SEIC, 25/08/08LNG supervisor conducted work on a lower jetty without raising a work permit. He probably climbed outside of barriers, slipped, fell down in the water and drowned. His body was found later 2 km away in the open sea. Raising a permit to work outside of safety barriers would have taken time, but could have saved his life.
Rule 1. Work with a valid work permit when required

TNK-BP, 19/05/05Unsupervised contractor crew started tank cleaning job without prior gas testing. When the first contractor who entered the tank lost consciousness, the other 3 rushed to save him one after another without any PPE, which resulted in two deaths.

Prop shaft rotates when engine is running IP’s clothes before removal
SPD 27/02/09 An operator decided to clean the equipment while the engine was running. His clothes got caught by rotating shaft and he was hit against the structure which caused his immediate death

SEIC 02/02/07 A driver was speeding on a slippery road which resulted in a collision with an upcoming truck and driver’s death. The driver was wearing a seat belt, but the belt was obstructed with a screw to make it more comfortable and did not function properly.

SEIC, 28/01/07Kamaz crew bus with two people on board crashed through a road barrier, traveled 50m in the air and came to rest at the bottom of a steep slope. The driver was killed, the passenger survived. Both were intoxicated with alcohol

SEIC, 13/09/044x4 Vehicle with four people on board at a speed of 50 km/h drove into an unexpected road washout. Three people were wearing seatbelts and had minor injuries. The forth one, sitting in the rear row, did not put on her seat belt and had multiple injuries and unfortunately died 2 months after the crash..

SPD, 11/04/07 A driver was working for long hours without proper resting, which resulted in him getting asleep while driving. This caused a head on collision with an truck and driver’s instant death. A JMP prescribes driver’s rest hours. Following a JMP could have saved the driver’s life.
