MAPLE MANOR ITEMS TO BRING:

1. Medications / Prescriptions / Medical Needs:If you are taking prescribed medication, you must bring them with you in their original bottles, or bring the prescription with you.If you need medical supplies (glucometer, inhaler, crutches, etc…) please bring those with you as well.Please hand all of the above to staff upon arrival.

2. MA / Insurance / Prescription Cards:If you have any of the above you must bring them with you. If you have recently applied for medical assistance please let admitting staff know where you applied.

3. Identification:You will need some type of identification (driver’s license, social security card, ID card, etc…) please bring those with you as well.

4. Personal Items:Bring casual clothing for about 5-7 days. Appropriate attire is expected. N o clothes with messages advertising drugs/alcohol, bars, vulgarity etc. are permitted. Bring shoes and clothing suitable for physical fitness. Staff has the right to say whether clothing is appropriate. Bring your own toiletries (soap, shampoo, deodorant, comb/brush, toothbrush, toothpaste, etc. Please do not bring products that contain alcohol (such as mouthwash, cologne, hairspray or body spray). Bring your own cigarettes/snuff.

5. Money There is a pop machine at the Manor and also we go to the stores once a week. You might want to bring change for the pay phone as it will use up calling cards very quickly. If you have a co-payment through your insurance you will be required to pay this. Also any medications not covered by insurances must be paid for. Any money or personal items brought to Maple Manor are your responsibility and not that of Maple Manor.

6. Pay stub (If working).

ITEMS NOT ALLOWED:

1. Walkmans, radios, video games, cellular phones, beepers, cameras, Blackberry, IPOD or personal CD player with headphones, personal computers or handheld organizers (check with staff if possible before coming with any electronic devices).

2. Non-recovery related literature (books, newspaper, magazines, word searches etc…3. Food or drinks of any kind. We supply food, drinks and snacks.4. Firearms, knives, scissors, letter openers or sharp objects of any kind.5. Please let staff know if your vehicle will be in our parking lot while you are in treatment. You will

be asked to hand over your keys to staff while in treatment, if you are leaving your vehicle in our parking lot.
