Need Notary Public for this part! Must sign in front of him/her.

Child Maltreatment Registry

•Complete and mail by October 18!• $10 check made out to “Arkansas Department of Human

Services.”

• Must be notarized.

• Mail to address in the first paragraph.

• Check “Licensed Teacher.”

• Do not complete school district information in bold near the bottom of the page.

Need Notary Public for this, too!

Have They Cleared?

• You will receive the “Verifying a Background Check or Issuance of a License” document electronically from Mrs. Ringgold with instructions on how to see if your Background and Child Maltreatment Central Registry Checks have been approved.

• VERY Important: Email Mr. Vaughn (kenv@uca.edu) when NCBC says “Approved.”

• Save this document! You will use it to check to see if your license has been issued after your Internship II semester.

Application for Teaching License

•Use black or blue ink.

• Follow Mr. Vaughn’s

instructions.

•Do not work ahead!

•NEXT semester, send $75 fee to Arkansas Department of Education.

Placements for Internship II

• Crystal Voegele contacts schools to make placements.

• NO REQUESTS!!! There is a set protocol.

• Placements are final when posted.

• Can still change up to last minute!

• You will receive an email from Mrs. Voegele with a link with the username and password to check placements.

Spring 2014 Calendar

• You will be on the public school’s fall calendar, NOT UCA’S.

• Make contact with your mentor teacher after your placement is posted THIS SEMESTER!

• You will begin your placement on your mentor teacher’s first day back in January. (This is before UCA begins.)

• It is your responsibility to find out the date from your mentor teacher.

• Internship II calendar is available and subject to change.

Graduation Paperwork•Mrs. Voegele will take care of sending graduation paperwork toyour advisors. You do not doanything with about this paperwork until your meeting at the beginning of the spring semester.

•Pay $30 graduation fee in McCastlain early and SAVE RECEIPT!!! You’ll need to provide a copy at our January 31st meeting.

• Register for Internship II as soon as registration opens.

• Think “professional attire” as you see clothes on sale over the next few months.

• Clean up Facebook, etc. now.

VERY IMPORTANT!!!!

Email Mrs. Voegele! Check your email!You will attach the electronic version of your personal

narrative.

Also let Mrs. Voegele know in that email if you have any potential conflicts of interest in any area schools.
