Instructions: How to Upload a Photo Using MyHealthConnections

Thank you for joining our research study testing a virtual wound visit after you receive a defibrillator or pacemaker at UCHealth!

Please find instructions on how to send your doctors at UCHealth a picture of your wound for the virtual visit.

You can either download the UCHealth phone app or you can send a photo over your computer. You will find instructions for both in this packet – chose the method that is easiest for you.

Please note, you will need to login to your UCHealth online account. The study’s research assistant, Rebecca Guigli will be happy to find this information out for you!

Using your cell phone to upload a photo

1. Open the UCHealth app on your phone.

2. Login using your Username and Password.

3. You’re in your account! Now, click on “Messages” in the center blue bar.

4. Click on the bottom red box that says “Send a Message.”

5. Click on “To” box and select your cardiologist’s name.

6. Select the subject line “Non-urgent Medical Question.”

7. Almost there! Now, attach your photo. Below the Subject box, you should see a line saying “Attachments.” Click the box.

8. Click on “Attach image or video.”

9. Select if you’d like to “Use Camera” (if you haven’t taken a photo yet) or “Chose from Album” (if you’ve already taken the photo with your camera).

If you select “Use Camera,” you’ll take a photo from your camera!

You’ll want to make sure you put the camera in “selfie mode” by selecting the bottom icon of the camera with two arrows in it.

Once you take a photo and select “Use photo,” the photo gets added to your account.

If you select “Chose from Album,” you’ll use an existing photo in your phone.

Click on “All Photos” and select the photo you’d like to upload.

Once you select a photo and select “Use photo,” the photo gets added to your account.

You can send up to three photos if you’d like!

You can label the photos as “wound photo 1,” “wound photo 2,” and “wound photo 3,” if you decide to upload more than the required one picture.

10. You can ignore the text box! 11. Click on the “send” button in the top right hand side!

12. Congratulations, you did it! You should receive a confirmation that your image was sent on the next page. The Cardiology clinic will call you at your scheduled time to finish up your virtual visit!

You can send up to three photos if you’d like!

You can label the photos as “wound photo 1,” “wound photo 2,” and “wound photo 3,” if you decide to upload more than the required one picture.

Using your computer to upload a photo

1. Go to the website: https://www.uchealth.org/access-my-health-connection/ Or google “UCH My Health Connection”

2. Login using your Username and Password.

3. You’re in your account! Now, click on “Message to My Doctor’s Office.”

4. On the next page, you should see a message titled Send a Non-Urgent Message. Now, select “Two business days.”

5. You’re almost there! Next, select your cardiologist as the email recipient. 6. Select the subject line “Non-urgent Medical Question.” 7. In the text box, you can just type that you’re sending a photo of your

wound for the virtual wound study. 8. Lastly, attach your photo. Below the text box, you should see a line

saying “Attach an image or video” with a browse button. Click “Browse.”

9. Now, find that photo that you saved on your computer.

Select your cardiologist here

Select “Non-Urgent Medical Question”

“The image of my wound is attached”

Click “Browse”

10. You can enter a label for your photos, if you’d like. You can enter up to

three photos!

11. Once you’re done with that, just click “Send.”

12. Congratulations, you did it! You should receive a confirmation that your image was sent. The Cardiology clinic will call you at your scheduled time to finish up your virtual visit!

TIPS FOR TAKING A GOOD PHOTOGRAPH:

1) Hold the camera 8-12 inches from your skin 2) Do not use a flash 3) Take the photograph in a well-lit room 4) Be sure to show the entire incision and the area below the incision

(where the pacemaker or defibrillator sits under your skin)

Notice, the images are clear, and you can see the entire wound and the area around the wound.

Your incision may or may not have the strips like the photograph on the right

Examples of photos that we can’t use

In these photos, the wound and the area around the wound are cut off.

This picture is too dark for our nurses and doctors to evaluate the site.

Lastly, please make sure the camera is facing you when you take the photo!
