview of the nature, effects, and/or detection of poisons and/or the treatment for poisonings.
It may also give brief information on current/past testing associated with animal/lab testing - reviews.
Ecological Information
In brief, this section may provide detailed
information about how the science of the
chemical/product interacts in relationships between living organisms and the
environment.
For example: the leak and/or spill of certain
chemical/product may need immediate control (clean-
up) due to concerns which may damage our
environment; water, and/or other living species.
Transport Information
This section gives specifics for appropriate moving, hauling, and delivery techniques
associated with a chemical/product.
It usually mentions hazard class, identification, packing types, labels, and types of
transportation.
Regulatory Information
This section mentions the regulations associated with various local, state, and/or federal mandates.
Some that you are probably familiar with are: – The Environmental
Protection Agency– Occupational Safety Health
Administration– Right-to Know, etc.
Other Information
This section may include:

Labels
When we speak of M.S.D.S., we must also ensure that you understand the necessity of labels.
Labels are used to educate the public and to provide ample information in the event of an Emergency.

Question:The label is on the 5 gallon bucket. What happens when I only need a little bit of the contents, not a whole 5 gallon bucket? Should I label my small, clean plastic container?
Yes. If you were to forget and/or step away from the container, another person may not know what it is and/or the hazards associated with it.
Additionally, environmental issues may be of concern if the contents were improperly leaked or disposed of.
Labels
Question:When I am transferring a chemical/product, should it have its own container? Or, can I use an old paint can?
No. You must use a clean container meant for use of the chemical or product you are using.
If the container isn’t clean then the other chemical or product in the can/could cause additional hazards with the unknown chemical or product.

Our employee handbook is a basic overview of what is expected from our staff. Please refer this handbook in order to understand Company policies, procedures, and benefits. This handbook is not meant to cover each and every situation and/or to answer every question. Please contact your Superintendent and/or Project Manager for specific concerns not listed. Individuals are multi-faceted and local, state, and/or federal laws and regulations are updated or changed on a regular basis.
Please pull out your Employee Handbook and read, review, and share any questions/concerns you may have.

contact information. Then, just keep it accessible in your wallet or tool box so that you can contact them in the event of an illness, and/or company
related issue.

Mandatory Report of Injury, Illness, Sprains & Strains
All Sack Company and SCM, Inc. staff must be aware of and/or have signed the Injury, Illness, Sprains & Strains Reporting Form.
This form clearly indicates that every employee must report any concerns/signs of injury, illness, sprains & strains immediately to his/her Superintendent and/or Health & Safety Director the moment it occurs while you are performing the task(s) associated with your trade/profession.
Mandatory Report of Injury, Illness, Sprains & Strains
Our Company expects for each and every employee to adhere to this at all times. In the event that one chooses to delay reporting an
incident for any reason, he/she shall be subject to a three-day suspension from his/her trade or
profession from H.A. Sack Co, Inc. or SCM, Inc., without pay.

Drugs Don’t Work in Georgia
Refers to our Company Drug and Alcohol Free Workplace.
1. Now you shall Read, Understand, and Review our Drug and Alcohol Free Workplace Training (4).
2. All New Employees shall go thru the Employee Section in Full- Read, Understand, Review, and Sign.
3. You must now Read, Understand, and Sign our Drug and Alcohol Free Workplace Policy (3 of 3 pages-9/08).
4. After Completion of such you; the new employee must STOP; and ask your trainer any questions that you have about this policy.
Employee Responsibilities/Accident Prevention Compliance Procedure/Policy
• You must now be have a (4 page 1/09) document listed as above in front of you.
• You shall now read, understand, and sign this Compliance Procedure/Policy as clarification of our expectations for our employees.
• If you would like to further discuss and/or if you have any questions regarding a particular issue you must, STOP, ask your trainer any questions that you have. If you choose not to ask, we will presume that you understand and accept the policy/procedures in full.

Many of the documents are required by local, state, and/or federal mandate. However, it needs to be clear that you, the employee, and new member to our organization, must be responsible in reading, understanding, and signing of all of the necessary components.
We may use e-mail, telephone calls, and/or written letters as needed to review your information and/or referrals. In the event that we find inconsistent and/or falsified information and/or documents, you will be asked for additional information and/or excused from consideration for employment.

There are a number of items you must provide and/or read, review, understand, and sign as part of local, state, federal and/or Company Requisite. Again, each employee is responsible for providing all of the necessary documents in full.
