All changes should be entered as new records

Enter a new record to change any part of a current record

Never delete records – this alters history

Only update records to:

Correct typos (i.e. Farifield can be corrected to Fairfield); or

Update to a bad address;

Expire a significant other

Keyboard command “1 (space) a”

Every client should have an address record

If necessary enter a homeless address and identify as a “bad address”

Addresses and phones numbers should be verified at each appointment during check-in/registration

It is not necessary to receive a form from direct service provider (i.e. clinician or specialist) to update an address or phone number

Enter a new record for each new address or phone number – if a phone number changes enter a new address record

Number, Name, Type, City,

State and Zip Code

Include Apartment # when

applicable

Whenever possible try to update during the episode

opening/update so effective date will match episode

Always try to complete at a minimum: Number, Name, Type, City, State and Zip Code

Include Apartment # when applicable

If entering a ½ Street Address (i.e. 123-½ Any Street) put the entire address in the Name field.

Census Tract field is not used

To enter an additional phone number such as a cell phone use the Significant Others screen (see sample slide in Significant Other section)

Updates should be entered as new records unless correcting a typo

Every child should have at least one significant other

It is necessary to receive a form from direct service provider (i.e. clinician or specialist) to update significant other record

Exception: parent(s) upon first registration

If entering a record based on a Release of Information

(ROI) form – must enter expiration date

Clerical is not responsible to verify ROI completed if receive form requesting update by direct service provider
