HSE Management System Weekly HSE Report Purpose: Scope: Instructions: Reporting period: Documen &AG W-RA &AG W-R This weekly and monthly reporting template has been developed with the pu reducing the complexity in reporting HSE statistics from project sites to Office. With the roll out of this template, all other templates for repor statistics from project sites to head office become obsolete. This template is applicable to all project sites in which LEI has overall and is responsible for site activities. Statistical HSE data reported in this template shall be compiled by Site Manager / In-charge for all projects. Further the data shall be reviewed checked by Lead - CNH. After compilation and checking, the data shall be presented to Director - Construction. From the different tabs provided, site HSE manager / in-charge shall fill site data in the tab of his particular site. Any changes or any informati not be filled in other tabs. Formulas for certain criteria have been inco in the excel sheet and these cells are locked to protect the formula. After roll out of this template, sites will send the HSE statistics throu single template only on weekly and monthly basis. The same data shall be presented to LEHQ for further compilation, hence sites must exercise caut while preparing and sending the weekly data. For January 2014, Week starts from Wednesday (i.e. 01 January 2014) hence reporting period will be from Wednesday to Tuesday. Each Wednesday (e.g. 22, 29_ _ _ etc. January 2014), by forenoon, weekly HSE statistics of pre week shall be submitted to [email protected] and pious.markose@li le.com for further compilation. Monthly HSE statistics shall be submitted to [email protected] an [email protected] by 2nd Date of the month for previous month. Note: Managament Information System (MIS) pertaining to HSE reports submi CLIENT / PMC remain unaffected with the roll out of this template.
