Only a few of the ~34,000+ facilities that have selected Walgreens are beyond 75 miles from the nearest Walgreens location.
We intend to address these via a clustered
approach where possible.Even though there are 56 that are beyond 75 miles, many are clustered
together, and we’ll group together to streamline to reach those more
distant areas.

Timeline for 1st clinics to be scheduled/completed
Complicated timeline due to differences among the jurisdictions – we cannot activate scheduling until the jurisdiction is approved. Then it depends which facilities can be scheduled and that is determined by the jurisdiction.
Jurisdiction that allowed for 1A – SNF only we expect to do within 3 weeks
Jurisdiction has activated 1A and B then the SNF and LTC can be scheduled at the same time if they are on the same site
What to expect
When our team contacts you (Local Leadership or Vaccine Lead)• Exchange contact info• Set up dates for 3 clinics at your site
o Agreement is for 3 separate clinics 21 or 28 days apart
• Vaccine Lead will send a link to you to begin to enter residents/staff info being vaccinated into our COVID-19 Registration Portal must be complete prior to clinic• Name, address, DOB, gender• Race / Ethnicity
What the COVID registration portal looks like
• Consent can be obtained through your facility’s protocol
• The Vaccine Administration Record (VAR form) is required• This form is Walgreens legal record
that we keep as part of the patient record per CDC requirements as well a pharmacy regulations
• An authorized person can complete and sign on behalf of the patient

Vaccine Information
Pfizer
• Stored ultracold frozen -70°C• Minimal interval for 2nd Dose 21 days• Should not be administered before 21 days,
but can be administered after
Moderna
• Stored frozen at -25° to-15°C• Minimal interval for 2nd Dose 28 days• Should not be administered before 28 days,
but can be administered after

What can be signed before the clinic (once EUA fact sheet is a received) vs what sections must be signed the day of the clinic?
Sections A1, A2, C (Consent, Insurance):• Can be filled out / signed prior to the clinicSection B1, B2 (Screening):• Must be filled out the day-of the clinic. This section can be filled out by the patient or a
staff member at the facility who supports the Walgreens Team Member in screening the patient.
Section D (Day-of Admin Information); • Will be filled out by Walgreens Team Member the day of the clinics
Can consent be received via email from a family member? What proof must be provided on the VAR form for email consent?
Yes. Please follow your standard facility protocols for obtaining consent from family members or other authorized persons. When consent is received via email, the Walgreens VAR form still needs to be completed. The form indicates that an authorized person can complete the VAR on behalf on the patient.
Can consent be received via phone from a family member? What proof must be provided on the VAR form for verbal consent?
Yes. Please follow your standard facility protocols for obtaining consent from family members or other authorized persons. When consent is received via email, the Walgreens VAR form still needs to be completed. The form indicates that an authorized person can complete the VAR on behalf on the patient.
