1. Welcome

1.1 Indications for Use A software system used with the Microsoft Kinect v2 intended to support the physical rehabilitation of adults in the clinic and at home. The system includes rehabilitation exercises for the upper extremity, trunk, and lower extremity with audio-visual feedback & graphic movement representations for patients as well as remotely accessible patient performance video. Patient assessment, exercise guidance and approval by a medical professional is required prior to use.

1.2 Warnings and Cautions Before using VERAClinic, please read all the following warnings and cautions. The VERA System and its associated features are not intended, designed, or implied to diagnose or prevent any condition or disease, to ascertain the state of your patient's health, or to be a substitute for professional medical care.

Warning statements indicate conditions or practices that could result in serious injury with the use or misuse of the VERA system. Caution statements indicate conditions or practices that could result in minor or moderate injury to the user or that could damage equipment or other property.

1.2.1 Clinician Warnings Warning: When you build a patient’s Therapy Plan, carefully select the appropriate physical therapy exercises. If a patient performs exercises based on an incorrect Therapy Plan, the patient could become injured. Warning: When you build a patient’s Therapy Plan, thoroughly evaluate and consider the patient's functional status and health history.

1.2.2 Patient Warnings and Cautions

Warning: The VERA system is designed for patients who are at least 22 years old and at least five feet tall. Caution: Federal law restricts this device to sale by or on the order of a physical therapist or physician. Caution: Keep your VERA username and password confidential and in a secure location. If you share this information with others, your personal profile can be compromised. Caution: Performance information, such as 'adherence' and 'repetitions,’ included in the VERA system software and the algorithms for evaluating patient movements have not been reviewed

by the FDA. Clinicians are advised to use standard assessment practices as well as using the video recording of the rehab session provided by the VERA software to complete patient evaluations. Caution: The VERA system may not perform correctly for individuals with

• Non-standard skeletal features. • Orthotic devices or assistive devices that significantly deform an individual’s skeletal

features, such as orthotics with spring-loaded finger extension systems or other protruding parts.

Caution: Non-skeletal deforming orthotic devices, such as standard braces or splints, may be used with VERA. However, for users with reflective orthotic devices, it is recommended that the user wears non-reflective clothing over the device. Caution: While the VERA system detects forward and lateral trunk compensation, it does not detect all movement compensation strategies, such as scapular compensation. Patient performance information stored and provided by the VERA system is intended to serve only as an aid to a clinician when considering a patient’s progress. It is not intended to replace an in-person physical therapy assessment. Engagement with any prescribed exercise requires prior patient assessment, exercise guidance, and approval by the medical professional, who is responsible for assuring patient-exercise suitability and performance adequacy. Caution: Clinicians are advised to evaluate each patient’s performance ability and assess the risks, vulnerable populations, and risk mitigation options associated with using the VERA system prior to recommending the system to a patient.

1.3.1 Support Videos 1. To get more detail on

how to use the system and how certain features work, use the dropdown with your username in the top right corner of the navigation bar and click on Support.

2. Search for a specific video by typing its topic or description in the search bar.

1.4 Signing In 1. Enter your username (email) and password on the

login screen and click Sign In. 2. If you forgot your password, select the Forgot

Password link to have it reset. 3. Select the ‘Remember me’ checkbox if you would like

the system to remember your login information.

Note: For proper functioning of the VERA software, make sure your computer is using an internet connection that is 1.5 MBPS or faster.

1.5 Reset Password 1. From the Login screen, select Forgot password? 2. Enter your email address and click Send Reset Link. 3. Check your inbox or spam folder for an email with a link to

create a new password. Follow the Password Guidelines to create and Save your new password.

2. Main Navigation Bar

1. Click Patients to access the Patient Dashboard. 2. Click Protocols to access the Protocols List.

3. Click Clinicians to access the Clinicians List. 4. Use the search bar to find patients by name. 5. Click on Add Patient to add a patient to the system. 6. Click on the video camera icon to open the Therapy Visits application in a new browser tab. 7. Click on the bell icon to open the Notification Center and view any notifications. 8. Click on the arrow next to your name to open a dropdown that allows you to manage your

password, access the Support page, view this user manual, or log out of the application.

