Weekly Payment Reimbursement Request This form is intended to assist employers seeking reimbursement for weekly income support payments made to a worker.

Use one form per worker. Please contact your claims agent if you need help completing this form.

This form must be submitted within three months from the date you paid the worker the payment you are seeking to be reimbursed.

1. Evidence of payment / pay records must be attached.

2. ReturnToWorkSA will only reimburse income maintenance payments which the worker was legally entitled to receive. It is the employer’s responsibility to ensure (by obtaining a written authorisation from Return to Work SA or the claims agent) that the worker was entitled to receive a particular payment of income maintenance.

3. Tick the ‘Nil earnings’ box if the worker did not perform any work during the week.

4. Only include amounts paid for work performed in your employment by the worker. Do not include any allowances or benefits relating to annual leave, sick leave or other leave type.

5. Weekly payment reimbursement requests and pay slips can be emailed to [email protected] for reimbursement.
