II. TEACHING ACTIVITIES:

1. Teaching Activities Opening Statement: My strengths in teaching are in course design, preparation, and synthesis of content for presentation. I enjoy planning new courses in emerging areas of science and developing curriculum to meet the needs of the learner audience. My goal as a curriculum designer is to match the content, mode of delivery, and learning exercises to the level of receptiveness of individual learners. Part of the planning process includes thoughtfully articulating the competency objectives of the course, as well as each lesson and exercise. Also important is to develop evaluation methods which appropriately test the level of competency I seek to instill in learners and provide actionable feedback for improvement. I have training in facilitation and am experienced in creating a welcoming classroom environment, soliciting feedback, and managing group interaction in a classroom setting. My mid-level career teaching achievements, by criteria, are as follows: Developing and teaching major components of courses At KUMC, I have been course director for two graduate-level, full-semester courses, supporting the needs of two separate degree programs. In Introduction to Healthcare Operations (HP&M 850), I teach the major approaches to performance improvement: Six Sigma, Lean, the Institute for Healthcare Improvement approach, and Implementation Science. I incorporate hands-on learning activities, including coordinating placement of students in Kansas City area healthcare settings so they can develop performance improvement skills by applying strategies learned to real health system challenges. Student assessments for this course have steadily increased since I took over the course and the real-world projects consistently earn high appraisals. In the doctoral program, I designed a new full-semester length advanced course Introduction to Health Policy (HP&M 901), which students rate highly. Part of the responsibility of teaching this core course is to ensure students gain fundamental competencies in health policy research, the demonstration of which is required for their continuation in the doctoral program. Competencies are evaluated by administering the comprehensive Doctoral Competency Exam 2, for which I write a bank of policy questions for selection and evaluate the competencies of students who have taken the exam (10 students to date). Developing significant evaluation techniques For the Master of Health Services Administration program, I provide a unique opportunity for students in our program to demonstrate their competencies across the program by sitting for a Lean Six Sigma Green Belt certification exam, offered annually. I recently expanded the exam to allow experienced performance improvement professionals in the KU Health System to also sit for the exam. To date 29 students have earned this distinction, highly desired by employers. Supervising graduate students and participating on dissertation committees Our department requires doctoral students to sit for three separate qualifying exams prior to sitting for their oral exam and defending a dissertation. I have served on the Qualifying Exam 2 Committee since 2014, responsible for assessing all students' health policy competencies. In addition to assessing these core competencies, I have served on five additional, individualized Qualifying Exam 3 committees, which test students' competencies in a specific subfield. I chaired one Qualifying Exam 3, writing a subfield exam on health economics with input from colleagues. I have served on numerous dissertation committees, including serving on comprehensive oral exam committees for three students and chairing the dissertation of one student, who graduated in May. In addition, I have served as mentor on two students’ Agency for Healthcare Research and Quality (AHRQ) R36 dissertation applications, a prestigious national award not previously sought by students in our program. Advising, mentoring 25 graduate and post-graduate learners I also serve as a research mentor and committee member to many students in the social sciences, medicine, nursing, and health professions as well as urology residents and fellows, providing enriching research opportunities in cancer care delivery. My participation on the selection committee and as a mentor in the Department of Urology's Society of Urologic Oncology-recognized Fellowship Program, broadens the program's training opportunities to include health services research and helps to build its national reputation as an increasingly competitive program. Together these research experiences have resulted in numerous regional and national presentations, publications in high impact journals, and enduring collaborations with trainees beyond graduation. Developing significant educational and curricular materials Finally, our team has made innovations in continuing education by designing a continuing education activity co-sponsored by the South Central Section of the American Urological Association, which has been highly regarded by the practicing urologists who enrolled. The pioneering curriculum is designed on the principles of transformative learning theory and uses innovative experiential learning and reflective writing exercises to build skills to provide guideline-concordant care.

