RESIDENT STUDENT MEDICAL AND HEALTH INFORMATION As part of the occupancy requirements, all resident students living in university sponsored housing must have the required immunizations to complete the application process. ALL resident students including interna- tional students must provide a photocopy of your health insurance card and immunization documentation with the Student Medical Information Form when you send it to the Department of Residential Life by August 1 for the fall semester; and by January 1 for the spring semester. If necessary, students can receive immunizations at the clinic during orientation. You must notify us by August 1 if you are participating. Your student account will be charged for each vaccine you receive. Pricing subject to change without notice. As a resident living in university sponsored housing , on campus or in Cotta Hall, you are required to have the following immunizations: Mumps (MMR is acceptable) Meningitis Tuberculosis Screening (PPD) ResLife office is notified if a resident student has a positive tuberculosis screening. A mandatory chest x-ray and additional medical attention is required. The resident student must provide the university with written documentation when this x-ray and follow up medical attention has been received. INTERNATIONAL STUDENTS: The 2-part Tuberculosis screening (PPD) MUST be received in the United States. ALL RESIDENTS: Mumps and Meningitis vaccines can be received at home for the following sessions by July 10 for FALL; by November 10 for SPRING; by April 10 for SUMMER. STUDENT ATHLETES: Please submit a health form with shot record to the Residential Life office. Physical forms go to the Athletic office. If you do not comply, you will be denied access to your room until requirements are met. RETURNING RESIDENT STUDENTS: if you have supplied this infor- mation already you do not have to do this again. Griffin Bucks is a declining balance account to purchase meals open to faculty, staff and students without meal plans including Cotta and Southwest residents. For resident students this is in addition to your meal plan and does not replace your meal plan. You can use Griffin Bucks to purchase additional food items in the DSAC Griffin Grill, or additional meals in the Ryan dining room. You must use your ID for purchases and there must be money in your account. Please tell the cashier that you are using either your meal plan or Griffin Bucks.

to set up a Griffin Bucks account, deposit a minimum of $25 into your student account through the Business Office. For each new deposit made you receive a 10% bonus. Money in this account is non-refundable and non-transferable. All transactions involving Griffin Bucks must be through the Business office. Their office hours are Monday-Friday, 8:30 am –4:30 pm or call 314-889-1405.

Griffin Bucks carry over from the fall semester to the spring semester, however, NOT from spring to the following fall semester.

Resident students are responsible for reporting lost or stolen ID cards to Student Affairs. Replacement cards are $10.00. Fontbonne assumes no financial liability for a lost or stolen ID card.

Renter’s insurance information - Carrying Renter’s Insurance for your personal belongings is recommended. In the event of a fire, flood, theft or other emergency, the university is not responsible for the replacement cost for a student’s personal be-longings which are damaged, lost or stolen. Personal belongings include but are not limited to textbooks, jewelry, bicycles, clothing, computer and other electronics. If you find your belongings are not covered under any homeowner’s policy it would be advisa-ble to investigate Renter’s Insurance.

The university does not endorse any specific carrier, however an assortment of bro-chures from various Renter’s Insurance companies can be found in the Student Affairs office located in Medaille Hall. Policies are generally based on quality and quantity of possessions. Prices are affordable for most residents. Check with your insurance carrier also.

Resident hall Information On campus residents receive a building access card and a room key at check-in. A protective carrying case is provided for the access card. Keep the card out of your pocket and off your key ring! Replacement Costs are $30 for any lost or damaged card. Any lost room key will require a door lock change for the safety of our residents. The charge for a lock change is $30. Your university ID photo is taken in the Student Affairs office in Medaille Hall anytime during the first 2 weeks of classes. The photo ID is used for your meal plan, Grif-fin Bucks and other uses around campus for the duration of your stay at Fontbonne. The first ID is free. Replacement cost is $10. You will receive a Go Cup in your bag of goodies if you are on a meal plan.

