Infection ControlWhat is Hand Hygiene? Hand hygiene (or hand washing) removes the visible soil and kills the germs that may be on your hands. Good hand hygiene can be done by using an alcohol based hand rub or using soap and running water.

Hand hygiene stations (containing alcohol-based hand rub) can be found at the entrances to each hospital as well as throughout the building. Please use them when entering and exiting any patient care areas.

Posters on Hand HygieneAt SBGHC we want our patients and staff to know how we are doing with hand hygiene and to feel comfortable talking about it.

In various locations throughout the hospitals, you will see posters about the hand hygiene practices at that site. These posters will:

• Tell you how well that site is doing at cleaning their hands before entering a patient’s room and providing care; and

• Tell how well the site did last quarter compared to this quarter.

If you have any questions about these posters, or about SBGHC’s commitment to hand hygiene please contact our Infection Control Manager at ext. 4312.

Sometimes there is a coloured (isolation) sign placed on your door. What does isolation involve?

• You will be placed in a private room or you may share a room with someone else who has the same germ.

• A sign will be placed on your door and chart to alert hospital staff and visitors that you require this isolation.

• Staff caring for you should wear gloves and gowns and sometimes a mask to prevent them from carrying the germ to other patients.

If you are a patient in isolation, it is very important that:• Staff, doctors, family and friends clean their hands before and after contact

with you.• You wash your hands after using the bathroom and before leaving your room. • While you can walk in the hallways, avoid areas used by patients, staff and

visitors. You may have to wear a mask and/or gown.• If you have uncontrolled diarrhea or open draining wounds, you will be asked

to remain in your room.

Can family & friends visit me if I am in isolation?There is no reason to restrict your visitors. Visitors should always wash their hands before their visit with you and wear a gown and gloves. When they are ready to leave, they should remove the gown and gloves and immediately wash their hands. It is unlikely that your family will catch this germ if they wash their hands well.

Safety and SecuritySBGHC is committed to providing the safest environment possible for our staff, students, volunteers, patients and visitors. We will:

• Promote education, mutual respect, cooperation and understanding to prevent and manage aggressive behavior and intimidating actions.

• Not tolerate any form of aggressive behaviour, violence, threat of violence and/or intimidation by any staff member or members of the community.

• Not tolerate any individuals displaying any type of aggressive behavior either verbally or in a physical manner not related to a medical condition. They will be requested to leave the hospital immediately.

• Notify police if the individual fails to leave immediately. • Provide a secure environment for moms and babies at all times.

Hospital staff and volunteers are easily identified by their photo identification badge. Please notify a staff member immediately if you observe any suspicious activity or individuals, if you have any security concerns, or if you have lost a personal item.

Scent PolicyDue to allergies among the public, patients, and staff, our hospital has adopted a scent free policy. Please refrain from wearing perfumes, aftershaves, scented hair sprays, hand lotions, etc., while you are visiting SBGHC.

SmokingAs part of our commitment to the health of our patients, visitors and health care workers, our buildings and properties are 100% smoke free.

Smoking is not permitted indoors or outdoors on any property owned by SBGHC, including parking lots, walkways and lawns.

Hospital InformationAccessibilityThe Ontarians with Disabilities Act, 2001 (ODA) was enacted to improve opportunities for people with disabilities and to provide for their involvement in the identification, removal and prevention of barriers to their full participation in the life of the province.

The ODA requires each hospital to prepare an accessibility plan and review it annually. SBGHC’s Accessibility Plan was developed with community consultation in 2004 and continual improvements have been made each year to increase the accessibility of: the four hospital sites, policies, programs, practices and services for patients and their family members, staff, health care practitioners, volunteers and members of the community with disabilities.

Effective January 1, 2010, SBGHC was required to comply with the Accessibility for Ontarians with Disabilities Act, 2005, Customer Service Standard. The standard is part of new legislation that will create a barrier free Ontario for people with disabilities. SBGHC is committed to fulfilling the requirements of the Act and providing patient and family centered care to people with disabilities.

Admission All personal information (necessary for hospital and government records) will be considered strictly confidential (see page 6). It is important that you make staff aware if you have a Health Care Directive or if you have been in a hospital out-of-province during the past year.

Be sure to bring:• Ontario Health Card – Your Ontario Health Insurance Plan (OHIP) will cover

all your in-hospital costs except for: • Preferred accommodation (semi-private or private accommodation)• Telephone • Television• Non-urgent transportation

• Other Health Insurance Cards (supplementary insurance information)• Medications – Please bring a list of current medications. If you are to

continue on a medication not on our formulary, your own medications will be issued to you. You may be asked to arrange for a family member to bring these medications to the hospital. You are requested not to self-administer medications while in hospital. It is important that the physician knows what medications you are on. You are responsible upon discharge to ensure you have your home medications and dosette. Please give all of your medications

to the nurse and we will return them when you are discharged. Please tell your doctor and nurse if you are allergic to any medications or suffer from any other allergies. If you have any questions about your medications, your doctor, nurse, or hospital pharmacist will assist you.

