NAVIGATE PLAN

WHAT’S IN IT FOR YOU

You must choose a primary care physician in our network.

You can choose any primary care physician in our Navigate network. Once you choose your primary care physician, he or she will provide most of your regular and routine care. This will include annual well visits and preventive care, as well as care for sickness or injury. You must get a referral from your primary care physician before you see another network physician or specialist.

You save money when you use our network.

If you need a referral, you save money when referred to network physicians and specialists. Depending on your plan, without a referral, your costs may be a lot higher or they may not be covered at all. And emergencies are covered anywhere in the world, including emergency care done at a non-network hospital.

Your preventive care is covered 100 percent in our network.

Preventive care services include routine wellness exams, well-child exams, physicals, mammograms, flu shots and other immunizations. As long as you see network doctors, you don’t even have to worry about copayments.

You have coverage for a wide range of prescriptions.

You can fill your prescriptions through one of our 60,000 retail network pharmacies across the U.S. If you have our mail-order service, you may save money with a three-month medication supply, which is mailed to you with standard shipping at no additional cost.

You have coverage for a routine vision exam.

You and your covered dependents are eligible for one routine vision exam. The exam must be done at a network eye clinic. The coverage does not include the purchase of eyeglasses or contact lenses.

When you enroll in a Navigate Plan, you get a health plan that puts you on the path to the quality of care you deserve. And you get the protection you need from major health care costs.

The plan comes with predictable copayments and other costs when you see your primary care physician or need a prescription, so that there are no surprises.

Transition of care is available if you are receiving time-limited care for certain medical conditions from a doctor who is not in our network.

HOW THE PLAN WORKS

You must select a primary care physician in our Navigate network once you enroll. Your primary care physician can be a general practice, family practice, pediatrician or internal medicine physician. You can select one physician for your entire family, or each covered family member can select their own. Some states will have different requirements for what qualifies as a primary care physician. If you do not select a primary care physician, we will assign a physician for you based on your location and physician availability.

Once you select your primary care physician, he or she will be your first point of contact when you need care. They will provide most of your regular and routine care. This will include annual well visits and preventive care, as well as care for sickness or injury. Your primary care physician will see that you receive the highest quality of care at the right time.

If your primary care physician feels that you need special care beyond what they can provide, they will help arrange it for you and will provide a referral to see a network physician or specialist.

You take an active part in your health care when you choose a primary care physician. People who are active in their health care decisions:

After you enroll, you will be able to see all of your referrals on your member website, myuhc.com. You can even print your referral to bring to your appointment.

Obtaining referrals to see another physician or specialist.2

If you enroll in the Navigate Plan, you must obtain a referral from your primary care physician before you can see another network physician or specialist. Without a referral, you will have no coverage for that visit, which means you will be responsible for the entire cost. In addition, if you receive care from a physician or specialist who is not in our network, you will also not have coverage.

Referrals are not needed for services from certain network providers, such as obstetricians/gynecologists, behavioral health or substance use disorder clinicians, or for routine eye exams from network providers. You also do not need a referral for services from a convenience care clinic or urgent care clinic as long as they are in the network. Emergency care does not require a referral and is covered anywhere in the world, including at non-network hospitals.

Some procedures or services will require that your primary care physician or specialist get approval, or prior authorization, from UnitedHealthcare before they can be performed. Please see your benefit plan documents to learn more.

HOW THE PLAN HELPS YOU NAVIGATE YOUR WAY

Your primary care physician will ask questions to try to understand what may be causing the illness. This may include questions about your life and lifestyle habits. If necessary, your physician may order some tests.

If you require care that is an emergency, call 911 or go to the nearest emergency room immediately.

You do not need to see your primary care physician.

You’re not feeling well. You make an appointment to see your primary care physician.

3Provide a treatment plan and/or prescribe medications

Most of the time, your primary care physician will be able to provide the quality care you need. He or she will also educate you about your illness or injury and give you information to help you manage and improve your health.

Refer you to a specialist

Your primary care physician may feel you need treatment that they cannot provide. In this case, they will refer you to another network physician or specialist. And they will work with the specialist to make sure you get the right care.

24-hour registered nurses

You can call a nurse for help anytime. They can help answer questions such as Should I see a doctor for this problem? Does my medication have side effects? and What are my treatment options? They can direct you to network doctors and even schedule appointments.

Healthy Pregnancy Program

We will help soon-to-be-mothers through every stage of their pregnancy and delivery. We will check for risks, share baby-healthy tips, and keep you informed by phone and newsletter.

Care management

We’ll work with you and your physician to help you understand your options if you need care. We’ll also let you know about special programs to help you make health care decisions, such as disease management programs. Some health care services may require “prior authorization.” This is the approval a person receives for care before it is provided. Please see your benefit plan documents to learn more.
