BUILD KNOWLEDGE: Do the Hopkins dementia module (go to http://www.peaconline.org, choose Internal Medicine from dropbox, and create account to log in) CHOOSE A PATIENT: Choose a patient in whom you suspect cognitive impairment in your continuity clinic (or the South Shore clinic if cannot find a patient in your own clinic) during this rotation. EXPERIENCE: 1) Perform a MOCA (www.mocatest.org - takes 10-15 minute. More sensitive than MMSE for diagnosis, but not validated for staging) or MMSE (See PDF on website) with this patient. What is their score? 2) Ask about their ADLs and IADLs. What are the impairments if any?

ANALYZE: 1) Do they have mild cognitive impairment (MCI)/mild neurocognitive disorder or dementia/major neurocognitive disorder? How severe is their dementia and why?

2) What kind of dementia do you think the patient may have and why?

3) Do you want any further workup? For example, labs or imaging?

4) What kind of management / interventions could you consider? Write down the top 3 interventions you think would be beneficial for this patient.

5) In your answer to question 4, did you include a cognitive enhancer? Why or why not?
