Michael W. Kennedy, PMP, MSMPM Director of Informatics
Michael Kennedy has worked in ESRD since the mid-1990’s. He has worked in various roles at the dialysis facility-level, as a CMS supporting contractor, and at the ESRD Networks. Michael served in a key role during the movement of all Network data systems to the Standard Information Management System (SIMS). Furthermore, he has served as lead on the CMS-2744 Annual Survey project.

Shane Perry has worked for Network 4 (the states of Pennsylvania and Delaware) for 14 years. Prior to his current role, Shane served as the Director of Information Systems for Network 4 since 1999.

Rhonda Lockett has been a part of the ESRD Network program for 33 years and has been actively involved with the development, testing and deployment of CROWNWeb.

What’s the Latest?
Clinical Data Submission
• Starting with January 2013 data, CMS has moved to a standard data submission schedule in which clinical data is due by the end of the following month.
• February 2013 clinical data is due by March 31, 2013.

CMS-2744 Submission
• Facilities may begin the 2012 CMS-2744 submission process if all 2012 patient data is in CROWNWeb.
• Facilities must review and work to correct any errors that may display before submitting a CMS-2744 for acceptance.

Purpose of the CMS-2744
Overview of Purpose
• The CMS-2744 is designed to collect information regarding:
Treatment trends
Utilization of services
Patterns of practice in treating ESRD patients
• The CMS-2744 form is the only Office of Management and Budget survey of all facilities that provides valid census and dialysis patient movement information.
• The information is used to assess and evaluate the local, regional and national levels of medical and social impact of ESRD care.

Overview of Purpose
• The CMS-2744 Annual Facility Survey:
Captures facility, treatment, and staffing data for the survey year (January 1 – December 31)
Historically, provided data to be uploaded onto CMS’ Dialysis Facility Compare website
Helps identify the top 50 transplantation centers in the nation

Getting Started
• The CMS-2744 form in CROWNWeb is identical to the paper form.
The fields and CMS instructions are the same.
The submission process is different.

Getting Started
• Facilities will initiate the CMS-2744 completion process in CROWNWeb by generating a form, and submitting it for acceptance by the Network.
• Facilities must ensure that their 2012 CMS-2744 form is in a “FINALIZED” status in CROWNWeb by May 1, 2013.
• Check with your ESRD Network for local due dates.

CMS-2744 Reports and Tools
• CROWNWeb gathers data from the Facility Details, Admit/Discharge, Treatment Summary, and Patient Attributes screens to populate most of the CMS-2744 form.
• The CMS-2744 form will present warning and error messages if the counts do not balance.
• Facilities can utilize CMS-2744 reports to determine where a correction may be needed.

Common Errors
Common Errors
Most of the errors facilities will see after initially generating the CMS-2744 are related to:
• Duplicate patient records, and
• Patients that have “System Discharges” instead of valid discharges (i.e., transfer to another unit).
