Who are our services for?
Our services are predominantly targeted at adults
requiring domiciliary care, reablement, respite
and independent living support.
However, we can provide a wide range of services, to
people with a learning disability, a physical disability
or mental health support needs, people in extra care
schemes and supported living accommodation.
We also offer support to Service Users with complex
health needs and people living with dementia.
Our aimOur aim is to support people so they enjoy life as
independently as possible. Our Care Assistants/
Support Workers are trained to enable rather than
create dependence. We help people create their own
plans to measure progress over time and work
towards improving quality of life according to what’s
important for the individual. All of our Care Assistants
and Support Workers are routinely provided and/or
recruited according to individual needs, preferences
and specified requirements.
At the Human Support Group we value our staff
and Service Users so much so we ensure that every
member of staff is signed up to complete our
comprehensive induction and training scheme.
We ensure that at all times, services are provided
which comply with the requirements of the:
n Care Quality Commission (Registration)
Regulations 2010. In particular those
regulations which refer to the arrangements
for the provision of personal care and staffing.
n Care and Social Services Inspectorate Wales -
the regulator for social care and social
services in Wales.
n Care Inspectorate Scotland - the independent
scrutiny and improvement body for care
services in Scotland.

Fred has needed quite intensive support overthe last two years following a stroke, coupledwith high cholesterol and blood pressure.He depended on two Care Assistants, fourtimes a day for meals, personal care and withgoing to bed.
Thanks to his courage, strength anddetermination Fred only needs one Care Assistant,morning and night, as he is able to get around hishome with a wheelchair and pushes himself tostand and walk with his frame.

At Homecare Support our mission is to make life a
little easier by extending that helping hand. It’s our
job to take the worry away and let our Service Users
get on doing what they do best – enjoying their
independence.
With our wide experience and local knowledge we
can help with the vital advice and guidance on
getting access to this much needed support. We
know our way around the care system, what financial
support and information is available and how to
make it work for the Service User.
We provide support in the following areas of care:
Personal care
Personal help and support with bathing, washing,
dressing and other personal care routines.
Meals
Preparing, cooking and serving meals in your own
home – as well as tackling the washing up!
Domestic assistance
Help with the every day aspects of life including
washing, cleaning, bed making, collection of
benefits, pensions and shopping.
Hospital discharge
Specialist care and support if a Service User is
recuperating from an operation or illness.
Holiday companions
We can also offer companionship and assistance
during a vacation, one of our holiday companions
can help Service Users with their personal care needs
throughout their holiday.
Respite care
Care for Carers – everything from just an hour to
full day or weekend cover, providing a vital lifeline
to those who need a welcome break with peace
of mind.
Rapid response
Urgent support needed at short notice?
Our rapid response service provides much needed
support until a longer-term care solution is found.
Palliative care
Highly specialist palliative care and support at a
critical and sensitive time.
All in all we offer a fully personalised service tailored
to the individuals support needs.

Hilda recently suffered a stroke and neededsupport to ensure she could remain living inher own home. With the help of our SupportWorkers she is now well on the way to recoveryand returning to a level of independence shefeels comfortable with living in her own home.

The aim of reablement is to provide the support a
person needs to readjust to living at home, for a
period of up to 6 weeks, and enables them to live a
confident, independent lifestyle with as little support
as necessary when the reablement period ends.
HSG Reable provides help and assistance to
individuals when they leave hospital or respite care
after an injury or illness, helping them regain their
independence and stay safe during their recovery.
To make sure we provide a service that is meaningful
and purposeful to the individual, we assist them in
setting their own personal goals for their care, we
help the Service User achieve them through a
reablement plan that is individually tailored for
them, and reduces the amount of support they
need over time as their health improves.
We provide our services 365 days a year, 7 days a
week from 7am-11pm, so we can provide the service
a Service Users needs, when and where they need it.
Our Service Users are referred by Local Authorities,
and are individuals who have been highlighted to
have the potential to improve and regain their
independence.
The focus of reablement is on restoring independentfunctioning rather than resolving health care issues.The objective is to help people relearn how to dothings for themselves rather than the conventionalhome care approach of doing things for people.Reablement appears to be welcomed by peoplereceiving the service, and represents an investmentthat may produce savings.
Evidence from research and practice has indicated,there is great potential of reablement improvingpeople’s outcomes and delivering a cost-effectiveservice across health and social care.

A recent article from the BBC(http://www.bbc.co.uk/news/health-19560827)has highlighted findings from the British Geriatricssociety who stated that current NHS and councilintermediate care providers are only meeting half ofthe demand due to bottlenecking which is causingdelays in discharge. HSG Reable’s flexible service isdesigned and focused to provide an answer to thesechallenges in social care provisioning.

Ad-hoc placements

Assurance and risk management
We require all of our staff to undergo a Disclosure
and Barring Service (DBS) check which is reviewed

requirement. We are fully compliant with all new
legislation and also link to the Home Office to
ensure all staff have the right to work in the UK
as well as being trained in the identification of
fraud documentation.
Added value
A 24 hour 365 days per year responsive out of hours
service. Once the office closes at 6pm, we have a
team of consultants who work between 6am – 11pm
Monday to Friday and from 7am – 11pm at the
weekend. Our office based consultants are on back
up, on call. This is an added value service unique

With over 5 million older people living on their own,
and 50% citing television as their main source of
companionship, loneliness is ever-present within our
communities.
Cyril Flint, an elderly pensioner living on his own in the
Trafford area of Manchester, was a classic example of one
such individual. Having spent twenty lonely Christmas
holidays on his own, since his wife had passed away,
Cyril’s story not only evoked a wave of compassion
within the community, but it also made us want to do
something to help.
Setting up an informal befriending service, designed to
prevent social isolation seemed like the perfect solution.
Since it’s creation the service has grown throughout the
Manchester area and we now have an increasing number
of active volunteers throughout the communities of
Manchester and Salford.
