The purpose of this notification is to add a dosage chart to the Directions for Use.Please note that the addition of this chart does not affect the dilution ratio or the minimum\maximum usedilutions and simply provides a tabular presentation of use directions which have already been approvedby the Agency.

    In support of this submission I have enclosed the following documents:

    1) Application for Pesticide Notification, which includes a signed statement certifying compliancewith PR Notice 98-10,

  • PRECAUTIONARY STATEMENTS

    HAZARDS TO HUMANS AND DOMESTIC ANIMALS

    DANGER: CORROSIVECauses irreversible eye damage and causes skin burns. Do notget in eyes, on skin, or clothing. Wear goggles or face shield andrubber gloves when handling this product. Harmful if swallowedor inhaled. Avoid breathing dust. Wear protective eyewear(goggles, face shield or safety glasses). Wash thoroughly withsoap and water after handling and before eating, drinking,chewing gum, using tobacco or using the toilet. Removecontaminated clothing and wash clothing before reuse.Prolonged or frequently repeated skin contact may causeallergic reactions in some individuals. When prolonged orfrequently repeated contact could occur, use chemically resistantgloves and full body clothing.

    PHYSICAL AND CHEMICAL HAZARDSOxidizing agent. Contact with combustible materials may causefire. Wet product decomposes exothermically and may causecombustion of organic materials. Pressure may increase due todecomposition in confined spaces or containers.

    ENVIRONMENTAL HAZARDS/Vhen applying PAK™27 to water bodies, avoid solid particlesfrom falling on nearby ground where birds and feed may bepresent. Undissolved particles of sodium carbonateperoxyhydrate may be corrosive to beaks of birds.

    Do not discharge effluent containing this product into lakes,streams, ponds, estuaries, oceans or other waters unless inaccordance with the requirements of a National PollutantDischarge Elimination System (NPDES) permit and thepermitting authority has been notified in writing prior todischarge. Do not discharge effluent containing this product intosewer systems without previously notifying the local sewagetreatment plant authority. For guidance contact your State WaterBoard or Regional Office of U.S. EPA. For additional information,refer to the product Material Safety Data Sheet.

    DIRECTIONS FOR USEIt is a violation of Federal law to use this product in a manner

    inconsistent with its labeling. Do not allow the product to comein contact with other pesticide products, cleaners or oxidative

    agents.

    Precaution: Decaying algae can deplete dissolvedACC6Dt6d oxygen which can lead to fish kill, if excessive. Apply

    with 8 to 10 hours of daylight remaining. Do notreapply within 48 hours. If treating a large lake orheavy bloom, treat 1/3 to 1/2 of the area and wait 2 to3 days before treating remainder of water. Consult withthe State agency with primary responsibility forregulating pesticides before applying to public watersto determine if a permit is needed.

    IF IN EYES: - Hold eye open and rinse slowly and gently with water for15 -20 minutes. Remove contact lenses, if present, after the first fiveminutes, then continue rinsing eye.- Call a poison control center or doctor for treatment advice.

    IF ON SKIN OR CLOTHING - Take off contaminated clothing.- Rinse skin immediately with plenty of water for 15 - 20 minutes.- Call a poison control center or doctor for treatment advice.

    IF INHALED: - Move person to fresh air.- If person is not breathing, call 911 or an ambulance, then give artificialrespiration, preferably mouth-to-mouth if possible.- Call a poison control center or doctor for further treatment advice.

    IF SWALLOWED - Call a poison control center or doctor immediatelyfor treatment advice.- Drink promptly large quantities of water.- Do not induce vomiting unless told to do so by a poison control centeror doctor.- Do not give anything by mouth to an unconscious person.
