Transforming Lives – One Smile at a time

After making any donation of personal funds to the Transforming Lives – One Smile at a time campaign, take these steps to generate a 2-for-1 match from Johnson & Johnson

STEP 1

• Go to http://corporatecontributions.jnj.com/Pages/ClicktoCare.aspx• You may want to save this link as a favorite for future reference

• Selecting “matching gifts program”

STEP 2

• Input your WWID, e-mail or Username

• Input your Network Password STEP 3

• Select “Submit a New Request”

STEP 4

• If you have made a personal cash/check donation directly to Operation Smile - Select “Cash/Check”

• If your donation is not this type please refer to the “help” icons on screen to make the correct selection STEP 5

• Select “Search for a charity”

STEP 6

• Input “Operation Smile” in the Agency Name field

• Then select “Find” STEP 7

• Select “Operation Smile Inc” from the selection window

STEP 8

• Input the date of your donation

• Input the Tax Deductible Gift amount you personally donated to Operation Smile

• Input Gift amount to match (if not the full amount)

• Input Purpose: “ETHICON-US-Team Name-Individual Name”

• Select “Continue”

• Complete the AFFILIATION window as appropriate to your specific circumstance

• Then select “Continue”

• Check the details of your request to ensure that it is accurate

• Complete the Donor Certification section according to your circumstances

• Once you have completed everything to your satisfaction, select “Confirm” STEP 9

• You will now receive an email confirmation of your request
