• the number given to an individual who has applied for benefits State ID

To Submit a partially complete application, the
user must Save & Exit, then when logging back
in, PEAK will give the option to “Submit your
application” or go back to continue filling it out.

An applicant is directed from Connect for Health Colorado to PEAK if they choose to seek financial assistance
PEAK and Connect for Health Colorado do not share login information; separate accounts need to be created if applicable
PEAK checks for an existing case in CBMS: applicants with an open, active case are referred to Report My Changes; applicants without are issued a new case number
When electronic verifications don’t align, Real Time Eligibility is not possible and an application moves to a manual process
An applicant with an active, existing benefit account is able to add a person or the Medical Assistance Program and potentially receive Real Time Eligibility via Report My Changes
