tr.L B&B Permit Application Checklist Welcome to the 8&B Permit Application Checklist. lt is designed to guide you through preparing your submittals in a way that leads to the smoothest and quickest processing of your B&B permit application. lt is reeommended to read the whole checklist (and application form) through before preparing any submittals. A 500-Foof Boundary Notification List. The 500-Foot List is a list of TMK numbers, names, andaddresses of all the owners and lessees of record within 500 feet of the subject property's boundaries.The list shall be compiled from the most current list available at the Real Property Assessment Division ofthe Department of Finance at the time of application (mauipropergTtax.com). Refer to 5}A-Foot BaundaryNatification List & Map lnstructions for more information on how to complete this item (MCC, Section1e"64.040(BX1)).

Submit a locatjon map (MCC, Section 1S.64"040{BX2)). This shall be drawn to scale, identifying thelocation of the subject property within the general area. Refer lo 500-Foot Boundary Notification List & Maplnstructions for more information on how to complete this item.On this location map:a. Clearly identify the subject propefiy;

b. Clearly identify atl lots within 500 feet of the subject property's boundaries;c. Draw a line indicating the 500-foot boundary; andd. lnclude all the TMK numbers within that area gJ have a clear systematic method to match each lot

on the map with the list of awners within 500 feet of the subject property"

Submit a site plan (MCC, Section 19.64.040(A)(4)). You may want to consider hiring someone withexperience in drawing site plans. This site plan shall include and be:a. Drawn an I Yz x 11 or 11 x 17 inch paper. ln the case of large parcels and plan areas, full-sized

hard eopies will be accepted, and in which case digital plans in PDF forrnat will also be required;b. Drawn to scale. ldentify scale used on each plan page;c. Show the entire lot and property lines;d. ldentifu the location of all dwellings and structures on the property; ande. ldentify ali on-site parking areas and driveways, and indicate all B&B stalls specifically"

Submit a parking plan (MCC, Section 19,64.040(BX2)). You may want to consider hiring sorneone withexperience in drawing plans. This parking plan shall include and be.

a. Drawn onBTz x 11 or 11 x17 inch paper. in the case of large parcels and plan areas, full-sizedhard copies will be accepted, and in which case digital plans in PDF format will also be requireet;

b. Drawn to a scale that conveys the required detail. ldentify the scale used on each plan page; andc" Frovide a detailed plan of all on-site parking locations, the surface material, who will be using each

parking space, the dimensions, isle width, and maneuvering areas. Parking spaces shall be aminimum o't 8% x 18 feet" Tandem parking and grasscrete are perrnitted. Parking requirements canbe found in MCC, Section 19"36A"010, Designated Number of Spaces, B&ts Home.

Subrnit proof that any proposed B&B dwelling was built in compliance with County regulations at thetime of eonstruction. On site plan, show all building, electrical and plumbing permit numbers for eachstructure. lf a structure is exempt, please reference building permit exemption number.

Provide the record of completed building permits (building, plumbing, and electrical) showing flnalapproval for the original construction and all additions. ln the Maui County KivaNet database, permits rnarkedwith a decision code of "A" {approved) and with a completion date meet the requirernent. Permit documentsshowing the inspection history and approved final inspections may also meet the requirement"There are several methods for documenting a structure's building permit history. First, building perrnits formore recent structures, usually built after 1978, can be found online at: htip:l/kivanetl 0g.co.maui.hi.us/kivanel!2lpermitlindex.cfm . Second, the online records of the County's Real Property Tax (RPT) offlce canbe checked by conducting a "Parcel Number Search" in which the "Tax Map Key #" is entered and parceldata tracked by RPT, inscluding building permits, are displayed. Start the search at: https:/lqpublic.schneidercorp.com/Application.aspx?App=;yauiCountyH!&Layer=Parcels&PageType=SearchFinally. if an online search does not find building permits for the original strctures andlor all latermodiflcations, you may need to contact the offices of the Departrnent of Public Works (DSA Section,270-7379), or the RPT office in the County Service Center at Maui Mall in Kahului (270-7297, 7A E,Kaahumanu Ave., Suite 416) to see if they have permit records which are not online. Note that buildingperrnits were not required priorto 1952; in these cases, submit evidence that the structure was built prior to1 e52.

lf there are structures that do not have cornpleted building permits, file for Miscellaneous lnspectionsfor building, plumbing and electrical reviews through the Department of Public Works, DSA Division. Submitcopies of all approved inspections.

