FIRST DIAGNOSIS CANCER BENEFIT$10,000 option also available

DOCTOR’S OFFICE VISITS:Physician’s Office Visits – 10 per year each insured adult, 5 per calendar year for all children combined; limit one visit per week per insured.

DAILY HOSPITAL BENEFITS:Daily hospital confinement cash benefit from the first day up to 365 days per lifetime (Dependent children limited to $100 per day)

Pays in addition to other coverages.

Pays benefits directly to the Insured, where assigned. After medical treatment due to sickness or accident, the Insured must file a claim.