• Personal items - You should bring only necessary personal toiletry articles such as a toothbrush, toothpaste, comb and brush, shaving items, or cosmetics (no scented products – see page 8). Although the hospital provides patient gowns, you may want your own pyjamas or nightgown together with a bathrobe and slippers. Bring your own mobility aids (cane, walker) including proper fitting shoes and slippers.

Please do not bring: valuable papers or documents, scented personal care products, items of sentimental value, jewelry or expensive clothing, large amounts of cash, alcoholic beverages, TV, DVD players, radios, or plug-in appliances unless otherwise approved by staff.

The hospital cannot accept responsibility for lost, stolen or damaged personal articles and/or money. If you are unable to send valuables home, please ask a nurse to place item(s) with Registration for safekeeping.

AnimalsIn general, animals can be brought in to the hospital only under the following conditions:

• The animal is a designated Service Animal.• The animal is part of the St John Ambulance Therapy Dog Program or similar

approved program.• It is deemed that a short visit by a personal pet would be beneficial for the

well-being of the patient. The Director Patient Care or delegate approves such visits within the hospital. Animals must have vaccinations up to date.

AccommodationOur Patient Registration staff will make every effort to provide you with the type of accommodation you request. Our hospital provides standard ward, semi-private, and private rooms. Semi-private and private rooms (if requested by you) do cost more and must be paid by you or your insurance company. If you have insurance coverage, please let our staff know.

Bank MachineA 24-hour bank machine is located only at our Kincardine site, next to Registration.

BillingPatient Accounts can be settled at Patient Registration.

Cardiac Rehabilitation (Kincardine Site)Patients are required to have a physician’s referral. Cardiac Rehabilitation is an individualized program of exercise and education designed to help you live a healthier lifestyle and reduce your risk of future heart events.

Cellular PhonesThe use of wireless devices in patient care areas shall be controlled to maintain a safe and supportive patient care environment, and to protect patient privacy and the confidentiality of information. This is especially true when taking photographs with any kind of phone or device within the hospital.

Charges for Take Home ItemsWhen items such as canes, crutches, some medications, etc., are supplied before your discharge, the cost will be charged to you. Walkers and wheelchairs are hospital property and are not to be taken out of the hospital. Payment for items can be made at Patient Registration.

Charges for Non-residents of OntarioAny patient who is not a resident of Ontario is charged a rate set by the Ministry of Health, based on the actual cost of the hospitalization.

Community Care Access Centre (CCAC)If you are assessed and it is determined that you require home care, CCAC is a community agency that works with the hospital to help patients and their families plan a smooth discharge to the community with the right supports in place. CCAC staff may see you in hospital if you need help at home and they can provide information about other community health support services.

Consent to TreatmentYou may be asked to sign a consent form for certain tests, procedures and treatments, including transfusions of blood and blood products. Before signing, ensure you know the benefits and risks of having treatment and the risk(s) associated with not having the treatment. If you want more information or have questions before signing, talk to your doctor. You have the right to refuse treatment, but please ensure you are fully informed before making your decisions.

Co-Payment (ALC)Co-payment may commence when a patient is in an acute bed and has been designated as an Alternative Level of Care (ALC) patient awaiting a nursing home bed.

Diagnostic ImagingDiagnostic Imaging (includes Bone Densitometry, CT Scanning, Mammography and Ultrasound) is open from 8:00 – 4:00, Monday to Friday, at all of our locations. All patients must check in at the Registration desk upon arrival at the hospital.

If you have to come to the hospital for a test, always bring your health card with you to your booked appointment time and your requisitions from your physician.

Discharge PlanningThe health care team will assess your discharge needs from the time you are admitted as a patient. Questions regarding your health care will be answered before discharge. Prescriptions and follow up appointments, if necessary, will be given on the day of discharge. The Community Care Access Centre (CCAC) is available and able to answer questions on home care services. Please see the Patient Transfer section (page 16) for information on transportation arrangements to get you home.

DonationsDonations can be made directly to the South Bruce Grey Health Centre or to our supporting Foundations and Auxiliaries. For more information, please see page 20.

EthicsThe Hospital has a comprehensive ethics program. If you or your family experiences an ethical issue/concern, ask a nurse to explore the issue with you and they will contact one of the Ethics Committee members.

