I acknowledge that I am aware of all the risks inherent in the Event and certify that I am physically fit, have not been

    otherwise informed by any physician and know of any restrictions imposed on me by my own physician that would in any way prevent me from actively participating in the Event.

    In consideration of being permitted to participate in this Event, I, on behalf of myself, my successors in interest heirs, assigns and representatives, hereby fully release and agree tohold harmless The Leukemia & Lymphoma Society, Inc., Calasanz Martial Arts, Inc and their affiliates, their officers, trustees, agents, employees and representatives, successors

    Help us fight a way to a cure for bloodcancers in the first Train Like a Fighterfor The Leukemia & Lymphoma Society.Participants collect pledges and dedicatea 2 hour Fighters Workout to honorthose that have leukemia and otherblood cancers. Although the trainingwill be challenging people of all athleticlevels and ages will be able to participatein a safe environment and help in thefight against leukemia.

    A diagnosis of Leukemia once meantcertain death. Thanks to the dedicationof LLSs researchers, 8 out of every 10children diagnosed are now cured, butleukemia still claims the lives of morechildren and young adults under the ageof 20 than any other disease. Help usreach our goal of a total cure. Anyonecan sponsor a participant with a flatdonation. All funds raised will supportLLSs programs of research, education,and local patient aid.

    In order to participate please completethe information, raise at least $100, signthe waiver and return to your eventcoordinator or the address on thereverse of this form with your donationsbefore Tuesday, June 1, 2010.
