Causes for Errors
Run Patient Detail Report
• Look for Lot Numbers that did not match your active inventory in the IIS.
Monitor fridge via review of reconciliation page
• Look for lot numbers that may be off due to incorrect selections in the EMR
Make Corrections to EMR & IIS Patient Record
• Correct errors found
How to Find Errors
1. Select “By Service”2. Type in the Vaccination
Date Range for the report. (We recommend running your monthly report date range each time)
3. Type in a Birth Date Range that includes only children less than 19 years of age
4. Ensure that patient VFC eligibility is set to “All”
5. Choose “No” under Publicly Supplied Vaccine
6. Click Create Report.
Running the Pt Detail Report
Was this vaccine record brought in as an official historical vaccine?
Was it administered by your facility and wrongly recorded as historical?
OR
Double Check Historicals
Different Facility Listed
A float staff member was not logged into the correct location
Fill In Missing Information
Missing Fields = No Match
Double Check Lot/Manufacturer
Compare with• What is on the BOX• What is on the Reconciliation Page
Sanofi-Pasteur PMC
GlaxoSmithKline SKB
Merck MSD
Novartis NOV
Pfizer PFR
Other Possibilities
1. Inventory not received in IIS before administering
2. VFC or Insurance improperly recorded (UNK)
3. Vaccine actually given from wrong supply
4. PPD Test or travel vaccines can be ignored
5. Duplicate vaccine given to same patient
• May be due to twins or siblings having records
inappropriately merged in the IIS. Confirm in your
records whether this is true and call the Help Desk.
Run Lot Recall Listing
• Generates a list of patients who received this lot
• Compare with any records you may have
• Find missing patients on either list
UPDATE MESSAGES – Lot Number
Other Vaccine Details (vaccinator, route, site, etc.)
DELETE MESSAGES- Vaccine Date
Vaccine Names
Fix in Patient Record
