Selection Document Checklist: Candidate Application Form Resume Essay Teacher Recommendation 1 Teacher Recommendation 2

1. Candidate Application FormNote that this form includes a listing of community service hours completed as well as signatures of adult sponsors verifying your completion of these activities. Candidate forms submitted with this portion empty will be deemed ineligible.

2. ResumeA formal typed resume is to be submitted with your selection documents.

3. EssayEssays should be no more than 500 words in length, typed, double-spaced, 1in margins in Times New Roman 12 pt. font. Where appropriate, works cited must be in MLA format. Choose ONE of the topics below for your essay.

1. NHS looks for candidates who embody the characteristics of leadership, scholarship, service and character. In a well-organized and thoughtful personal narrative, tell us about something that happened in your life wherein you have exemplified one or more of these characteristics.

2. What outrages you? What are you doing about it?

3. Provide us with the title of a course that you could teach your peers. Give reasons why you think this course needs to be taught.

4. Tell us about your most exciting pursuit or your most disappointing failure and how the triumph or failure has influenced you.

5. "Honesty is the best policy, but honesty won't get your friend free birthday cake at the diner." - Overheard in the city of Chicago.

Does society require constant honesty? Is it problematic to shift the truth in one's favor, even if the lie is seemingly harmless to others?

6. Read the poem “Theme for English B” by Langston Hughes. (Available on the web at http://www.eecs. harvard.edu/~keith/poems/English_B.html). Saying who we are is never simple. Write a truthful page about yourself for us, an audience you do not know --a very tall order. Consider that each of us is of a certain age and of a particular family background. We have lived somewhere and been schooled. We are each what we feel and see and hear. Begin there and see what happens.

4. Teacher Recommendation 1 & 2Have two teachers complete the recommendation form (for a total of two recommendations).
