18 months Field Care Manager Acuity Healthcare Solutions As a Field Care Manager I assist members, their caregivers and families in the navigation of health care system. Initially a perform a comprehensive evaluation of member’s health care needs, medical and social history is obtained and we developed a plan of care, which integrates member’s medical history, knowledge base and ability to care for ADL’s. At the end of my evaluation a plan of care is developed for each member according to his or her needs and each week needs are assessed an evaluated and at that time if new issues or problems are identified they are then added to the member’s plan of care. .
6 months Surgical Coordinator Florida Hospital Fish Memorial September 2007 –March 2008 assisting the surgical department manager in managing the surgical board with outpatients and inpatients until I was laid off during a “Work Force Reduction Program”