Fire Drills/Hospital CodesFire exits are clearly marked throughout the building. In the event that the fire alarm sounds, please remain calm. Return to your room and await instructions from the SBGHC staff as they are trained in dealing with emergency situations.

Your cooperation is essential. Elevators will be shut down; patient rooms and hallway doors will be closed until an “ALL CLEAR” announcement is made on the speaker system. If you should discover a fire, pull the nearest Fire Alarm (if possible) and call for assistance immediately.

Fire prevention is everyone’s responsibility – please report any unsafe conditions you see. Fire drills are conducted routinely for your added safety.

Food Services/Patient MealsA variety of food, snacks and beverages are available 24 hours a day at all four locations from vending machines in the food court areas.

Patient meals are served between 8:00-8:30 a.m. for breakfast, 12:00-12:30 p.m. for lunch and 5:00-5:30 p.m. for dinner. Upon admission, nursing staff will ask patients about any diet restrictions or allergies – your diet is ordered by your physician.

Registered Dietitians provide inpatient and outpatient diet counseling services at all sites.

Visitors may purchase a meal tray in advance of meal service.

Flowers, Parcels and BalloonsThese will be delivered to your room and we ask that no latex balloons be brought onto the premises. In accordance with our scent free policy, fragrant flowers (such as lilies and hyacinths) are not permitted in patient care areas.

Gift Shop HoursChesley: Monday to Friday from 2:00 - 4:00 p.m.Durham: No gift shopKincardine: Monday to Friday from 11:00 - 4:00 p.m.Walkerton: Monday to Friday from 10:00 - 4:00 p.m. (located on first floor)

Hairdresser/BarberThis service can be arranged by yourself or your family. Please notify the nursing staff of any appointments.

Help LineThere is an installation and monthly charge for this service. To arrange for this service, contact Registration staff.

Internet Please inquire at Registration if you require internet access.

LaboratoryOutpatient Hours at all sites: Monday to Friday from 8:00 a.m. - 2:30 p.m. (Statutory holidays excluded).

There may be special preparation for testing to be performed. Please contact the laboratory closest to you to ensure that you are informed of any special precautions or preparation prior to arrival at the laboratory. Patients must register at the Registration Desk for all laboratory testing prior to coming to the lab.

Services: The laboratory provides a broad range of testing, both within the department and by referral to other testing laboratories. The following range of tests is available for our inpatients, emergency patients, and outpatients:

• Clinical Chemistry and Special Chemistry • Hematology • Transfusion Medicine • Microbiology • Anatomic Pathology and Cytology • Serology

Our main referral laboratories are Grey Bruce Health Services, Hospital In-Common Laboratories, London Health Sciences Centre, and Public Health Laboratories.

Legal – Living Wills and Power of CareSBGHC, in your best interest, has the ability to make your wishes regarding treatments known to all doctors and nurses who may care for you while you are a patient here. You could be admitted as the result of an accident or prolonged illness and, if this happens, we would like to make sure we are following a course of treatment which you fully understand and approve of.

You may wish to discuss this with your doctor or nurse to make sure your wishes are respected. If you have already signed a living will, advance directive, power of attorney for personal care or similar document, we encourage you to provide us with a signed copy on admission. This ensures your wishes are known.

Lost and Found If you have lost an item or are reporting a found item, please contact the Registration desk. SBGHC is not responsible for any lost or stolen personal items, either within the hospital or parking lots.

MailIncoming mail will be delivered to your room daily and outgoing mail can be left at the Registration desk. Any mail received after your discharge will be sent to your home address.

Newspapers/Reading MaterialNewspapers are available at our Gift Shops – see page 13 for hours and locations. Book carts are located outside each gift shop (payment by “donation”).

Obstetrics/Family Birthing CentreSBGHC’s Family Birthing Centre, located at our Walkerton site, features:

• Prenatal and postnatal care• Large birthing suites, en suite washrooms with jacuzzi tubs,

kitchenette facilities • Labour support (one to one nursing care) • Postnatal follow-up clinic 48 hours after discharge

How do I access this service?Speak to your family physician for a referral to the SBGHC Family Birthing Centre. If you do not have a family physician, visit our website for a list of the physicians who are accepting new patients for obstetrical care.

Operating Rooms (Kincardine and Walkerton Sites)Hours of operation: Monday to Friday 7:30 a.m. - 3:30 p.m. On call for emergencies at the Walkerton site only. General surgeons are available at both sites.