2.1 Navigation Drop-Down

2.1.1 Password Management 1. Access the password

management screen by accessing the dropdown and selecting Password.

2. Enter your old password and your new password twice. The system will automatically check if the new password meets the password guidelines. Click Save once the guidelines are met.

2.1.2 User Manual You can access the user manual in two ways:

1. Click on your name in the top right of the navigation bar to open the dropdown and click on User Manual.

2. Scroll to the bottom of the Support page and click on Download User Manual.

2.1.3 Logging Out When you are finished with your session, logout by accessing the dropdown with your name in the top right of the navigation bar and click Logout.

2.2 The Notification Center 1. Select the bell icon from the navigation bar to open the

Notification Center. 2. A notification is triggered when:

• A patient completes a functional assessment that needs to be reviewed.

• A patient gets reassigned to you. 3. Click on the link in the notification to load the screen where

the appropriate action can be taken. 4. Remove a single notification by clicking the X or remove all

notifications by clicking Clear all.

2.3 The Therapy Visits Application 1. Select the video camera icon from the navigation bar to open the Therapy Visits application. 2. After waiting for the application to load, click Continue. The application will open in a new tab. 3. In the new tab you will enter the waiting

room. Click Join once a patient becomes available.

4. When finished, simply close the browser window to close the application.

3. Patients

3.1 An Important Note on Patient Data If the clinician doesn’t see any data for a patient after three successive days, during which they had exercises assigned and the patient reports that they have been adhering to their home exercise plan as assigned, instruct the patient to refrain from turning off the device until all data has finished uploading. If you feel that the internet connectivity is inadequate, please contact Reflexion Health Support.

3.2 The Patient Dashboard On the Patient Dashboard, you can sort, filter, and search for your patients, view the status of their therapy plan and assessments, and track their activity.

1. Filter: By default, the Patient Dashboard displays all Active patients assigned to you (My Patients). Use the filters to toggle which set of patients you want to view.

2. Search: Find a specific patient using the search field. The results shown will vary based on the filters selected. You can also use the search bar in the top left of the main navigation bar.

3. Patient Detail: Click on a patient’s name to open the Patient Detail for that patient.

3.2.1 Patient Dashboard Headers 1. Patient column – displays the name of the patient and the name of his/her clinician.

1.1. Click on the patient’s name to access the patient’s detail page or the clinician’s name to access the Clinicians page.

1.2. If the patient has a care team, an icon will display next to the patient’s name. Hover over the icon to see the members of the Care Team.

2. Alerts column – shows how many alerts the patient has and the priority. Click on the alert to open the Alerts section of the patient’s detail.

2.1. High Priority alerts are identified by a red color. 2.2. Moderate Priority alerts are identified by an orange color.

3. Plan Status column – shows the patient’s therapy plan status. 3.1. If the patient does not have a therapy plan assigned to them, click Add Plan to add a

therapy plan to the patient. 3.2. If the patient has a therapy plan and is not on hold or discharged, click Edit Plan to

edit the patient’s therapy plan. 3.3. If the status is On Hold or Discharged, you can still edit the patient’s therapy plan

through the Therapy Plan screen on the Patient Detail page. 4. Assessments column – shows the status of the Functional Assessments and whether they are

scheduled, missed, or ready to be reviewed. Click on the status to display more information on the assessments and review them, if necessary.

5. Last Activity column – displays the last time the patient interacted with the VERA system. 3.2.2 Sorting by Header

3.2.2.1 Default Sorting By default, the patients on the Patient Dashboard are sorted by their alerts status in the following order with unread alerts appearing first in bold over previously read alerts:

1. New high priority alerts 2. New medium priority alerts 3. High priority alerts that have been read, but are still open 4. Medium priority alerts that have been read, but are still open 5. No alerts in order of the most recently viewed patients

3.2.2.2 Sorting by Heading Click on a heading in the Patients list to reorder the patients according to the specific heading. Click on the heading again to reverse the order. • Patient: reorders patients alphabetically by last name. • Alerts: reorders patients according to the default sorting in the previous section. • Plan Status: reorders patients according to their therapy plan status in the following order: On

Hold, Edit Plan, Add Plan • Assessments: reorders patients according to their assessments status in the following order: To

Review, Missed • Last Activity: reorders patients according to their most recent interaction with the VERA system.

