Please download and save a copy of your entry form before completing it – please note that any changes made to forms open in your web browser will not be saved.
The total word count of your entry should be no more than 1000 words. If your entry form exceeds this limit your entry will not be accepted and you will not be notified.
Section 1 does not count towards the 1000 word count. Section 2 MUST NOT exceed 1000 words. Each question in section 2 carries its own individual word limit.
A maximum of 5 attachments (e.g. tables, graphs or photos) may be included with the entry form. These attachments must be image files (jpg, gif, png or tif) and must not exceed 5MB in total.

The Rules
1. The Apprentice of the Year Category is open to Apprentices who have completed an Apprenticeship with St Helens Chamber in 2017/18 or are currently on an Apprenticeship with St Helens Chamber. Nominations in this category must be made by the business employing the Apprentice.
2. St Helens Chamber Business Awards are free to enter.
3. Entries are treated in confidence but will be shared with a select group of individuals for judging purposes.
4. All entries must be electronic.
5. A maximum of 5 attachments (e.g. tables, graphs or photos) may be included with the entry form. These attachments must be image files (jpg, gif, png or tif) and must not exceed 5MB in total.
6. Any website links used within your entry will not be activated by the judges. Supplementary information may be requested at a national judging level.
7. Please note the maximum word count for this category is 1000 words. This word count limit does NOT include the questions or the information required in section 1. If your entry exceeds this word count it will not be accepted and you will not be notified.
8. Entrants may be asked for staff or customer testimonials.
9. Entries will be judged by an impartial panel of judges.
10. Shortlisted nominees will be notified by 27th April and invited to take two complimentary tickets at the Awards. Additional tickets will be available at £30+VAT. The St Helens Chamber Business Awards Ceremony takes place on 17th May 2018.
11. Winning entrants will be supported to enter a relevant national award for their category.
12. Entry to more than one category is permitted.
13. Applicants must be authorised to enter their organisation into the awards programme.
14. Entries should be in English and should be produced using a word processor. All entries must be checked for spelling and grammar.
15. Incomplete entries will not be judged.
16. All entries that meet all of the rules will be acknowledged on receipt. Entry forms will become the property of St Helens Chamber and will not be returned.
17. Proof of sending entry (by email) is not proof of receipt by St Helens Chamber.
18. St Helens Chamber reserves the right to withdraw an award from any applicant supplying false information within their entry.
19. The closing date for entries is midnight 3rd April 2018.
20. The judges’ decision is final and no further correspondence will be entered into.
21. No employee of St Helens Chamber is entitled to enter.