Submit proof that the proposed B&S inside and outside areas accessible to guests meet the minimumhealth and safety standards established by the Departrnent of Planning. Submit a Department of Planning

Submit a Menu of the food items that will be provided to guests. Please contact the Departrnent of Health at54 High Street, Wailuku, for helpful information and regulations regarding this item.

Submit a copy of the House Polieies that will be posted within each guest room. The house policies shallinclude, at a minimum, the following provisions:

a. Quiet hours shall be maintained from g:00 p.m. to 8:00 a.rn., during which time noise in the bed andbreakfast home shall not disturb anyone on a neighboring property;b. Amplified sound that is audible beyond the propefi boundaries of the B&B is prohibited;c. Vehicles shall be parked in the designated onsite parking area and shall not be parked on the streetovernight.

Submit a copy of the Fire Escape Plan that will be posted within each guest sleeping room (MCC, Section19.64.030(H)). The exit plan shall include, at a minimum, the following:a. Complete floor plan of B&B home;b. "You are here" marked on the exit plan;

The exteriorand interior areas of the heating plant(s) do not present any overt signs of leakage, cracks, rust,scorched areas, missing components, or deteriorated seals. There are no rnissing or visiblydefective control elements. Clearances from combustibles have been rnaintained at all points ofall heating plant vent connections where visible and readily accessible: &inch clearances forsingle wall "Class C" vent connections; 1-inch to 2-inch dearances for double wall "Class B"vent conneetions or per UL provided labeling; O-inch clearanees for triple wall "Class A" ventconnections or per UL provided labeling.

Plumbing Systema. All gas piping mnnections where visible ard readily accessible have been maintained in a

professional state of repair. There exist no uncapped gas lines where visible and readilyaccessible. All gas valves at appliances present ready access. Each gas appliance is providedwith a gas shut-off valve within 6 feet of appliance and within the same room as gas appliance-Flexible gas connectors shall be made of stainless steef" The maximum length of flexiblestainless steel gas connectors is 3 feet at appliances, with the exception of 6 foot tengthspermitted for ranges and dryers. #22 Description of Ocean Blue Ohana B & B

The Hosts

April has been visiting the islands for the last 45 years, including getting married to Bob in 2015. They decided to spend their “golden years” in a place they love. They started to look for their Maui dream house in 2017 and anxiously waited for their new adventure. A year and a half later they moved in and absolutely love their new lifestyle. Bob worked for the Department of Water & Power in California and holds 22 world records in powerlifting. April is a dog lover, has a 30-year background in hospitality, and is part owner of Citydog! Club – providing dog daycare, boarding and grooming. Bob is retired and April works from home so they get to enjoy their new home full time with their 4 small dogs – Furby, Dolly, Mimi and Foxy Lady. They are looking forward to hosting guests in their home and sharing their love of Maui.

The Property

Our half acre property is in a quiet residential neighborhood at 3523 Malina Place in Maui Meadows. It includes a cottage and the main two-story house with a single garage and plenty of parking. We love the extra space of the property, quiet environment, friendly neighbors, and close proximity to the beaches, shops and restaurants.

We know there is an important concern in Maui regarding enough inventory of long-term rentals. Our cottage is a long-term rental. The tenants, Elissa and Rich, are fantastic people, love Maui and plan to stay long term. Elissa works at the Four Seasons and Rich is a retired pilot. They are avid kite surfers.

We live on the second floor of the main house which includes the main living area as well as a master bedroom and 2 small bedrooms which we use as our offices. The lower level has 2 bedrooms which we’d like to use as the B & B. We will only rent to single families. We are quiet people and plan to screen carefully to host quiet guests. We do not have any water features, such as a pool, which assists in our quiet goal.