TEXT ALERT SERVICE Fontbonne provides a text alert service to all students, faculty and staff in the event of an emergency or other situation where it’s important to reach the campus community (weather closures). Residential Life encourages residents to sign up for the TEXT ALERT SERVICE as the university no longer provides phone service in the residence halls. Participation in this service is voluntary. IT INFORMATION You have the choice of having either wireless internet access or Ethernet. An 802.11g OR 802.11n wireless card (standard on laptops three years old or less) and an up-to-date anti-virus is needed to access the on campus wireless network . Network passwords are provided upon your arrival. Contact IT at 314.719.8095. IT is located in Ryan 322, 7:30 am - 5pm, M - F. Landline phone service is not available in the halls.

Cotta Hall has wireless internet. Connection instructions are provided on the router in each apartment. Please contact ResLife office with questions.

Residential Life correspondence between residents and staff will go to your Fontbonne email. It is now your responsibility to set it up and read it frequently.

Announcements, newsletters and other pertinent information will also be on the Residential Life website.

All university housing is furnished. There is limited storage space in your room and/or apartment. Talk with your room-mate about what you will bring to your room before buying any of the following: mini refrigerator, small microwave, coffee/tea maker, video game console, TV, carpet/rug.

Season appropriate clothing-summers are HOT; win-ters are COLD! Due to limited storage space, only seasonal clothing should be brought with you. Exchange clothing when the weather changes during holidays or visits home.

Extension Cords must have the UL or ETL approval to be used in the apartments. If they do not have this mark of approval they can-not be used.

Never keep an extension cord plugged in when not in use. The cord will still conduct electricity until it is unplugged from the outlet.

Do not use extension cords that are damaged or cut.

Never cover any part of an extension cord with rugs or other ob-jects while it is in use.

Don’t overload cords with too many appliances.

FYI-UNIVERSITY HOUSING CANCELLATION POLICY

Please remember that when you sign a housing agreement to live in university housing, you must live in the housing for one full academic year ( at least, August to May or as specified in the agreement). You will not be allowed to break the lease at mid-semester (December) unless you meet one of the conditions below, and only at the sole discretion of Fontbonne University:

Official withdrawal from the University Graduation Dismissal from the residence hall or university by the University Failure to make payments to the University when due If room becomes uninhabitable due to fire or other casualty Marriage June 1 is the deadline to cancel your room for the fall semester to receive a housing deposit refund.

A few policies about your room You are responsible for all furniture in your room and/or apartment. If you remove any University furniture without the permission of the Director, you assume responsibility for the replacement cost of the missing furniture at check out. Replacement costs are charged to your student account.

1. St. Joe & Medaille rooms are furnished with twin bed frames, mattress & box springs, desk & chair, dresser, bulletin board & mini blinds. St. Joe 2nd & 3rd floor rooms and all Medaille rooms have EXTRA LONG TWIN MATTRESSES (36X80). South-west, Cotta and St. Joe 4th floor have standard twin beds. Southwest and Cotta Halls have living room, kitchen furniture, and appliances.

2. St. Joe & Medaille-only Poster Mounts purchased in the bookstore, can be used to hang pictures, posters, etc. Poster mounts must be removed when moving out. Damage charges are assessed if any holes, nails or screws are left on the walls.

3. Southwest residents can use tacks, push pins or small nails; NO adhesive sticker should be used. Damage charges are assessed if any adhesive is used on the walls.

4. Rooms can not be painted. Resident accounts will be assessed $400 with the possibility of additional discretionary charges. 5. Room consolidations can occur during the year. Explanation in the Griffin Scratch. 6. Any furniture brought from home must have floor saver felt pads under them including refrigerators. 7. Attention St. Joe Residents! Use floor saver felt pads under any non-university furniture including mini-fridges to avoid

floor damage charges when moving out. 8. All rooms and apartments are inventoried. Resident student accounts will be assessed a damage charge. Recycle bins

are in your room for your convenience. Recycle cans, bottles, and paper then empty into the larger recycling bins on your floor. You will be assessed $25 for a replacement if bin is missing at the end of the year. The charge will be on your student account.
