IF YOU ARE A PUBLIC SCHOOL SUBMITTING WITH TERMS1. Fill out the general account form in this packet.2. Get approved purchase order stating item, total amount and net terms
requested.3. Get Sales and Use Tax Exempt Form or Tax Exempt letter.4. Send forms to: [email protected] or mail to Phil Bryan.5. Wait for notification that your account is set up and your order is placed.