3.3 The Patient Detail Section Click on a patient’s name to load the Patient Detail section. Use the Patient Navigation Bar to access the following areas of the section: • Alerts: view and manage the patient’s alerts and recent activity • Therapy Plan: view and manage the patient’s progress summary and exercise plan • Assessments: view the patient’s functional assessment summary • Reports: view weekly, monthly, and episodic reports and generate one-time reports • Profile: view and edit the patient’s profile information

3.3.1 Patient Information At the top of any Patient Detail section, you can view the patient’s status, diagnosis, surgery date, and much more. Click on the down arrow to expand the section to display more patient information.

3.3.2 Alerts

3.3.2.1 Alert Types There are 5 types of alerts:

1. Health Alerts – triggered when the patient responds ‘Yes’ to a Health Status Check-In question. 2. Pain Alerts – triggered when the patient reports a high pain level (5 or greater). 3. Exercise Alerts – triggered when the system detects exercise trouble or a low completion rate. 4. Surgery Prep Alerts – triggered when the patient responds to one or more of the home prep

questions with “I need assistance” or “I need more time”. 5. Activity Alerts – triggered when the patient has low average daily exercise time, low engagement

with the software, or low exercise plan completion. 3.3.2.2 Open Alerts Section In the Open Alerts section, click on the alert’s link to take the appropriate action on the alert.

1.1. Once you act on the alert, you can ask to be reminded of the alert in 24 hours, 48 hours, or on your next login.

1.2. Click on the checkmark icon to clear an alert. 1.3. To clear all open alerts, click ‘Clear All’.

3.3.2.3 Open Alerts Notification If you have previously set reminders for an alert, you will get the reminder immediately on your next login. Click on Review Alerts to go to the patient’s Alerts page.

3.3.2.4 Recent Activity Section In the Recent Activity section, you can filter the types of activities by using the drop-down labeled ‘All Activity’. Activity items may also have links such that you can take appropriate action.

3.3.3 Therapy Plan

3.3.3.1 Therapy Plan Overview From the Therapy Plan page, you can view a summary of the patient’s completion and quality scores for their whole episode of care as well as the results of the patient’s last functional survey.

a. Completion – the percentage of reps completed by the patient.

b. Quality – the percentage of reps in which the patient did not receive feedback.

c. Survey Scores – recent survey scores.

3.3.3.2 Activity Summary 1. To view a Weekly Activity

Summary, scroll down on the Therapy Plan screen and select a date range OR Plan number in the purple bar.

2. Completion and quality ratings are shown in a bar graph for each day of the selected week/plan.

3.3.3.3 Home Therapy Plan

1. Scroll down further on the Therapy Plan screen to find the Home Exercise Plan section.

2. Each exercise prescribed will have a corresponding exercise tile.

2.1. The color of the bar at the top of the exercise tile represents a measure of progress. • Red – Quality OR Completion percentage is less than 50% • Yellow – Quality OR Completion percentage is between 50% and 70% • Green – Quality AND Completion are more than 70%

2.2. Shown at the bottom of the tile are the average completion and quality percentages as well as the assigned frequency and number of reps per day for the individual exercise.

2.3. If the patient has reported pain, you will see the pain range reported on top of the tile.

3. Click on an exercise tile to watch a video of the patient performing the exercise.

4. View additional session details by selecting the Show full session details link.

5. A session dot for each performed session acts as an indicator of overall progress for the exercise:

• Red – Quality OR Completion percentage is less than 50%

• Yellow – Quality OR Completion percentage is between 50% and 70%

• Blue – Quality AND Completion are more than 70%

3.3.4 Assessments

3.3.4.1 Functional Assessment Graphs 1. Scroll down on the Assessments

screen and find the Assessment Graphs section.

