Off-Site Support services offered at Quest Village are provided by Quest, Inc. Off-site services are available weekly to assist with community activities of daily living. Monthly fees apply.

Bronze | $220* *One of the following services assessed as being required Weekly bill-pay/budgeting assistance Weekly grocery shopping assistance Accompaniment to medical appointments up to two times monthly within a

20 mile radius

Silver | $330 Bill-paying and budgeting assistance (staff member will assist with the goal

of teaching the individual to do tasks independently) Weekly grocery shopping assistance (staff member will assist with creating

a grocery list and accompany individual to the store)

Gold | $660 Bill-paying and budgeting assistance (staff member will assist with the goal

of teaching the individual to do tasks independently) Weekly grocery shopping assistance (staff member will assist with creating

a grocery list and accompany individual to the store) Prescription-filling assistance (staff member will assist with the goal of

teaching the individual to fill tasks independently) Accompanying individual on medical appointments (within 20 mile radius/

up to 4x per month)

Housekeeping | $220 Housekeeping 1x per week to include bathroom, kitchen,

vacuuming and dusting

present and well Verbal reminders related to appropriate dress, hygiene

tasks and housekeeping Simple meal prep assistance (1 meal per day)

Silver | $660 AM/PM visual safety check to ensure individual is
