Dave: Whatdidyoubeforeyoubecomeamedicaldoctor?Whatdidyoustudyinyourundergradthatmaybegaveyouadifferentperspectivecomparedtotheaveragejust-doctor?

Dr.Veech: Asanundergraduateintheuniversity,Istudiedhistoryandliterature,whichwasaveryinterestingdepartment.Itwasfoundedbyachemist,CharlesY.EliotwhodecidedthateveryonethatwaseducatedneedtohaveknowledgeoftheBible,Shakespeare,Greekhistory,Greektragedy,Russiannovel,andaforeignlanguage.Youhadthree-houroralexams.Itwasverygood,verygood,verygoodundergraduateeducation.

Dave: Youstartedoutwithsomethingthatisn'ttraditionalforsomeonethatwenttomedicalschool.

Youalso,alongtheway,gotaPhDasaresearchbiochemistaswell,right?

Dr.Veech: That'sright.Yeah.IdidthatafterIwasatmyresidencyinmedicine.

Dave: Thereareveryfewphysicianswhoalsostudybiochemistryatthatlevel.Whydidyoudecidetodothat?Idon'twanttosayalmostunheardof,butit'ssorareanditgivesyousuchadifferentperspectiveonthings.Whatmadeyougotothatlevelofeffort?

Dr.Veech: Well,ifyouhavebeenaninternandresidentandyou'veseenafewpeopledie,yourealizethatyoureallydon'tknowverymuchatall.Youbettergolearnsomemore.That'swhatIdid.Ilookedinabunchofdifferentlabs,Iendedupgoingtoalab,Ithoughtwasthebestintheworld,Ithinkthatwasright.ItwasatOxfordandIstudieswithSirHansKrebs.Heis,Ithink,stillthebestbiochemistofthecentury.That'swhereIlearnedmybiochemistry.

Dave: Forpeoplelistening,thisisnoneotherthantheguywhodiscoveredtheKrebsCycle,whichiswhyit'snamedafterhim,right?

Dr.Veech: That'sright.

Dave: ThisisthecoreofhowourcellsmakeenergyviaATP.Inmyownlosingof100poundsandgettingmybrainback,IreadallofKrebs'thingsandeverythingIcouldaboutthecitricacidcycletofindwaystomakeitworkbetter.Wow,ifyoufixedyourmitochondriaandimprovetheirperformance,evenasmallpercentage,itjustpaysdividendseverywhereinyourbody.That'swhyIwasamazedyouactuallystudywiththeman.

Dr.Veech: Well,theessenceofwhatwe'resayingandhowketonesworkispreciselyintheKrebscycle.Manymanydiseases,forinstance,you'retobecongratulatedforlosing100pounds.Peoplethatareoverweight,haveanimpairedKrebscycle.Theyhaveanimpairmentintheirentirecellandinasensejustdoesn'tworkverywell.KetonesbypassthatblockofPDH.Allofthethingswe'resayingaboutKetosisareapplicabletoType2diabetes,areapplicabletoinjury,toinfectionandevenanumberofdiseases.Forinstance,Alzheimer'sdiseaseisablockinPDHinthebrain.It'sacentralgatewayintotheKrebscyclebutit'sveryeasilydisturbed.Whenit'sdistrubed,youcangetaroundthatdisturbancebygivingketones.

Thereisnothingdangerousabout,it'sanormalstateofman.BeforewehadMcDonald'soneverycorner,wefastedinbetweenthetimewewerechasingthemammothandwecaughtthemammoth.Inordertodothat,youneededtobeketotic.That'swhywehaveketosis.

Dave: Now,whenyousaythereisnothingdangerousaboutketosis,Iwonder,Iknowmanypeoplewhocangoinketosis,andtheystaythereforyearsandthey'rehappyasclams.WhenItriedanexperimentofextremelowcarbs,likeoneservingofvegetablesaday,therestfatandsomeprotein,triedtomimicanInuit-stylediet.Ididn'tdosowell.Iendedupinmybookrecommendingcyclicalketosiswhereyougointoketosis,youcomeout.Yougoin,youcomeout,ratherthanconstantlybeinginheavyketosis.DoyouthinkthereareriskstobeinginketosisforlongperiodsoftimeorisitsomethingthatjustworksformostpeopleandI'manexceptiononthatone?

Dave: Itmakesyoufeel,liketheflu.Youfeelreallycrappy.WhatI'vebeendoingforthepast10yearsorsoisshortcircuitingthatbecauseIfinallyfoundtheresearchthatIthinkexplainsit.Thatwhenyouhavealevelofjusta0.05,aprettyfarcryfrom7millimolar.YourCCKandyourghrelin,theseareyourhormonesforfulfillment,forfeelingfullandyourcravinghormone,yourhungerhormone.Theygetshiftedbyjustasmallamountofketone.You'renotquiteinnutritionalketosisbutyou'rewayhigherthannormal.Allofasudden,allofthecarbfluthingsthatIwasusedtohaving,whereIjustfeelcrappy,theyjustwentawaybecausemyhunger,Iwasn'tcraving.Ididn'tcareaboutfoodthefirstdayIdid.ThatcomesfromputtingbrainoctanewhichgoestoBHP,whichthengoestoketonesinBulletproofcoffee.

Thereisnocarbs.Thereissomethingthatjustgavemealittlebumpofketones,notabigbump.Thatseemedtohelp,butwhatyou'retalkingaboutisgettinghugebumpsofketonesbecauseweknowtherearethreekindsofketonesthatareuseddifferentlyinthebody.Canyougointomoreaboutthedifferentkindsofketonesandthathungersuppressionthing?