2. Select whether you want to view results for Tests or Surveys.

3. From the drop-down, you can select which functional assessment to plot on the graph.

4. The line graph is on a weekly timescale starting with the Episode Start date at week 0. 5. Click on the Edit Plan button or Print icon button in the purple Assessments bar to change or print

the patient’s therapy plan. 3.3.4.2 Functional Assessment Summary

1. Scroll down on the Assessments screen and find the Functional Assessment Summary section.

2. Select whether you want to view the summary in a calendar view or a list view.

3. The 35-day calendar view shows the status of assigned functional assessments. Click on a day to see the activities that are scheduled or were completed on that day.

4. The list view shows the score, type, and change for all assessments. Use the filters to find the assessment you wish to review.

3.3.4.3 Scoring Functional Assessments When a patient completes a functional assessment, you will receive a notification in the application asking you to review and score the assessment. You can also score the assessment when you see the To Review icon ( ) from the Patient Dashboard or the Functional Assessment Summary section on the Assessments page of the patient’s detail.

1. Click on the notification to open a list of the patient’s assessments. 2. Click on an assessment that needs to be reviewed to score the assessment. 3. If the assessment to be scored is an

extension test, use the measurement tool to help you accurately evaluate the angle of the joint.

4. Hover over the image and click to add a point. The tool will advise you where to place the next point on the patient’s body.

5. Add 3 total points to generate an angle (the created angle will automatically populate into the “Score” field). Click Save to apply the score.

6. To remove points, hover over and click on the point when you see a minus sign.

7. Click on the Clear All Points link below the image to remove all points at once. 8. To score other assessments, watch the video of the patient performing the assessment and then

Edit or Confirm the score.

3.3.5 Reports 1. From the Reports section,

you can access weekly, monthly, episodic, or one-time reports. Click on the report to open it.

2. Reports are autogenerated except for one-time reports, which can be created by clicking on the Create Report button. Select a date range and click the Run Report button. A full episode report is generated when the patient is discharged.

3. Add comments to any report by clicking on the icon in the Report Notes column. 4. Download a PDF version of any report by clicking on the icon in the PDF column. 5. Access text versions of each report section by clicking on the icon in the Copy & Paste column.

This is useful for pasting information into your EMR. 3.3.6 Profile

1. In the Profile section, you can update the patient’s information.

2. Be sure to Save Changes.

3.4 Managing Patients

3.4.1 Managing Patients from the Patient Dashboard 1. Click on the extra options

icon ( ⋮ ) in the far right column of the Patient Dashboard to open the Manage Patient menu.

2. From this menu, you can manage the patient’s profile, create a new plan, request a kit install, email the patient, discharge the patient, and place or cancel a patient’s hold.

3.4.2 Managing Patients from the Patient Navigation Bar

After clicking on a patient’s name to open the Patient Detail section, you can access the same Manage Patients menu by selecting the Manage button from the patient navigation bar.

3.4.3 Discharging/Reactivating a Patient If your patient is ready to be discharged, follow these steps:

1. Select Discharge Patient from the Manage Patient menu. 2. Click OK to confirm. 3. Cancel the discharge by selecting Reactivate Patient from the Manage Patient menu.

3.4.4 Placing/Canceling a Patient On Hold If your patient will not be exercising for a while, follow these steps to place a patient on hold:

1. Select Place on Hold from the Manage Patient menu. 2. Select a start date. 3. For the end date, choose a specific date, no end date,

or the next time the patient logs in. 4. Click Save. 5. Cancel the hold by selecting Cancel On Hold from the

Manage Patient menu.

3.5 Adding a New Patient 1. Click Add Patient in the top navigation bar. 2. In the Patient Information section, enter information

for at least the required fields outlined in purple. 3. Select Next to proceed or Quick Save to create the

patient with just the required info. 4. In the Clinical Information section, select a clinician

and enter known medical patient info. 5. In the Kit Installation section, enter the patient’s

shipping address. 6. On the Confirm page, select Edit to access the other

pages again or Save to finish.