I provide peer review and research mentoring through examination and constructive feedback on ongoing research of my academic peers and learners throughout KUMC and beyond. I am also highly committed to contributing to the advancement and sustainability of the field of implementation science through local, regional and national service in which I seek to advocate for the field, advise on infrastructure needs to advance the field, and share opportunities for entree and inclusion into the field. I have met several criteria for mid-level career service activities and have embarked on established-career level activities. Service to Medical Center Educational and Research Oversight Since joining KUMC, I have served on several departmental education committees for my own department and the Department of Urology, providing oversight for the Master’s Program in Health Services Administration, the doctoral Program in Health Policy & Management, and candidate selection for the Society of Urologic Oncology Fellowship. I have contributed as a judge for student and faculty poster sessions and am a standing reviewer for the

American Cancer Society Institutional Research Grant. As a member of the KU Cancer Center, I have contributed to training grants, mentoring of peer faculty, and currently serve on the Implementation Science faculty search committee. Implementation Science Expertise Provided to Regional, State, and National Initiatives I lend expertise to several inter-institutional, state and regional policy and practice organizations, including the Children’s Mercy-KUMC Implementation Science Working Group; the newly established Quality, Value and Innovation Consortium, which matches implementation science expertise to hospitals and health systems in our region to improve the health of Kansans and Missourians; and the regional Health Policy Interest Group which brings together funders, policy makers and researchers to address relevant health policy issues. I have also provided grant review service to a statewide foundation seeking to transform their funding portfolio to address social determinants of health. Most recently, I was invited to participate in the NCI's Implementation Science Cancer Consortium, a three-day planning activity to identify priorities and infrastructure needs for the field. Contributions to National and International Professional Organizations and Initiatives Among other professional society involvement at the local, regional, national and international level, I have taken an active role in the Society for Medical Decision Making, serving as an abstract reviewer for their annual meeting and worked with the American Society of Clinical Oncology to develop a survey of cancer programs. I am an active participant in the cancer cooperative group SWOG, serving on the Cancer Care Delivery Committee and the Survivorship Working Group of the Genitourinary Committee. I have been invited to serve as a consultant in implementation science by other universities. Contributions to National and International Peer Review I have contributed 18 scientific reviews to 15 clinical and social science journals, including Health Affairs, the top health policy journal in the field; Medical Care, the premier health services research journal; and Cancer, a leading oncology clinical journal. In 2019, I was invited to serve on the founding Editorial Board of the new international journal Implementation Science Communications. IV. RESEARCH AND SCHOLARLY ACTIVITIES:

