Login to your CHUSA page by going to www.chirohealthusa.comand clicking ‘PROVIDER LOGIN’ at the top of the page.
Login using the username and password provided in your welcome email. The password is temporary. You will be prompted to update your password after clicking login.
This is what you should see after completing your login to your CHUSA page.This is your ‘Home Page.’
You may create a shortcut on your desktop so you will only have to double-click to login. (see the next slide)
Just remember, you are always able to access your CHUSA page by going to our website and clicking ‘Provider Login.’
This login was created specifically for your clinic’s use. Do not share with anyone outside of your clinic staff.

Patients will list all dependents here; there is NO WHERE to list dependents
on the on-line enrollment form.

How to Enroll a PatientThe first step when enrolling a patient is to have them complete & sign the Patient Application (paper form.) This was attached in your welcome email or you may
download it from your CHUSA page. EACH PATIENT MUST DO THIS! NO EXCEPTIONS!
This form should NEVER leave your office! This form should stay on file in your clinic (paper or electronically.)

INSURANCE!
PATIENT ENROLLMENT: Enrolling PatientsAfter the patients complete & sign the paper enrollment application, click the tab ‘Patient
Enrollment.’ This is where you will enroll patients online & accept the membership fee.
PATIENT ENROLLMENT: Member InformationThe first part of the online enrollment is ‘Member Information.’
This is the information the patient provided on the paper enrollment form.
All fields with a red (*) are required fields. You may bypass the email field by unchecking the Go Green box. (If email is selected, the patient’s membership
packet & cards will be sent electronically within 24-48 hours.)
PATIENT ENROLLMENT: Order Summary
You will choose the program the patient wishes to join:
• ChiroHealthUSA for $49.00or
• ChiroHealthUSAPlus for $89.00
Remember, your patient may join CHUSA only and upgrade to the Plus membership later for an additional $40.
PATIENT ENROLLMENT: Payment Method
This is where you will enter the patient’s method of payment for their membership fee.
The patient may pay with check or credit/debit card. The payment will be submitted electronically to CHUSA.
If paying cash, accept the cash to your clinic account & submit a clinic check/card for the online membership fee.
Patients may not use HSA or FSA account cards for payment of the CHUSA membership fee as those program do not cover a membership fee, only services provided.
*NOTE: ChiroHealthUSA does not store any payment information.The patient’s memberships will NOT auto-renew.
• Credit cards accepted are: VISA, MASTERCARD, AMERICAN EXPRESS, OR DISCOVER
• Enter the card information.
• Note: when entering an expiration date, do not use a (/).
• This payment will transmit electronically to us after you click the submit button.
• For AmEx, the CVV code is 4 digits on the front of the card.
PATIENT ENROLLMENT: Credit/Debit Card Payment
• Enter the patient’s checking information. (Routing numbers are 9 digits.)
• The payment will transmit electronically to us after you click the submit button.
• The patient’s status in the database will show as ‘Payment Pending’ until the check clearsthe bank.
• Keep a copy of the check on file (for only 10 business days.) We will contact you if a copyof the check is needed to reprocess payment. That can occur if the account number isentered incorrectly. The patient’s bank will notify us if this happens and we will contact youfor a copy of the patient’s check.
• After 10 business days, you will know if the check has cleared and you can shred thepatient’s check. Their status in the database will also change and show as ‘Active’ whenthe check clears.
PATIENT ENROLLMENT: Check Payment
NOTE: there is a $10 fee for all returned checks. You will be notified if this occurs so that you may contact your patient.
PATIENT ENROLLMENT: Temporary Membership CardAfter you complete the online enrollment and submit payment information, click the SUBMIT button at the bottom of the page.
A temporary card will pop up for you to print for the patient. The patient may use this as a receipt for the program and as a membership card until they receive their membership packet by mail or email.
NOTE: This is ONE TIME POP UP ONLY. Once you exit this screen, you cannot go back.

The Patient Renew/Search page is where you can look up a membership, see a complete list of patient’s that you have enrolled, and renew memberships.
This database updates in real time – as soon as a patient is enrolled, you may search for them here.

