First Back to the basics of transcription in regards to operative reports.
Headings:Preoperative diagnosisPostoperative diagnosis
Keep in mind when the doctor says “same” for the postoperative you are to type what he/she stated in the preoperative.
For example:
PREOPERATIVE DIAGNOSIS: Cervical disk disease with progressive radiculopathy.
POSTOPERATIVE DIAGNOSIS: Cervical disk disease with progressive radiculopathy.
Operation or Procedure or both (If there are more than one enumerate).
For example:
