    Allergies Vision Document

    Allergies should be categorized i.e. whether it is Drug Allergy or Food Allergy orEnvironmental Allergy.

    We need to give provision for entering the Allergy Name (Drug Allergy/FoodAllergy/Environmental Allergy). Please note that it should be Ajax-Search.

    Here each Allergy of Drug or Food or Environmental should be a coded value meansshould come from proper database.

    Drug Allergy should come from Drug Database. Food and Environmental Allergies should come from ICD database.

    Whatever we enter in Allergy Textbox, for each of this allergy, we need to give provisionfor documenting more detail related to it like Severity, Status, Reaction Type, Reaction

    Description, Onset, Recorded Date, Notes, etc.

    Severity, we can give below options for selection: Very Mild Mild Moderate Severe Life-threatening

    The Status should be Active or Inactive. We need to give provision for documenting what type of Reaction the patient get

    specific to Allergy and also note that we should not restrict to one reaction, it can be

    multiple so that need to give provision for documenting more than one reaction to

    each allergy. It should also be Ajax-Search, the reactions should come from the

    database in which where we have created frequent list of reactions.

    Reaction Description - If Physician want to document more information related toReaction Type, he/she can utilize this 'Reaction Description' section and it is not

    mandatory.

    Onset (Start Date) has below options, physician can select either one of those. Unknown Adulthood Childhood Date (We will provide a calendar to select exact or approximate date)

    Recorded Date - Date of Recording should be automatically populated by thesystem.

    Notes - If any additional information physician may want to document, he can utilizethis 'Notes' section and it should be Multiline.

    We should be in a position to Delete or Update or Cancel the Allergy till it not linked toany encounter.

    There are some scenarios like patient may have more than one allergy so that we needto give provision for documenting multiple allergies and all of these should be

    maintained in a proper order.
