e MyStory Journey: Capturing each child’s story to personalize the care we provide pediatric patients and their families Evidence strongly supports the importance of capturing the personal story and preferences of a child admitted to an acute care hospital setting. At University of Minnesota Amplatz Children’s Hospital, we have developed an initiative, called MyStory, that recognizes not just the patient, but the child as an individual. Objectives: families that will be an Always Event when caring for children — MyStory o MyStory as an Always Event is a interprofessional effort to capture each patient’s “story.” o Document the “story” in a convenient tool within the electronic health record. o Use the patient’s “story” to personalize the care we provide to patients, and to involve them in care planning and decision making. patients by learning their story and engaging them in care planning and decision making.
