To whom it may concern: l will address each of the 11 recommendations that it is said the U.S. Army Maneuver Center of Excellence on Fort Benning will be, or has already implemented. I had received an email from the Deputy Chief of Staff, Sustainment MCoE, Fort Benning, GA. Enclosed in the email was the following statement, "Mr. Fabozzi and Ms. Rosalez have confirmed with me during a 9 March briefing to me that they had a plan for correcting the issues. Mr. Fabozzi has also provided a memorandum for record stating that nine of the 11 recommendations that specifically addressed DOTS procedures have been· put in place". It is these 11 recommendations that I will address because only about half have been implemented. Recommendation 2; There are Soldiers and DA Civilians preparing the DD FORM 2890 and 2890C without being properly trained. I was tasked with preparing a class and to be prepared to train the Soldiers _and DA Civilians at the ASP on how to prepare the DD FORM 2890 and how to properly fill out the DD FORM 626, Motor Vehicle Inspection (Transporting Hazardous Materials), by the DOTS Branch Chief, Ms Rosalez. I informed Ms Rosalez that my ability to certify employees would be ending on the 18th of April 2016 and that we needed to schedule the classroom to get as many people properly certified as possible. Unfortunately, she only scheduled one class and only a handful of personnel were present. may be under the rated weight, there is no way to tell if the truck is overloading the axle rating because no scales have

been purchased yet. This recommendation cannot be considered, fully implemented, until scales are purchased. This

means that there very well be ammunition trucks that are hauling ammunition and explosives over the roads on Fort

Benning that have too much weight on their axe ls. This can cause a major malfunction of the trucks drivetrain which will

result in an accident.

Recommendation 9; Nothing whatsoever has been done to fix this problem. Just last week, we .had Soldiers pull

up with a truck that had multiple items that were not c·ompatible. The truck had to be down loaded and the problems

fixed, prior to entering the ASP itself. There are no checks in place to determine if an item is not compatible. I advised

Ms Rosalez and Mr Seawright on numerous occasions, that the last four digits of the items NSN be placed on the

paperwork so that anyone who may be preparing a DD FORM 2~90 will know if the item should not be transported with

another item that it is not compatible with. This recommendation has just not happened at all.

Recommendation 10; I have not seen anybody be informed of any internal surveillance measures to ensure

adherence to any safety measures. I have witnessed the Supply Division Chief, Mr Tarrance, inspecting some tactical

vehicles that were in the process of leaving the ASP to deliver ammunition. I was told by him that there were several

problems but the Soldiers corrected them before being allowed to proceed. In my Plan of Action, I did send him the

requirements that each driver should have and the documents that are required in each vehicle. This information will be

attached as a PowerPoint Slide Show that I sent to him.

Recommendation 11; This has not happened at all. I have been asking every supervisor since January 2010 that

we needed scales to ensure that the truck axels or vehicle was not being overloaded. The WG-08s that I worked with

were all drivers that have driven trucks over the highway as regular civilian jobs. To this date, nobody has taken our

advice to purchase scales to ensure our safety and the safety of others we may drive by on our way to the drop

locations.
