• Enter Survey Tool
• Click Set Up Survey Tool
• Select Evaluation Option
• Select Cohort, for Readiness Evaluation then Save
• Cohort II– Effective 1/1/2008– No 2008 Submission– SNP 1-6– 7A & B not required
• Cohort I– Effective 1/1/2007
– 2008 Submission
– SNP 4-7 only SNP 7 is (2C & 3B)

Don’t Wait to Submit
Structure & Process Measures due to NCQA by June 30, 2009
– SNPs do not have to wait until June 30th to submit their S&P Survey Tool. If you finish early, you may SUBMIT early.
– Heavy volume at the end of June may cause delays. The sooner you get it in, the smoother it will be.

• NCQA survey team assesses performance against all Structure & Process measure requirements
• Plan Comment Period – document clarification
• Final scores analyzed for inclusion in report to CMS
• NCQA will not publicly report SNP data

• Build an electronic system for all documentation to be linked to Survey Tool– Not required to keep
documents in a central file….but this is a common practice
– Use any organizing principle you wish
– May pull from different areas – may help in version control

• Central reservoir of all reference materials for compliance
• Sort by document, standard, file path and date
• Load into the library and then link with specific elements

Document UploadingDocument Uploading
• Start early
• Send documents in bunches
• Copy your documents from your network to your hard drive, and upload from there
• Upload during off-peak hours
Uploading Documents
*Attaching & uploading are 2 distinct processes*
– Start with documents you have loaded and linked to elements in Library
– Must upload to NCQA secure server

• For security and data protection only allow file types:– .CSV, .DOC, .GIF, .JPG, .MPP, .PDF, .PPT, .RTF, .TXT, .VSD, .XLS
• Convert others to .GIF, .JPG via scanner or to .PDF using Adobe® Acrobat®
• Do not send Zip files to NCQA
• Save documents to 2003 version
• Do not send PHI

• To minimize file size and retain readability – Save scanned documents
in .GIF, .TIF, .JPG format– Color images: 80 dpi and 8 bit (256) color
settings– Text pages: 80 dpi and 2 bit (black and white
or line art settings)– Black/white image: 80 dpi and 8 bit (256
color) settings or 80 dpi and 8 bit scale (256 shades) settings

• Highlight key sentences • Focus on only what element
requires• Limit size and number
– Use only pertinent pages with cover page that provides the context
– Unless element requires multiple documents, one is sufficient
– Use hyperlinks in documents if you wish

• Utilize page & paragraph number field
• Brief summary of information – focus on what the element requires
• If element has multiple factors, consider using this field to map out information
• Not sufficient to cut and paste info into this box, must still attach supporting documentation

• Comment Box is different from the Support Text Box– Requires SNP to include additional information
related to a specific element or factor
• SNP 1: Element A Comment Box– NCQA is looking for information regarding the eligibility criteria used by plans for case
management and the % of members enrolled in case management
• SNP 1: Element B Comment Box– NCQA is looking for other referral sources for CM
