BENEFITS

This is plan that requires you to select a PCP. Your PCP will guide you through the health care system so you can get the right care at the right time.

•Pay less by using Tier 1 providers. They have been recognized for providing value.

•You need to select your personal PCP from the network. Each enrolled person must select a PCP.

•Save money by staying in our network. If you don’t use the network, you’ll have to pay for all of the costs.

•You need to get online referrals from your PCP to see a network specialist. If you see a network specialist without a referral, you will be responsible for the full cost of the service.

•Preventive care is covered 100% in our network.

Must be a general practice, family practice, pediatrician or internal medicine physician

Pay less using our Tier 1 providers.

Look for Premium Tier 1 network care first.

It’s easy for you to find Premium Tier 1 physicians. These providers have been recognized for providing value. Look for the Tier 1 symbol when doing a search.

Where you go for care can make a big difference in cost. You’ll pay a lower [co-pay] [and] [co-insurance] when you use these Tier 1 providers.

UnitedHealth Premium® Tier 1 providers have been recognized for providing value.

[Tier 1 hospitals can charge less for the same services compared to other hospitals. Tier 1 hospitals are any hospitals that aren’t labeled Tier 2.]

Free-standing facilities and centers are medical facilities in the network that are not connected to hospitals or another medical facility. They usually charge less. They may include radiology centers, surgery centers, and more.

Urgent care clinics

Navigate® Plus Plan

If you go out-of-network, your costs may be higher. Out-of-network providers can even bill you for amounts higher than what your plan will cover. For COVERAGE DETAILS, see your official health plan documents.

There’s coverage if you need to go out of the network. Choose what’s best for you.
