• 1 Transition Assessment – Post Secondary Goals Survey

• Writing Transition PLs to reflect needs and direction

• Tightening up Plans (That folder in GV)

• Wrote a plan to help with all this and more

The Plan…1. By June 2015, at least 80% of transition PLAAFPs will be written to include

transition assessment data and statements that ties academic and related service to measurable annual goals, as measured by a 25% random sample file audit of the district.

2. 70% of HS students will complete a career interests inventory, a Life Skills Inventory, and a post-secondary goals survey at least once annually.

3. Student and Parent training will be provided 4 times per school year on IEP and transition processes.

4. Transition planning team will develop a transition policy and procedures manual by August 1st 2014

5. Transition training will be provided to all special education staff at least 1 per school year.

but mostly Heather, Tabitha, Lynn, Shelly and AmyWe will need your help to write the awesome IEPs you already do! Plus

help with parent trainings and getting students to actually do those assessments.

What: Yeah, we just talked about that.

When: We’ll be getting this ball rolling now, picking up speed through the summer, and be ready for the next plan by June of 2015.

Where: statewide?

Why: because we are awesome and we know it, we just need to show it!

How: With the help and support of each other, of course!
