Together, Carl and his wife came up with the idea for the CallMyDoc app to offer automated patient call answering to:

This application enables doctors to know who is calling and why, without getting up from the table at a restaurant. A doctor like Carl’s wife can review which patient is calling her and why in a transcription of the patient’s message. Then, she can view a summary of the patient’s chart and decide what to do.

The app has the ability to call the patient back to tell them the doctor will call in 10 minutes. The doctor can send a prescription refill from the app, which then tells the patient where to pick it up. And when the doctor is finished, the app documents the call and puts it back in the patient’s chart.

CallMyDoc eliminates all of the manual work of entering data into the EHR, which can slow practices down when documenting after-hours calls. In October 2020, healthfinch’s prescription renewal delegation engine, Charlie, celebrated an “App of the Month” distinction—for the third year in a row.

How to optimize and manage your remote office workflow

In the wake of the COVID-19 pandemic, working from home will be the reality for the near future. While this will bring

good things (the cat purring on your lap), it also brings new challenges (your child crying in the other room) as the calls keep streaming in. In addition, change is fast-paced. As a manager, how will you make sure your remote workers are up to date, working efficiently, but also that the correct information is being disseminated to patients?

Opargo

Scheduling controls and data to the rescue

Many solutions promise to make patient scheduling easier, but Opargo is different. Opargo

supports better scheduling by using artificial intelligence and machine learning to align schedules with user preferences and offer insights and options for optimizing time.

Opargo Vice President of Field Operations Nancy Harvey shares her insights on scheduling challenges she’s faced over the years. Read on to learn more about our September App of the Month.

From Nancy Harvey:1998 is the last time I personally scheduled a patient’s appointment while working in a doctor’s office. We were winging it. The practices I worked in were well-respected and well-run organizations, so the fact that there was almost no structure for scheduling was largely just the norm at the time. We had no written guidelines, just loose real-time direction.

When I say “real-time direction,” that basically means being told what we did poorly—after the fact. What’s fascinating is that until the last six years (starting with Opargo’s conception) the tools that schedulers have had access to are almost exactly the same ones we used in 1998. This has resulted in years of inaccurate and inefficient scheduling causing many problems related to patient care, operations and revenue.

PATIENT CARE

In many practices, schedulers are asked to make patient appointments on a first-come, first-served methodology. While emergencies get worked in and prioritized, unless the patient is having a true emergency situation the scheduling staff is just filling the empty slots. This

often results in patients waiting too long for care, being forced to seek it elsewhere or simply giving up and never seeking it at all.

For many patients, navigating the healthcare system can be overwhelming and there are simply too many barriers to receiving care. Eventually their issues escalate and they end up in our local emergency departments. As providers, we must be able to do better.

OPERATIONAL CHALLENGES

When patients are not scheduled properly, the downstream impact is also immeasurable. Providers deal with issues that range from days that are too hectic with complex patient visits bumping up against one another, to days that have too much downtime and are underutilized.

Either way, the impact causes provider burnout, decreased quality of patient care and lack of access for patients. If at the end of the day our providers feel frustrated because of the lack of value they brought to their community, we will end up with huge issues as an industry. Again, we can do better.

DECREASED REVENUE

Everything that occurs in a medical practice begins during the moment the patient’s visit gets scheduled. The downstream impact runs deep through the organization, affecting patient engagement, clinical workflows and, of course, billing. If a visit is scheduled improperly, the result could be that your organization won’t even be paid at all.

Often there are rules in place about how frequently an appointment can be scheduled, and the schedulers may be forced to use inefficient and manual processes. What happens next is likely

to be some of that “real-time direction” where the scheduler is admonished for not being able to manage a broken process. To say it one more time, we can do better.

DATA TO THE RESCUE

The good news is the Allscripts Practice Management solution is capturing extremely valuable data we can learn from, each time you schedule an appointment and subsequently bill for it. For years now, the Opargo team has analyzed this data to identify trends. These trends can validate struggles your practice might have had and can be used to better manage patient no-shows and other last-minute schedule changes.

This data can also provide visibility into actual patient demand, which can help providers make decisions about how to structure the flow of their clinical day. A large number of the findings can be used to drive change without any changes in technology. Other findings can lead to vast improvement by utilizing existing technology in a slightly different way and incorporating new functions, like Opargo’s predictive analytics tools.

One way or another, we owe it to our patients and our medical providers to place a greater emphasis on scheduling operations. Now there are tools available to automate the process and provide accountability across the scheduling organization, it is more possible than ever. I am looking forward to a day when these scheduling inefficiencies are a thing of the past, as it has been 20-plus years in the making.

ThoroughCare

Boost revenue and patient health outcomes with care management programs that complement your EHR

There has never been a more important time to keep your patients well-managed in

between their traditional office visits. Many healthcare providers offer care management services to their patients that were not reimbursable from Medicare before 2015. Over the last five years, The Centers for Medicare & Medicaid Services (CMS) has created a variety of programs every provider should be offering to their patients.

The programs provide a bridge to satisfying the requirements of the Quality Payment Programs by promoting interoperability, demonstrating process improvements and driving clinical goal achievement. These offer a great way to keep patients engaged in their health while providing significant revenue opportunities to the clinic.

1. Chronic Care Management (CCM)CMS will reimburse you $42 by providing a minimum of 20 minutes of CCM per patient per month. For practices with 100 CCM patients, that means potentially earning an additional $4,200 per month.

2. Remote Patient Monitoring (RPM)RPM can offer an additional $119 per patient per month to your practice. This includes $54 for 20 minutes of RPM staff time and $65 for providing the patient with a device to be used for daily monitoring.

3. General Behavioral Health (BHI)BHI will reimburse doctors, on average, $48 per patient per month for 20 minutes of non-face-to-face care management services. BHI services can be used to manage patients with anxiety,

depression, substance abuse issues and alcohol abuse issues.

4. Annual Wellness Visits (AWVs)CMS allows practices to bill for a Welcome to Medicare/Initial Preventive Physical Examination (IPPE), with an average national reimbursement of $168. Afterward, practices can bill for an Initial Annual Wellness Visit (IAWV) with an average national reimbursement of $178. Finally, CMS allows practices to bill for subsequent AWVs with an average national reimbursement of $117. Best of all, these wellness visits have no out-of-pocket cost to the patient.

What’s stopping providers from using these programs?Even with the high reimbursements and value to the patients, these programs are still underutilized across the country. Many practices cite the following concerns as reasons not to use them:

TOO TIME INTENSIVE

It’s always a challenge to find time in the day to do more. Many practices have found ways to flex or adapt their schedules to include telehealth visits due to COVID-19. In the same way, we can allocate time in a schedule for these programs. Fortunately almost all these programs can be performed by clinical staff under the general supervision of the provider. Most groups use a Certified Medical Assistant (CMA).

PATIENTS WON’T WANT TO PARTICIPATE

Now more than ever, patients want to stay connected with their providers. At a time when many patients aren’t leaving their homes, these programs are the perfect way to keep their conditions well-managed. As patients get used to connecting with their providers virtually,

it is easier than ever to implement complementary remote monitoring and check-ins.

PROGRAMS ARE TOO CONFUSING/INVOLVE TOO MANY RULES

Managing the programs in spreadsheets or templates can be cumbersome and tedious, and practices don’t want to risk getting audited if steps are missed.