Services:• Cancer surgeries – melanoma, breast, colon• Caesarian sections • Dental – adult and pediatric - restorations, extractions• Endoscopic procedures – EGD and colonoscopy• Endoscopic Carpel Tunnel release • ENT – ear tube insertions, septoplasty, tonsillectomy, adenoidectomies• Excisions, revisions• Laparoscopic Surgeries – Gall Bladder, Hernia, Bowel Resection,

Appendectomy, Tubal Occlusions, • Hernia Repair: Umbilical, Incisional, Inguinal hernias• Vasectomies

Palliative CareSBGHC works with a variety of health care partners and provides palliative care at all of our sites. We offer spacious, private rooms and strive to provide a safe, comfortable environment for patients and families.

ParkingSBGHC has paid parking which costs $3.00 per exit . There is 30-minute free parking at all sites to enable patient drop off and pick up. Should you need to come to SBGHC on a frequent basis, a monthly pass card can be purchased at Registration at a reduced rate. Palliative CareSBGHC works with a variety of health care partners and provides palliative care at all of our sites. We offer spacious, private rooms and strive to provide a safe, comfortable environment for patients and families.

ParkingSBGHC has paid parking which costs $3.00 per exit . There is 30-minute free parking at all sites to enable patient drop off and pick up. Should you need to come to SBGHC on a frequent basis, a monthly pass card can be purchased at Registration at a reduced rate. Patient LoungesThese are available to all patients and their families and consideration should be shown to other patients who may be relaxing in the lounge.

Patient TransferIt is the responsibility of the patient and/or family to arrange and pay for transportation home when you are discharged from SBGHC after an emergency visit, appointment or hospital stay. This includes transportation to long-term care facilities.

Transfer to another hospital for care related to your current admission remains the responsibility of SBGHC.

If a family member or friend is not able to provide transportation home, there are several service providers available for a fee.

Registration and SchedulingRegistration is located at the Emergency entrance of most sites. This centralized approach helps us provide efficient and high quality service. Please report to this area upon your arrival at the hospital.

Restorative CareSBGHC’s 10 bed Restorative Care unit helps patients regain their independence after the acute phase of an illness with the expectation that they will be able to return home.

For suitable candidates, the program serves as an alternative option to applying for placement in a Long Term Care facility.

Patients care for themselves (dressing, bathing, taking medication) with assistance if required, and participate in daily activities designed to regain strength and independence.

The support team includes an Occupational Therapist, Physiotherapist, Nurses, Personal Support Workers (PSW) and an Occupational Therapy/Physiotherapy Assistant.

How do I access this service?Patients from all of the hospital sites in Grey and Bruce Counties are screened by Community Care Access Centre (CCAC) Case Managers for program eligibility.

Satisfaction SurveysYou may receive a patient satisfaction survey after you leave the hospital. Your opinion is important to us as we continually strive to improve services for our patients and their families. All surveys are kept confidential through our external provider.

If you would like your clergy (faith leader) to visit, please ask your nurse or family member to contact them. If you have no specific religious affiliation and a visit is desired, please notify your nurse and the Hospital Chaplain will be contacted. Community clergy and faith leaders come into the hospital regularly to visit members of their congregations. The Day Chaplain can help to connect you with them or make arrangements to visit you at your request.

TaxisYou can ask Registration or a nurse to call a taxi for you or give you the numbers of local providers. This service will be used at your expense.

Telephones A daily rate applies for telephone hookup and patients are responsible for long distance charges. For local calls at each location, dial 9 to get an outside line.

Obstetrical Visiting Hours: 2:00-4:00 PM, 7:00-8:00 PM

Do not visit if you have a cold, fever or feel unwell.

• Special consideration is given to seriously ill patients and children. Extended hours are considered upon request.

• Visitors may be requested to leave the patient’s room during special procedures.

• Please limit visitors to two at a time. Visitors that are too noisy or causing a disturbance may be asked to leave.

• Children under 12 years of age are discouraged from visiting. When visiting, they are required to be supervised at all times.

• During visiting hours please be considerate of other patients.• If privacy is needed for visiting, please speak to the nursing staff.

Be Involved in your Health CareOne of the keys to getting the best health care is to be an active member of your health care team. This means taking part and being involved in every decision about your care. This also means asking questions of your health care team so that you can make informed choices, coming prepared for your medical treatments, and knowing what to do when you go home.

You should understand as much as you can about any:• Medical problem you have (your diagnosis)• Treatment or procedure you will have• Medicine you should take and how to take it

Here are some good questions to ask:• “What should I do when I go home?”• “What should I tell my family about my care?”• “Can you tell me more about my medical problem?”• “What does this test or treatment involve?”• “What is the purpose of this test or treatment?”• “Are there any other options?”• “What should I do (or not do) to get ready for a procedure?”

Tell a member of your health care team about your past illnesses and your current health condition. You are the one who knows the most about your health. Tell the members of your health care team everything you can, even if you think they already know, and even if you think it is not important.