3.6 Reassigning a Patient to a New Clinician 1. Navigate to the patient’s profile by selecting Profile from the Manage Patient menu. 2. In the Clinical Information section, select the Clinician dropdown, and then select the clinician you

would like to assign your patient to. 3. Assign other clinicians in the system to the patient’s care team by typing their names into the Care

Team field. Make sure to Save Changes.

4. Managing Therapy Plans

4.1 Types of Exercises 1. Tracked Exercises – As the patient performs the exercises, the Kinect sensor will track joint

positions across space and time to determine whether a repetition was completed. 2. Self-Report Exercises – The Kinect sensor will not track the patient’s movements. The patient must

keep count of the number of reps completed and report reps accurately. Self-report exercises are labeled with an “SR” in the patient’s therapy plan.

4.2 Creating & Editing Protocols 1. Select Protocols from the top navigation bar. 2. Click the Add Protocol button to create a new protocol or click on the extra options icon ( ⋮ ) on

the right to Edit or Delete the protocol. You can also preview the exercises in the protocol.

3. The Create/Edit Protocol screen will load. Select to view Exercises or Assessments. Add activities

to the protocol by clicking on the cards. You can search for the activity or use the filters provided to easily find the right type of activity to prescribe to your patient.

4. The activities you select will automatically appear in the section below. 5. To change the order that the activities are prescribed to the patient, select Reorder at the top of

the section and then click and drag the activities into the desired order.

6. Adjust the bias, sets, reps, daily frequency, and weekly frequency for each exercise. For each functional assessment, you can adjust the frequency and bias.

7. If the exercise is tracked, you can assign it as a self-report exercise by selecting the SR checkbox. 8. Click the Edit Options link to adjust additional exercise options (equipment, rest times).

9. When you are done, scroll to the bottom of the page and click Update Current Protocol if you

are updating a protocol or Save as New Protocol if you a creating a new protocol. 10. For new protocols, enter a name and description and click Save.

4.3 Applying a Protocol to an Existing Patient 1. When you navigate to a patient

without a therapy plan, you will be prompted to assign a therapy plan to the patient. Click Assign Plan.

2. Select Create New Plan to load the Create Protocol screen or select Apply a Protocol to apply an existing protocol to the patient.

3. If you selected Apply a Protocol, select a protocol from the drop-down and click Apply.

4.4 Creating or Editing a Therapy Plan for an Existing Patient 1. To create or edit a Therapy Plan for an existing patient, open the patient's Therapy Plan. 2. Select the Edit Plan button from the purple bar above the Activity Summary section. Select

Create New Plan or Edit Current Plan from the dropdown.

4.5 Managing Existing Protocols

4.5.1 Finding & Viewing Protocols 1. Click the Protocols tab in the top navigation bar. 2. Search for protocols by name using the search bar. 3. Click Preview to see a summary of the protocol in-line.

4.5.2 Editing & Deleting Protocols

1. On the Protocols screen, click on the extra options icon ( ⋮ ) on the right. 2. Choose Edit to load the Edit Protocol screen or choose Delete to delete the protocol.

4.6 Printing a Patient’s Therapy Plan 1. View a printer friendly list of the patient’s therapy plan in multiple ways:

• Select the print icon in the Therapy Plan bar.

• Select the Print link at the top of the Home Therapy Plan section.

2. A new browser

window will open and list the current assigned exercises and functional tests. Select the Print button.

5. Managing Therapy Plans

5.1 Editing a Clinician’s Information 1. Click Clinicians in the

top navigation bar to access the Clinicians screen, where you can create and edit active clinicians in the system.

2. Click on the extra options icon ( ⋮ ) on the far right of the clinician in the list and click Edit.

3. A dialog box will load with the clinician’s information. Update the information as desired.

4. Click Save Clinician to save the changes.

5.2 Creating Clinicians 1. Click the Add Clinician button on the Clinicians page. 2. Fill out at least the highlighted fields with the new clinician's

information. 3. Click Create Clinician to add the clinician to the system.

5.3 Emailing a Clinician Warning: Email is not a secure/approved means for transmitting Patient Identifiable Information or Patient Health Information.