Having the opportunity to have a B & B is a perfect fit for us. With April’s hospitality background, we feel that we can offer an awesome experience for our guests – sparkling clean, friendly service, and amazing space. Bob is very handy and will keep everything working perfectly.

We cannot rent the lower level for a long-term rental because our family visits and we need the space during those occasions. We have 4 kids and 6 siblings and their families. The B & B gives us the flexibility to accommodate our family and rent the space when family is not here.

Each day they can wake up and enjoy their coffee on the lanai looking at the incredible blue ocean – the unobstructed views make them feel like they are on top of the world! As you walk up to the entrance and into the great space, you immediately have incredible views of the ocean and neighboring islands. The open concept design features a 65" Samsung 4K Smart TV and Blue Ray DVD player. Ocean Blue Ohana has a large wet bar with refrigerator, microwave, ice maker, coffee maker, electric kettle, and blender with seating for three along the bar as well as a table for 4 across the way.

As you turn to the lanai, the view will take your breath away. Step out and enjoy the sunset with a glass of wine and pupu’s on a bar height teak table with swivel chairs. On the other side is a professional grill, comfortable couch, 2 extra-large swivel chairs with ottomans, and coffee table. The covered lanai offers protection from the sun or you can enjoy a rain storm while staying dry!

The master suite enjoys the view and direct access to the lanai through sliding doors. Chill on the spacious king-sized bed with cotton sheets and a large Samsung Smart TV directly across the bed. Across the way is an alcove with lap top friendly work space as well as plenty of shelving to put your gear. Step into the en suite bathroom with two sinks, large modern walk-in shower, and complimentary Alba bath products.

On the other side of the home, you'll find the second bedroom which enjoys a view to the North and Maalaea Bay, queen-sized bed, cotton sheets, and large Samsung Smart TV directly across the bed. Plenty of storage with large closet, chest of drawers and side tables. The second full bathroom is just off the second bedroom and has a shower/tub with complimentary Alba bath products.

Head to the beach just a few minutes away or explore the island. Included for your enjoyment is everything you need - beach towels, Tommy Bahama beach chairs, umbrella, cooler, and kid’s sand toys. We suggest you keep these in your trunk so they are handy when exploring. Upon return, refresh yourself with a cool drink as you enjoy the stunning view and spectacular sunset!

Beautiful beaches with long stretches of white sand and crystal-clear waters invite ocean sports of snorkeling, kayaking, swimming, stand-up paddle-boarding, and scuba. Perhaps you want to just relax with a book on the beach or walk/run the Wailea beach path.

Fine dining and beach front restaurants are close by. Here are a few – Spagos at the Four Seasons, Ku at the Fairmont at Wailea, Morimoto Maui at the new Andaz Maui and the award-winning Mala Wailea in the Wailea Beach Marriott Resort & Spa.

Nearby you will also find The Shops of Wailea with numerous choices of shops and restaurants, an ice-cream parlor, coffee shop and gourmet market.

If you like Golf, there are many choices including Wailea Golf Clubs with world-class Gold and Blue & Emerald Courses, and Makena Golf & Beach Resort are within a few minutes drive.

Don’t forget to get a pampering massage and treatments at Spa Grande at The Grand Wailea, just a few minutes away.

Please respect our 9:00 pm to 8:00 am quiet time as per our Maui County B & B permit. Please be respectful of our quiet residential neighborhood. Amplified sound that is audible beyond the property boundaries of the B & B is prohibited.

Only registered guests are allowed on the property.

A maximum of two vehicles shall be parked in the designated onsite parking area which is the concrete pad near the entrance to the B & B and the corner of the parking pad. No parking on the street overnight.

Please observe Hawaiian style by removing your shoes before entering.

Please wash all sand off prior to entering the cottage. Hose bib at entry.

Observe Hawaiian culture by respecting the environment. Please do not litter. View the recycling list on the bulletin board and place recyclables in the blue receptacles.

Laundry, including beach towels, shall not be placed on the lanai railings.

Please do not flush sanitary products down the toilet or put food down the sink.
