What To ExpectBeing well-prepared for your appointment will ensure that the physician has all of the needed information to provide the best possible care for you. To understand what to expect for your first visit, please read through this brochure. You’ll find practice hours, payment policies, information about our committed physicians, along with our first visit procedures. We look forward to providing foot care for you and your family.

New PatientsYour first visit establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, such as medical history, and give you time to get to know your doctor.

You may save time by printing out and completing the patient forms in advance of your appointment. Both the Patient Demographics and Medical Information forms can be found on our website.

Canceling AppointmentsWe understand that unexpected issues come up that require patients to cancel and reschedule their appointments. We ask for at least a 24 hours notice when canceling or rescheduling your appointment. This will allow us time to schedule another patient who may be in need of urgent care. There will be a charge for missed appointments. We also ask that you call us if you are going to be late for your appointment.

Testing ServicesDigital X-ray services are offered on-site. For our patients who need more in-depth testing or hospitalization, Podiatry Associates of Plainville is affiliated with Sturdy Memorial Hospital. Sturdy offers broad access to the necessary resources and technology needed to care for our patients.

What To Bring To Your AppointmentYou should arrive for your appointment about fifteen minutes early to check in at the front desk. We will ask for your insurance card each time you come so we are absolutely sure we have your current insurance information. We will also verify your home address and contact information and collect any co-pay due.

ReferralsMany health insurance companies require a referral from your primary care physician before seeing a specialist. If your insurance requires a referral, you must obtain it prior to your scheduled appointment. Billing ProceduresWe submit billing claims to your insurance company within a day or two of your visit. This is why we ask to see your insurance card each time you see the doctor. Most insurance companies send payment and an itemization of any patient co-pays, co-insurance or deductibles within a month. We then send you an invoice for these items, for which we expect payment.

We know that some health expenses can be unexpected. We also recognize that health insurance can be confusing. Our staff is available to help you with any questions or concerns you may have.

Release of Medical InformationFederal laws require all health providers to maintain patient privacy. For patients 18 years of age and older, no health information can be released to another party without the consent of the patient, even to a parent or other relative. We cannot release medical information without the patient (or parent if under 18) signing an authorization to do so.

We look forward to providing expert foot care for you and your family.
