PrEP Is Available in Evansville by Dennis Myers, MSN FNP AAHIVS

Are you and your partners prepped for HIV prevention? Simply put PrEP (Pre Exposure Prophylaxis) is a medical means of preventing HIV acquisition/sero-conversion to HIV once exposed. Just as a reminder, if you are into sex, drugs, "rock and roll" you are at risk for acquiring HIV or sero-converting to HIV post coming into contact with a positive HIV person. MSM (men having unprotected anal intercourse with men) remain at greatest risk for acquiring HIV. Sero discordant cou-ples (one is HIV positive, the other is not); male to female, female to male transgender persons engaging in high risk sex behaviors; persons engaging in sex for hire, drugs, etc.; persons injecting drugs; person using stimulants; persons with a history of anal/genital infections; persons who continue at risk behaviors as above and who have been on PrEP inconsist-ently are the very persons who should be on PrEP and take it very seriously. Truvada is the drug of choice. When taken daily at the same time every day, along with barrier protections, the chances of NOT acquiring the HIV virus with any exposure rises to 44-75%. It rises to almost 100% if you are negative and are hav-ing relations with an positive HIV partner who is on his/her medications, is !00% adherent to those medications and has an UNDETECTABLE viral load and REMAINS undetectable. The MOST important rule to follow when taking PrEP is to take it 100% of the time, every day, at the same time if you wat to remain protected. You can stop it any time without medical side effects. Remember, you stop the drug, take it inconsistently your protection drops and if your risk factors remain high you acquire HIV when exposed. You have to have good kidneys to take this drug. Side effects include fatigue and nausea. These slow down and stop the longer you remain on the drug. Truvada is expensive and you need some form of insurance or a source for the drug. In Evansville AIDS resource group is recommended. The first step in getting on Truvada is finding an empathetic provider, who is knowledgeable about the drug and is willing to maintain you on this drug AND is willing to see you every 3 months for follow up and to check to see if you are still HIV negative. The provider should and will perform a complete sexual and medical history on the first visit. Lab tests will include a 4th generation HIV 1 - HIV 2 screening test to make sure you are not currently HIV positive. Never use any rapid screening tests. A hepatitis A, B, and C screens will be drawn as well as screening for all STDs, a blood count for any anemia or cur-rent infections; and most important of all KIDNEY functions tests. Then and only then will you be prescribed Truvada. You will be given a 3 month supply with the above instructions. Rechecking these labs is MANDATORY in 3months if you are staying on the drug. If you turn up HIV or Hepatitis B positive during those three months, PreP stops and Hepatitis B and HIV treatment begins for LIFE!!! What PrEP is NOT is a "club drug". It is not a "morning after pill". You must be on this med for at least 1 full week before protection kicks in. Taking PrEP intermittently is NOT recommended nor advised. You can stop PrEP any time without medical side effects. If you choose to stop PrEP, re think your high risk behaviors. Should you choose to start PrEP the afore mentioned "rules of the road" are mandatory and not negotiable. Find a pro-vider who will back you up 100% Anything less you risk acquiring HIV. Keep it wrapped and lubed! Choose your partners wisely! PrEP does not protect against STDs - syphilis, gonorrhea, chlamydia, anal warts, herpes etc.