PATIENT RENEW/SEARCH: Find Member by Member Number
You may use the ‘Find Member by Member Number’ look-up if you have a member number for ANY patient, even if the patient did not enroll in your clinic.
PATIENT RENEW/SEARCH: Name Look-up You may use the name look-up to find any patient that joined CHUSA in YOUR clinic. You are only able to view your patient database using the name look-up.
PATIENT RENEW/SEARCH: Viewing Your Entire Patient DatabaseThe box in the middle of the Patient Renew/Search page is where you may go to see a complete list of all patients that you have joined CHUSA in your clinic.
• ALL: Click here to view a list of every patient who ever joined the program in yourclinic – active, expired, terminated memberships.
• ACTIVE: Click here to view a list of only active memberships.
• EXPIRED: Click here to view a list of only expired memberships.
In addition to having access to this search method, you will receive a monthly report by email on the 5th of each month that will:
1. List all patients who joined the previous month – use this to check your records for anyduplicate memberships or any memberships that were not entered online.
2. List any patients that chose to cancel their CHUSA memberships the previous month. Thesepatients are no longer members of the program and are not eligible for CHUSA discounts.
3. List any patients with memberships that will expire the following month. This will give you amonth to touch-base with those patients and let them know their memberships will need tobe renewed.
PATIENT RENEW/SEARCH: Renewing Patient MembershipsPatient memberships are valid for one year from the date that they joined. Each year, patients must renew their memberships to continue to receive CHUSA discounted fees at your clinic.
There is a shortcut to renewing a patient membership! But remember, the patient must complete and sign a new paper membership application upon each renewal.
Next, go to the Patient Renew/Search page and locate the patient’s membership using the member number lookup or the name lookup.
PATIENT RENEW/SEARCH: Renewing Patient MembershipsClick the RENEW button to the left of the Member Number.
Clicking the RENEW button will open the patient’s saved member information. Note: payment information was not saved. Simply check the member information and update any changes.
Next enter new payment information and click RENEW at the bottom. A temporary card will pop-up again for you to print for your patient (just like when entering a new patient.)
ORDER BROCHURES• CHUSA keeps you stocked with brochures for ChiroHealthUSA and
ChiroHealthUSA Plus at NO COST TO YOU!
• To reorder, click the tab ‘Order Brochures’ and complete the order form.
• You will receive your shipment within 7-10 business days.
ENROLLMENT FORMS• Each patient must complete and sign a paper enrollment application when joining
the CHUSA program (and when renewing a membership.)
• These enrollment applications are located on your CHUSA page under the tablabeled ‘Enrollment Forms.’
• Print copies of the enrollment forms for your patients to complete.
• We recommend printing a few in advance so that you have them ready whenpatients want to join.
Important Reminder: The patient enrollment form should never leave your clinic. Patient’s must enroll in the clinic. Keep the patient enrollment form on file in your clinic – saving them electronically if you’re paperless is acceptable. The patient may havea copy of their enrollment form if requested.
MARKETINGWe do have marketing material available for your clinic to purchase. Marketing CHUSA is not required, however, we have had requests for some materials and they are available under the marketing tab. Please follow the instructions in the marketing manual for how to incorporate these materials into your clinic’s promotional plan.

MANAGE USERSYou have the option to create a username/password for staff in your clinic. This is not required, but is an option if you wish to create individual log-ins.
Click ‘Manage Users’ to create additional usernames for your clinic staff.
• The ChiroHealthUSAPlus program costs $89 for a 1 year membership.
• In addition to the chiropractic discounts, this program also offersdiscounts for: Vision, Dental, Pharmacy, & Lab/Diagnostic Networks.
• Patients may choose to just purchase CHUSA membership &upgrade to CHUSAPlus later.
• The cost is $40 to upgrade (still equals $89.)
• The expiration date will NOT extend.
• Clinic staff members are eligible for this program at a discountedrate of $30…call us to register!
• You will also receive brochures for this program in your welcomepacket and you will be able to reorder them at no cost to you.