1. Research and Scholarly Activities Opening Statement: Since coming to the University of Kansas, I have developed a research portfolio building on the strengths of KUMC clinical collaborators and institutional resources, forging an innovative research niche which focuses on improving cancer care delivery among cancer specialists, particularly, the non-oncology providers who deliver more than 20% of cancer care in the United States. As such, using large claims databases, I have identified provider-, practice-, and policy- determinants of high-quality urological cancer care. I have adapted methods used to engage primary care physicians in quality of care interventions for urological surgeons. In particular, I have applied these methods to engaging community urologists in offering cancer clinical trials to their patients, consistent with national guidelines, and am embarking on a national trial of our Kansas-developed intervention through the NCI clinical trials cooperative group infrastructure. My work is increasing the reach and utilization of vital scientific progress by facilitating its implementation and sustained use in community settings, where 80% of patients get their care. This work is exemplified by my recent National Institute of General Medical Sciences-funded project to implement precision medicine in community oncology, a mixed methods implementation research study to identify theoretically-based, effective interventions to support community oncology programs in providing targeted cancer therapy. Simultaneously, I am contributing to the establishment of the field of Implementation Science by validating and expanding on the Theoretical Domains Framework across numerous studies and furthering the field by differentiating attributes of innovation adoption from those of implementation. Further, my ongoing work will contribute to our understanding of the use of opinion leaders as one of the most effective implementation strategies to change clinical practice. During this time, I have demonstrated that I am a productive and nationally influential health services researcher and implementation scientist. My mid-career level achievements are as follows: Acquisition of National Databases on Behalf of the Institution I have submitted several successful applications to acquire national datasets for research use by students and colleagues in the Departments of Population Health and Urology, including SEER-Medicare and the National Cancer Database, and conducted primary data collection of a national sample of cancer programs. These projects have resulted in several publications and numerous presentations for students, residents, fellows, and faculty peers. Significant Extramural Funding Productivity Since 2014, I have been awarded more than $1,000,000 as Principal Investigator. Thirteen extramurally-funded grants, contracts and subawards support my research agenda to improve cancer care delivery and access to care. My current NIH awards, from the National Institute of General Medical Sciences (NIGMS) and the National Cancer Institute (NCI), will support my research agenda to better understand determinants of implementation and sustained use of evidence-based cancer care in community settings, including the rural communities vital to the mission of the University of Kansas. My funding from NIGMS through a COBRE award will also serve to build infrastructure at the institution to support other research in precision medicine through the Kansas Institute of Precision Medicine. In addition, I have submitted more than 12 other federal and foundation proposals as PI, with $600,000 in external funding currently pending peer review. I have also supported almost $5,000,000 in additional submitted and funded research as co-investigator and have collaborated on numerous other federal grant applications with KUMC researchers. National Innovation in Analytics, Policy and Policy Implementation In addition to my funded work, I have collaborated on several innovative, national modeling competitions to improve the treatment of cancer and other chronic disease patients. Prostate Cancer Dream Challenge efforts culminated in new predictive models of survival for metastatic, castrate-resistant prostate cancer patients. Current work on the Centers for Medicare and Medicaid Services Innovation Center Challenge will apply artificial intelligence to develop new models to identify chronic disease patients at risk for hospital readmission and activate physicians to proactively address at-risk patients’ needs. In addition, I have published national research briefs on shared accountability in primary care and created a toolkit to support national cancer guideline implementation in urology practice. I have also pioneered an opinion-leader led training program for clinicians to translate research into practice, which has been delivered to providers representing eight U.S. states and Mexico in four venues, cementing a national reputation through development of educational programs for clinicians. Research Publications in High-impact Peer-reviewed Journals As a result, I have developed a strong record of peer-reviewed publications, cited 1282 times (508 in the last five years) with an H-index of 13 (H-index of 10 in the last five years), as measured by Google Scholar. I have nine senior author manuscripts published or in press, with an additional seven co-authored manuscripts published in peer reviewed journals, including high impact journals such as Lancet Oncology, Cancer, The Journal of Oncology Practice, and Implementation Science, the leading journal of my field. National Exposure Shared with Trainees Together, with my collaborators, including more than a dozen trainee collaborators (n=14), I have been invited to present this body of work at numerous prestigious regional, national and international meetings through poster presentations and invited podium presentations. As senior author, I have been first author on approximately half (n=10) of these presentations, sharing first-author recognition with trainees and collaborators in a large number of opportunities. In addition, I have been invited to speak on my research within KUMC, at numerous local and regional venues, and several other universities. National Research Recognition

Demonstrating my national reputation as a scientist, I have been invited to serve on a national, standing grant peer review study section and to contribute my expertise in implementation science as founding editorial board member of the international journal Implementation Science Communications. My scientific contributions have culminated in recognition for excellence in research with local and national awards, including awards bestowed by the KU Cancer Center, the international Project Data Sphere Prostate Cancer Dream Challenge, the Association of American Cancer Institutes Clinical Research Initiative (shared with a trainee), selection as an NCI-funded MT-DIRC fellow, and as the competitively-awarded 2020 AcademyHealth/National Cancer Institute Visiting Scholar for Healthcare Delivery Research. During my term, I will work with colleagues at the National Cancer Institute to conduct research on rural oncology practices’ participation in research and, via the national platform provided by AcademyHealth, the pre-eminent Health Services Research professional society, I will convey a national agenda for expanding rural participation in cancer care delivery research.

D. Unsponsored Research My portfolio of funded research is supplemented with several unsponsored research activities which provide research opportunities for colleagues, students, residents and fellows. I serve as the Principal Investigator responsible for research and data security for several national databases. These successful, competitive applications provide opportunities for our institution to conduct analyses on cancer care delivery through the SEER-Medicare program, the National Cancer Database, the NCI's National Community Oncology Research Program, and SWOG. Participation in international data challenges provide institutional access to other data, including multiple data sets of randomized controlled clinical trials of prostate cancer treatments and enhanced Medicare claims data sets. Other multi-center collaboratives offer additional opportunities for trainees and peers to engage in national implementation science efforts.
