Compassion fatigue and moral distress can creep into the caregiving experience, diminishing the joy of the work. Self-care is a non-negotiable activity which preserves wholeness of character and positively impacts the quality of care provided. This presentation will provide tangible methods for strengthening resiliency, and engage the participants in activities which cultivate gratitude.

Attendees will learn about the role and importance of social media during this session. We will discuss the importance of storytelling as a branding tool, and how to utilize the digital world to enhance the story being told.

Using Remote Access of Medical Records to Drive Quality Improvement in Diabetes CareDr. David Baldwin, MD, Endocrinologist, Rush University Emily Hendel, MS, CNP, Director of Clinical Services, CommunityHealth

In August 2018, a team of nine highly-qualified professionals, students, and volunteers began a quality improvement project at CommunityHealth, using a model of remote consultation with the Athena EMR system. We’ll share the follow-up data for our cohort from September 2018 to August 2019, showing significant improvements in diabetes control. We will also present the toolkit showing how to implement our approach.

This session features two free clinics going through the rigorous process of Patient-Centered Medical Home (PCMH) transformation, and obtaining recognition through the National Commit-tee for Quality Assurance. They will discuss transformational changes resulting from their PCMH implementation which have made a major impact on the clinics, staff, and patients; they’ll offer strategies and tools which any clinic may implement to enhance care delivery, improve quality and address key elements of patient-centered care.

The misuse of, and addiction to, opioids has become a national crisis. This panel discussion begins with an overview of the crisis, and encompasses current trends. Innovative programs will be shared, including successes and challenges. Attendees will gain knowledge of resources for their patients, and ways to incorporate similar programs into their own communities.

There are always many moving pieces of legislation and lawsuits happening at the federal and state level. One example: the pending health repeal lawsuit which could affect millions of people all in the US. In this session, participants will learn about federal and state legislation that may affect or benefit clinics and patients. Participants will also learn where to find more information about such legislation, how to get involved, and what they can do.

Gender Inclusive Care and Medication AccessScout Bratt, MEd, Outreach and Education Director, Chicago Women’s Health CenterLisa Fiedler, MPH, Clinical Services Director, Chicago Women’s Health Center

Chicago Women’s Health Center (CWHC) has practiced an affirming, collaborative approach to health care for over forty years, and has more than a decade of experience providing informed, inclusive care for women, trans people, and people with gender-expansive identities. In this workshop, CWHC will share lessons-learned, inviting participants to reflect on inclusion in their own spaces and practices. Additionally, CWHC will present unique barriers to medication access faced by trans people, and strategies its clinic uses to support clients in overcoming these barriers.

Dental Providers – Roundtable Discussion – Lessons Learned – Base Practices – Challenges Shawn West-Marconi, MBA, Executive Director, Will-Grundy Medical Clinic

In this session, participants will be able to share and learn from their peers: what it takes to open and manage oral health services in a free and charitable clinic. Come to this session prepared to share lessons-learned, the challenges and successes of providing oral health in a free and charitable clinic. Come to this session if your clinic is contemplating providing oral health services: learn from your peers!
